LUMBAR MYOFASCIAL PAIN SYNDROM

CLINICAL SYNDROME The muscles of the back work together as a functional unit to stabilize and allow coordinated movement of the low back and allow one to maintain an upright position. Trauma to an individual muscle can result in dysfunction of the entire functional unit. The rhomboids, latissimus dorsi, iliocostalis quadratus lumborum, multifidus, and psoas muscles are frequent sites of myofascial pain syndrome. The points of origin and attachments of these muscles are particularly susceptible to trauma and the subsequent development of myofascial trigger points. Injection of these trigger points serves as a diagnostic maneuver and a therapeutic maneuver. The muscles of the back are particularly susceptible to the development of myofascial pain syndrome. Flexion/ extension injuries to the back or repeated microtrauma secondary to improper lifting and bending may result in the development of myofascial pain in the muscles of the back. Myofascial pain syndrome is a chronic pain syndrome that affects a focal or regional portion of the body. The sine qua non of myofascial pain syndrome is the finding of myofascial trigger points on physical examination. Although these trigger points are generally localized to the regional part of the body affected, the pain of myofascial pain syndrome often is referred to other areas. This refered pain often is misdiagnosed or attributed to other organ systems, leading to extensive evaluations and ineffective treatment. Patients with myofascial pain syndrome involving the muscles of the low back often have referred pain into the hips, sacroiliac joint, and buttocks. SIGN AND SYMPTOMS The trigger point is the pathognomonic lesion of myofascial pain and is thought to be the result of microtrauma to the affected muscles. This pathological lesion is characterized by a local point of exquisite tenderness in affected musles. Mechanical stimulation of the trigger point by palpation or streching produces not only intense local pain, but also referred pain. In addition to this local and referred pain, there often is an involuntary withdrawal of the stimulated muscle that is called a jump sign. This jump sign also in characteristic of myofascial pain syndrome. Taut bands of muscle fibers often are identified when myofascial trigger points are palpated. Despite this consistent physical finding in patients who suffer from myofascial pain syndrome, the pathophysiology of the myofascial trigger point remains elusive, although many theories have been advanced. Common to all of these theories is the belief that trigger points are the result of microtrauma to the affected muscle. This microtrauma many occur as a single injury to the affected muscle or may occur as the result of repetitive microtrauma or chronic deconditioning of the agonist and antagonist muscle unit.

In addition to muscle trauma, a variety of other factors seem to predispose the patient to develop myofascial pain syndrome. The weekend athlete who subjects his or her body to unaccustomed physical activity often develops myofascial pain syndrome. Poor posture while sitting at a computer keyboard or while watching television also has been implicated as a predisposing factor to the development of myofascial pain syndrome. Previous injuries may result in abnormal muscle function and predispose to the subsequent development of myofascial pain syndrome. All of these predisposing factors may be intensified if the patient also suffers from poor nutritional status or coexisting psychological or behavioral abnormalities, including chronic stress and depression. The muscles of the low back seen to be particularly susceptible to stress-induced myofascial pain syndrome. Stiffness and fatigue often coexist with the pain of myofascial pain syndrome, increasing the functional disability associated with this diease and complicating its treatment. Myofascial pain syndrome my occur s a primary disease state or may occur in conjunction with other painful conditions, including radiculopathy and chronic regional pain syndromes. Psychological or behavioral abnormalities, including depression, frequently coexist with the muscle abnormalities associated with myofascial pain syndrome. Treatment of these psychological and behavioral abnormalities must be an integral part of any succesful treatment plan for myofascial pain syndrome. TESTING There is no spesific test for lumbar myofascial pain syndrome. Testing is aimed primarily at identifying occult pathology or other diseases that may mimic myofascial pain syndrome ( see differential diagnosis). Plain radiographs help delineate bony abnormality of the lumbar spine, including arthritis, fracture, congenital abnormalities (e.g trefoil spinal canal), and tumor. All patients with the recent onset of myofascial pain syndrome should undergo magnetic resonance imaging(MRI) of the lumbar spine to rule out occult pathology. Screening laboratory tests consisting of complete blood count, erythrocyte sedimentatiion rate, antinuclear antibody testing, and automated blood chemistry testing should be performed to rule out occult inflammatory arthritis, infection, and tumor. DIFFERENTIAL DIAGNOSIS Lumbar myofascuak pain syndrome is a clinical diagnosis of exclusion that is supported by a combination of clinical history, physical examination, radiography, and MRI. Pain syndromes that may mimic lumbar myofascial pain syndrome include lumbar strain; inflammatory arthritis; and disorders of the lumbar spinal cord, roots, plexus, and nerves. Congenital abnormalities, such as arteriovenous malformations and trefoil spinal canal, and spondylolisthesis also may mimic the clinical presentation of lumbar myofascial pain syndrome. TREATMENT Lumbar myofascial pain syndrome is best treated with a multimodality approach. Physical therapy, including correction of functional abnormalities ( e.g poor posture, improper chair or

computer height) and the use of heat modalities and deep sedative massage, combined with nonsteroidal anti inflammatory drugs (NSAIDs) and skeletal muscle relaxants represents a reasonable starting point. If these treatments fail to provide rapid symptomatic relief, local trigger point injection of anesthetic and steroid into the myofascial trigger point area is a reasonable next step. Underlying diffuse muscle pain and sleep disturbance and depression are best treated with a tricyclic antidepressant compound, such as nortriptyline, which can be started at a single bedtime dose of 25 mg. When performing trigger point injections, careful preparation of the patient before injection helps to optimize results. Trigger point injections are directed at the primary trigger point, rather than the area of referred pain. It should be explained to the patient that the goal of trigger point injection is to block the trigger of the persistent pain and, it is hoped, provide long-lasting relief. It is important the the patient understand that with most patients who suffer from myofascial pain syndrome, more than one treatment modality is required to provide optimal pain relief. The use of the prone or lateral position when identifying and marking trigger points and when performing the actual trigger point injection helps decrease the incidence of vasovagal reactions. The skin overlying the trigger point to be injected should always be prepared with antiseptic solution before injection to avoid infection. After the goals of trigger points injection are explained to the patient and proper preparation of the patient has been carried out, the trigger point to be injected is reidentified by palpation with a sterile gloved finger. A syringe containing 10mL of 0,25% preservative-free bupivacaine and 40 mg of methylprednisolone to be injected is attached to a 25-gauge needle of a length adequate to reach the trigger point. For the deeper muscles of posture in the low back, a 3 -inch needle is required. A volume of 0.5 to 1 mL of solution is injected into each trigger point. The patient should be informed that a series of two to five treatment sessions may be required to abolish the trigger point completely. SIDE EFFECTS AND COMPLICATIONS The proximity to the spinal cord and exiting nerve roots makes it impreative that this procedure be done only by clinicians well versed in the regional anatomy and experienced in performing interventional pain management techniques. Many patients complain of a transient increase in pain after injection of trigger points. If long needles are used, pneumothorax or demage to the retroperitoneal organs, including the kidneys, also may occur. Clinical Pearls Trigger point injections are an extremely safe procedure if careful attention is paid to the clinically relevant anatomy in the areas to be injected. Care must be taken to use sterile technique to avoid infection and universal precautions to avoid risk to the operator. Most side effects of trigger point injection are related to needle-induced trauma to the injection site and underlying tissues. The incidence of ecchymosis and hematoma formation can be decreased if pressure is placed on the injection site immediately after trigger point injection. The avoidance of overly long needles helps decrease the incidence of trauma to underlying

structures. Spesial care must be taken to avoid pneumothorax when injecting trigger points in proximity to the underlying pleural space. Antidepressant compounds are the primary pharmacological treatment for myofascial pain syndrome. Tricyclic antidepressants are thought to be more effective than selective serotonin reuptake inhibitors in the treatment of this painful condition. The precise mechanism of action of antidepressant compounds in the treatment of myofascial pain syndrome is unknown. Some investigators believe that the primary effect of this class of drugs is to treat the underlying depression that is present in many patients who suffer from myofascial pain syndrome. Drugs such as amitriptyline and nortriptyline represent good first choices and should be given as a single bedtime dose, starting with 10 to 25 and titrating upward as side effects allow.

SINDROM NYERI MYOFASCIAL PADA LUMBAL

KLINIS Otot-otot punggung bekerja bersama sebagai unit fungsional untuk menstabilkan dan memungkinkan gerakan terkoordinasi pada punggung bagian bawah dan memungkinkan seseorang untuk mempertahankan posisi tegak. Trauma otot dapat mengakibatkan disfungsi seluruh unit fungsional. Rhomboids, otot latissimus dorsi, quadratus iliocostalis lumborum, multifidus, dan otot psoas adalah tempat yang paling sering sindrom nyeri myofascial. Titiktitik yang berasal dari otot ini sangat rentan terhadap trauma dan perkembangan selanjutnya yang memicu nyeri myofascial. Injeksi pada triger point berfungsi sebagai manuver diagnostik dan manuver terapeutik.

Otot-otot punggung sangat rentan terhadap perkembangan sindrom nyeri myofascial. Fleksi / ekstensi pada cedera punggung yang berulang atau microtrauma sekunder seperti mengangkat yang tidak benar dan membungkuk dapat mengakibatka sakit myofascial pada otot belakang. Sindrom sakit myofascial adalah sindrom nyeri kronis yang mempengaruhi sebagian fokal atau daerah tubuh. The sine qua non merupakan pemeriksaan fisik yang ditemukan pada sindrom nyeri myofascial. Meskipun trigger point umumnya ditemukan dibeberapa bagian tubuh, rasa sakit dari sindrom nyeri myofascial sering terdapat di daerah lain. Rasa sakit ini sering salah didiagnosis atau dikaitkan dengan sistem organ lainnya, menyebabkan evaluasi yang luas dan pengobatan yang tidak efektif. Pasien dengan sindrom sakit myofascial sering melibatkan otot-otot punggung bagian bawah berupa nyeri ke bagian pinggul, sendi sacroiliac, dan bokong. TANDA dan GEJALA Trigger point adalah lesi pathognomonic dari rasa sakit myofascial dan dianggap sebagai hasil dari microtrauma ke otot-otot yang terkena. Lesi patologis ini dicirikan oleh sudut otot halus yang terkena. Rangsangan mekanik dari trigger point dengan palpasi atau meregangkan kulit menyebabkan nyeri lokal yang kuat. Selain rasa sakit lokal dan sebagaimana dimaksud, ada pula penarikan otot yang sengaja dirangsang yang disebut sebuah tanda melompat. Lompatan ini masuk juga dalam karakteristik sindrom nyeri myofascial Pita kencang dari serat otot seringkali teraba ketika trigger point myofascial dipalpasi. Meskipun temuan fisik sesuai dengan pasien yang menderita sindrom nyeri myofascial, patofisiologi trigger point myofascial masih sulit dipahami, meskipun banyak teori telah dikemukakan. Umumnya semua teori-teori beranggapan bahwa trigger point adalah hasil dari microtrauma ke otot yang terkena. Microtrauma ini banyak terjadi sebagai cedera tunggal pada otot yang terkena atau mungkin terjadi sebagai hasil dari microtrauma berulang atau kronis deconditioning dari unit otot agonis dan antagonis.

Selain trauma otot, terdapat berbagai faktor lain yang mempengaruhi pasien untuk mengalami sindrom sakit myofascial. Para olahragawan yang hanya melakukan aktivitas fisik setiap minggu atau jarang dapat mengakibatkan sindrom nyeri myofascial. Sikap tubuh yang buruk ketika duduk sambil mengetik keyboard komputer atau duduk saat menonton televisi juga telah terbukti terlibat sebagai faktor predisposisi terhadap perkembangan sindrom nyeri myofascial. Sebelumnya cedera dapat menyebabkan fungsi otot abnormal dan merupakan predisposisi perkembangan selanjutnya sindrom nyeri myofascial. Semua faktor predisposisi dapat meningkat jika pasien juga menderita status gizi buruk atau hidup dengan kelainan psikologis atau perilaku, termasuk stres kronis dan depresi. Otot-otot punggung bagian bawah terlihat sangat rentan terhadap stres akibat sindrom nyeri myofascial. Kekakuan dan kelelahan seringkali berhubungan dengan rasa sakit sindrom nyeri myofascial, yang dapat meningkatkan ketidakmampuan fungsional yang berhubungan dengan penyakit ini dan menyulitkan pengobatannya. Nyeri sindrom myofascial yang terjadi disini merupakan penyakit primer atau sekunder yang dapat bersamaan dengan kondisi lainnya, termasuk radiculopathy dan sindrom nyeri regional yang kronis. Kelainan psikologis atau perilaku, termasuk depresi, sering berhubungan dengan kelainan otot yang berhubungan dengan sindrom nyeri myofascial. Pengobatan pada kelainan psikologis dan perilaku harus menjadi bagian integral dari setiap pengobatan yang direncanakan untuk keberhasilan pada sindrom nyerimyofascial

PEMERIKSAAN Tidak ada tes spesifik untuk sindrom nyeri lumbal myofascial. Pengujian ditujukan terutama untuk mengidentifikasi okultisme patologi atau penyakit lain yang dapat menyerupai sindrom nyeri myofascial (lihat diagnosis diferensial). Foto polos membantu menggambarkan kelainan tulang dari tulang belakang lumbal, termasuk radang sendi, patah tulang, kelainan bawaan (misalnya trefoil tulang belakang kanal), dan tumor. Semua pasien dengan onset sindrom sakit myofascial harus menjalani magnetic resonance imaging (MRI) dari tulang belakang lumbar untuk menyingkirkan okultisme patologi. Skrining tes laboratorium yang terdiri dari darah lengkap, laju sedimentasi eritrosit, tes antibodi antinuklear, dan pengujian kimia darah otomatis harus dilakukan untuk menyingkirkan artritis inflamasi okultisme, infeksi, dan tumor. DIAGNOSISBANDING Sindrom nyeri myofascial lumbal adalah diagnosis klinis yang didukung oleh kombinasi dari gejala klinis, pemeriksaan fisik, radiografi, dan MRI. Sindrom nyeri yang mungkin menyerupai sindrom nyeri lumbal myofascial termasuk ketegangan lumbal, arthritis inflamasi, dan gangguan pada tulang belakang lumbal, radix, pleksus, dan saraf. Kelainan kongenital, seperti malformasi arteriovenosa dan kanal tulang belakang trefoil, dan spondylolisthesis juga dapat menyerupai presentasi klinis dari sindrom nyeri lumbal myofascial.

PENGOBATAN Sindrom nyeri lumbal myofascial paling baik diobati dengan pendekatan multimodalitas. Terapi fisik, termasuk perbaikan kelainan fungsional (misalnya posisi kursi, postur yang tidak benar atau ketinggian komputer) dan penggunaan modalitas panas dan pemijatan penenang, dikombinasikan dengan obat anti inflamasi nonsteroid (NSAID) dan relaksan otot rangka merupakan titik awal yang dilakukan. Jika perawatan ini gagal untuk memberikan bantuan yang cepat,injeksi anestesi lokal dan steroid ke daerah trigger point myofascial adalah langkah yang dilakukan selanjutnya. Jika nyeri otot menyebar dan menyebabkan gangguan tidur dan depresi paling baik diobati dengan senyawa antidepresan trisiklik, seperti nortriptyline, yang dapat dimulai dengan dosis tunggal 25 mg. Ketika melakukan suntikan trigger point, persiapan yang baik pada pasien sebelum injeksi membantu mengoptimalkan hasil. Tempat suntikan diarahkan terutama pada trigger point, bukan area yang sakit. Harus dijelaskan kepada pasien bahwa tujuan injeksi adalah untuk menghilangkan rasa sakit yang menetap dan diharapkan memberikan pengobatan jangka panjang. Penting bagi pasien yang menderita sindrom sakit myofascial untuk memahami, bahwa diperlukan lebih dari satu pengobatan untuk mendapatkan hasil yang optimal. Penggunaan posisi tengkurap atau miring ketika mengidentifikasi dan menandai trigger point dan saat melakukan suntikan sebenarnya membantu mengurangi kejadian reaksi vasovagal. Kulit pada tempat suntikan harus selalu diberikan larutan antiseptik sebelum injeksi untuk menghindari infeksi.

Setelah tujuan injeksi dijelaskan ke pasien dan persiapan yang baik pasien telah dilakukan, identifikasi ulang tempat suntikan oleh palpasi dengan sarung tangan steril. Sebuah jarum suntik yang berisi 10 ml 0,25% bupivakain dan 40 mg metilprednisolon harus disuntikkan menggunakan jarum berukuran 25 yang panjangnya cukup untuk mencapai trigger point. Untuk otot yang lebih dalam pada posisi belakang bawah, diperlukan jarum 3 inci. Sebuah volume 0,5 sampai 1 mL larutan disuntikkan ke setiap trigger point. Pasien harus diberitahu bahwa diperlukan pengobatan sejumlah dua atau lima kali untuk menghilangkan trigger point sepenuhnya.

EFEK SAMPING DAN KOMPLIKASI Kedekatan ke sumsum tulang belakang dan akar saraf yang keluar membuat impreative bahwa prosedur ini dilakukan hanya oleh dokter yang mengetahui anatomi regional dan berpengalaman dalam melakukan teknik manajemen intervensi nyeri. Banyak pasien mengeluh nyeri yang bertambah setelah dilakukan penyuntikan didaerah trigger point. Jika menggunakan jarum yang panjang, pneumotoraks atau masalah bagi organ retroperitoneal, termasuk ginjal, juga dapat terjadi.

KlinikMutiara Injeksi trigger point adalah prosedur yang sangat aman jika lebih memperhatikan anatomi klinis yang relevan pada tempat yang akan disuntikkan. Perawatan harus menggunakan teknik steril untuk menghindari infeksi dan kewaspadaan untuk menghindari risiko kepada operator. Kebanyakan efek samping injeksi trigger point berhubungan dengan jarum yang dapat menyebabkan trauma pada tempat suntikan dan jaringan di bawahnya. Insiden ecchymosis dan pembentukan hematoma dapat diturunkan jika segera menekan tempat suntikan setelah injeksi. Mekanisme yang tepat senyawa antidepresan dalam pengobatan sindrom nyeri myofascial tidak diketahui. Beberapa peneliti percaya bahwa efek utama dari kelas obat ini adalah untuk mengobati depresi yang terjadi pada banyak pasien yang menderita sindrom nyeri myofascial. Obat-obatan seperti amitriptilin dan nortriptyline merupakan pilihan pertama yang baik dan harus diberikan sebagai dosis tunggal sebelum tidur, mulai dengan 10 sampai 25 dan titrasi kenaikan sebagai efek samping memungkinkan.
