Fixed Fire Fighting System

Deficiencies with Action Code 30 (means 'Ground for Detention') Foam Monitors on catwalk seized and partly broken/damaged.(Tanker only) Deficiencies with Action Code 17 (means 'To rectify deficiencies before departure') CO2 system power failure alarm not working when activated - defective system. Fixed fire extinguishing system not serviced/checked as required on a yearly basis. Fixed fire extinguishers installation for CO2. Validity date of the certificate was expired. Some brackets of the fixed fire fighting system were not tightened and the bottles were not properly secured. Sand box found without shovel. Deficiencies with Action Code 16 (means 'To rectify deficiencies within 14 days') Foam monitor handle found broken and not freely moving. CO2 room pressure gauge on discharge line found defective. Fire Detection System

Malfunction and fault of fire detection panel - many zone found isolated. (under repair) Some areas in fire detector panel were isolated. Emergency Fire Pump

Fire & Gastight Doors

Deficiencies with Action Code 30 (means 'Ground for Detention') Accommodation fire doors (including entrance to E/R) found not closed. E/R cannot be closed properly in case of fire (Electrical cables were passing through the deck opening & Eng. Control room door). Deficiencies with Action Code 17 (means 'To rectify deficiencies before departure') Fire doors were not self closing (not closed automaticaly) and not closing properly. Various fire doors were lashed open and numerous self-close devices of fire doors were not in good condition. Self-closing fire doors found with hold back hook. Back hooks to be removed and "Keep Door Close" sign is recommended. Self closing device for fire door room found missing. Self closing device of the fire door was taken off. Self closing device on (Class A) fire door to E/R on upper deck was not provided. Deficiencies with Action Code 16 (means 'To rectify deficiencies within 14 days') Fire proof door in accommodation was unable to close tightly. Engine room fire door closing device found broken. Steering gear room entrance door from deck, the fire door seal found defective. Deficiencies with Action Code 99 (means 'Other') Galleys doors (Class A fire division) found not working and the other door was badly corroded at bottom. Ensure complete closure is possible for all fire doors in stair towers, accommodation space and machinery space access (Solas 74 amended II-2/46-77). A-O class fire doors between E/room and steering room differed from fire control plan (used combustible gasket).

Ventilators, Fire Dampers & Flaps

Deficiencies with Action Code 30 (means 'Ground for Detention') Engine room ventilation fan dampers found frozen and remote air closing inoperative. Machinery spaces (means of control) - inoperative. Funnel fire damper stucked/inoperative. Deficiencies with Action Code 17 (means 'To rectify deficiencies before departure') Fire dampers found seized and unable to close - inoperative and did not close properly from remote location. Engine room ventilation damper operating handle found broken. Fire dampers tight, stiff and not sealing properly. Fire damper, the open/close position was painted over. All fire flaps to be marked according to their function, flaps have to be regularly greased and moved to keep them working smoothly. No "Open / Close" marks for part of ventilators. Deficiencies with Action Code 18 (means 'To rectify deficiencies within 3 months') Cargo hold mushroom vents mechanical fan safety grills secure found wasted/broken. Deficiencies with Action Code 16 (means 'To rectify deficiencies within 14 days') CO2 room ventilator cover, securing bolts found missing. Several fire dampers & ventilation flaps to be marked. Fire/Ventilation dampers found with no marking "open" & "close". Deficiencies with Action Code 15 (means 'To rectify deficiencies at next port') Fire fighting measures: Ventilation/fire dampers, quick closing devices means of control, quick closing valves on M/E LO settling tank found not operating.
