Underdog Training Services Agreement (specifically regarding Boarding services) I/ We Katherine and Michael Shimmins (hereinafter referred to as the

client) have voluntarily employed Underdog Training & Behaviour Consulting (herein after referred to as Consultant) to assist me in the boarding and care of my dog (s) Daisy and Barclay. Professional Fees: Professional fees shall be $160 for 1 nights boarding at 393 Corrigan Rd Keysborough under the care of Underdog trainers Tamara and Stuart. Fees are due at the beginning of the boarding period unless otherwise agreed upon prior to the commencement of said period. Pick ups after 12pm midday will incur full boarding charge for that day. Payment can be made to the following account: Tamara Jackman BSB 633 000 Acc # 128 570 611 Bendigo Bank- please use your dogs name as a reference. I would like to drop off my dog at 8.15am on the 20/09/2012 (enter date) and collect my dog at 4.00pm on the 21/09/2012 (enter date) Please note these times should be pre-arranged with Underdog Training consultants. PLEASE NOTE- Once pick up and drop off times have been decided upon, notice is required if adjustments need to be made beyond 5-10 minutes. Please be aware that your pick up and drop off time influences our schedule and availability to complete other work as well as the schedule of the dogs residing at the property on that day. You MUST advise us if you will be late or need to change the scheduled time. Description of Services: I understand that the consultant will care for my pet during the boarding period including feeding, watering, play time, basic medication administration and basic grooming. I request that my dog be walked 1 times at the additional fee of $10 per walk. (this is not compulsory and you may enter 0 if you do not require the walking service) I request that my dog receive a bath before departure NO This service is POA depending on dogs breed, size and coat length and is to be paid by the client at the beginning of the boarding period. Client acknowledges that dog minding and dog walking may be an activity in which damage or injury to dogs and/ or persons may occur. Client will assume full financial and moral responsibility for the action/s of their dog now and in the future. This will include reimbursement in the event that the dog causes damage to fixed

property such as secure fencing, doors and walls and outdoor fixtures including decking and well established plants. If my dog is sick or injured, I give permission for the consultant to take my dog to a
veterinary clinic at my expense. My nominated veterinary practioner is Monash Veterinary Clinic I understand that the consultant may take my dog to a closer practitioner if required. I/ We the client agree that I, my/our heirs assignees and legal representatives will not make claim against, sue or attach the property of Consultant, their family, acquaintances or any other person acting on their behalf for injury/ damage done by ours dog (s) resulting from action or negligence, however caused by Consultant or their agents. I/ We Client forever release Consultant from any and all liability and demands which I/ We Client may have or may hereafter have for injury or damage to or by our dogs and assume all risks thereof.
