STANDARD OPERATING PROCEDURES

SOP FOR HANDLING EMERGENCY SITUATION The basic objective of this Standard Operating Procedure is to lay down certain guidelines for the Apmt Staff on duty to act in case of any emergency situation arising inside CFS due to accidents. The respective departmental head in close coordination will handle all emergencies with Manager Industrial Relations & Manager Safety & Security. For the purpose of handling emergency situations the CFS premises has been divided into small areas as given below keeping in view the functional responsibilities. An officer has been appointed to handle the situation as given against each area given below in the table. : 3: The other important telephone numbers are given at the end of this SOP (Annexure B). The place where the First Aid Box is kept is also listed at the end. (Annexure C) Types of situations Different types of emergency situations, which could arise during the course of operations in CFS, are listed below. 1. 2. 3. 4. 5. On occurrence of anyone of the above, the Officer/ person on site will immediately call the concerned officer nominated for dealing with the emergency. The Responsible Officer (hereafter RO) will rush to the site/spot & take actions as per steps given below.

No injury to person but damage to property (vehicle, containers, buildings etc.) 1. Call Duty Security Supervisor, hereafter DSS, (extn.3040/3050) & place a guard at the place of accident to ensure that no evidence is removed. 2. Inform Departmental Head and apprise him of the accident with details (as given at the end) who would in turn inform CFS Management/Customer depending upon the gravity of the incident/accident. DSS also be instructed to immediately inform Manager Safety & Security with details. 3. If third party vehicle is involved, have the vehicle documents such as Registration Papers, Road permit, Insurance Papers etc. and the driving license confiscated/seized through DSS. Instruct him to hand over the same to Manager Safety & Security, the next day, pending investigation. 4. Ensure that the evidences are not removed till such time a proper investigation is carried out and police FIR filed. If the work is going to be hampered due to nonremoval of evidence, special permission may be obtained from the Departmental Head to this effect or from Local Police as the case may be. 5. Ring up Jessai police station and lodge a verbal First Information Report (FIR) about the accident. The Departmental Head may be consulted for the exact Information to be provided to the Police in the FIR. Police person may be informed that appropriate authorities of Apmt CFS will file a detailed FIR at the earliest. 6. If third party property is involved, the accident site and all evidences in the site may be photographed through a professional photographer at the earliest opportunity in consultation with the Departmental head. /4

: 4: 7. Forward a written report on the incident to the Departmental Head at the first available opportunity with details. The format for report is given at annexure A. Minor injury to person but no damage to property 1. Take the injured person to the nearest place where First Aid Box is kept (Refer Annexure C). Administer him/her appropriate first aid depending on the type of injury. If the Concerned Officer is not trained on First Aid, the advice of other officers on duty in CFS may be taken. 2. Call Duty Security Supervisor (DSS) and ask him to depute a vehicle to take the injured to JNPT Hospital at JNPT Township. Also ask him to arrange for a Special Security Supervisor (SSS) to take the injured to Hospital. Note that the DSS is not sent out of CFS on any account. No Apmt employee is required to accompany the injured to Hospital. 3. In case of non availability of any contract vehicle, request/call one of the Apmt Officer on duty who is in possession of valid driving license and who knows driving to take the injured to hospital in the Apmt Maruti 800 located in CFS. The key is with security. The names of Apmt Officers who know driving and in possession of valid driving license are given in Annexure D. 4. Draw an amount of Rs 750/- from Emergency Fund from Manager - Finance or his staff during office hours and from Buffer Yard Officer on duty at other times. Hand over the cash to the SSS and obtain a manual receipt from him. Advice SSS to bear the cost of treatment of the injured from the emergency fund. 5. Advice SSS to obtain receipt for all the expenses incurred in treatment of the injured from the Hospital and submit the same after he is back from Hospital. 6. Dispatch the injured along with SSS and Rs 750/- to JNPT Hospital in the arranged vehicle. 7. If PFS/OFS/Transporter employee is injured in the accident, their respective supervisors on duty will get involved in treatment of their individuals. One of the Supervisors will accompany the injured to all the places. 8. Instruct SSS to bring the injured person back to CFS after the treatment along with doctors prescription. 9. Once the SSS is back find out from him whether the Hospital authorities made it a Medico Legal Case or not. If so, it becomes mandatory on our part to file an FIR with the same Police Station as that of JNPT Hospital. The necessary guidance to file an FIR is taken from the Departmental Head. A verbal FIR will suffice initially; later on, to be followed up with a written FIR. Otherwise, it is appropriate to file FIR with Jessai Police Station (Telephone No.27242452) for any claims related issues. 10. Give rest to the injured or send him back to his home depending upon the general condition and recommendations of the doctor. 11. Forward a written report on the incident to the Departmental Head at the first available opportunity with details. The format for report is given at Annexure A. /5

: 5: Minor injury to person and also damage to property 1. Call DSS (extn.3040/3050) & place a guard at the place of accident to ensure that no evidence is removed. 2. Take the injured person to the nearest place where First Aid Box has been kept (Refer Annexure C). Administer him/her appropriate first aid depending on the type of injury. If the Concerned Officer is not trained on First Aid, the advice of other officers on duty in CFS may be taken. 3. Inform Departmental Head and apprise him of the accident with details (as given at the end) who would in turn inform CFS Management/Customer depending upon the gravity of the incident/accident. Security Supervisor is instructed to immediately inform Manager Safety & Security with details. 4. If third party vehicle is involved, have vehicle documents such as Registration Papers, Road permit, Insurance Papers etc. and the driving license of the driver confiscated/seized through the Security Supervisor. Instruct the Security Supervisor to hand over same to Manager Safety & Security, next day, pending investigation 5. : 6: 13. Once the SSS is back find out from him whether the Hospital authorities informed the Police Station or not. If yes, it becomes mandatory to file an FIR with the same Police Station as that of JNPT Hospital. The necessary guidance to file an FIR is taken from the Departmental Head. Otherwise, it is appropriate to file FIR with Jessai Police Station (Telephone No.27242452) for any claims related issues. 14. Severe injury to person but no damage to property 1. Call Duty Security Supervisor i.e. DSS by quickest means (Tel: 7040/7050) with the First Aid Kit, Stretcher and two security guards to the accident spot. Place one Security guard at the place of accident to ensure that no evidence is removed. 2. Inform the concerned Deptl. Head about the accident in brief. Instruct Security Supervisor to inform Manager Safety & Security about the same. 3. If the accident has taken place during the office hours ring up Manager Finance (Tel: 3103/3104) or any of his staff to send across Rs 10,000/- to him with an IOU immediately. Otherwise, ring up Buffer yard Officer (Tel: 3152) on duty to keep Rs.10, 000/- from Emergency Fund ready along with IOU to be picked up on the way to Hospital. 4. Call the officer concerned to the spot to take the injured to Hospital and also the OFS/PFS supervisor if their employee injured. 5. Instruct DSS to arrange for a contract vehicle to carry the injured. In case of non availability of any contract vehicle, request/call one of the Apmt Officer on duty who is in possession of valid driving license and who knows driving to take evacuate the injured to hospital in the Apmt vehicle located in CFS. The key is with security. The names of Apmt Officers who know driving and in possession of valid driving license are given in Annexure D. 6. In case Apmt Vehicle is not available or having problems, ring up JNPT Hospital Causality Ward and seek the services of their ambulance. Give proper directions to the Hospital Staff for dispatch of the ambulance. Also instruct Security at Gates to guide the ambulance to the accident spot inside CFS without any delay through DSS. 7. Place the injured on the stretcher with the help of Security Staff and load him in the vehicle/ambulance. Also place the First Aid Box in the vehicle. Advice the officer taking the injured to Hospital to give first aid enroute wherever possible. It will be his responsibility to bring it back. Place one of the OFS/PFS supervisors also in the vehicle if one their employee is injured. /7

: 7: 8. The officer taking the injured to JNPT Township Hospital will be handed over Rs.10, 000/- received from Manager Finance or his staff. Other times, he will be advised to go via Buffer Gate collecting Rs. 10,000/- from the Emergency Fund held with Buffer Gate Officer. 9. When Yard officer takes the injured to Hospital (during off duty hours), the Reefer Yard Officer on duty will look after his duties till Yard Officer is back or suitable replacement reports for duty. 10. When Reefer Yard Officer takes the injured to the Hospital during off duty hours, Electrician will perform his duties on duty under the guidance of Container IN /OUT Gate officer till such time another Reefer Yard officer is arranged by either of them. 11. Duties of the officer taking the Injured to Hospital On the way to JNPT, collect Rs 10,000/- emergency fund from the Buffer Gate Officer on duty if it is other than office hours. Bear all the expenditure including any deposit sought by the JNPT authorities from Rs 10,000/- emergency fund. Obtain proper bills for the expenditure wherever incurred. Be in touch with the RO on the developments at Hospital through PCO. Co-operate with the hospital authorities if they decide to place the injured on the medico legal case (MLC). Do not delay evacuation of the injured to any hospital in Mumbai /Navi Mumbai if advised so by the Doctor on duty at JNPT Hospital. If need be hire the JNPT ambulance to evacuate the injured. The expenditure on the same is borne from Rs 10,000/- emergency fund. Clear all bills of JNPT Hospital then and their if the Hospital authorities demand, in order to avoid any delay in evacuation. If the Out Patient Department (OPD) of any of the Hospital in Mumbai/Navi Mumbai where the injured has been taken recommends emergency operation, accord consent to go ahead with the same. If the Hospital authorities require any deposit, pay from emergency fund. Keep aside Rs 2,000/- for miscellaneous expenses and deposit balance with Hospital authorities for operation. If more money is required, assure the Hospital authorities that it would be given at the earliest. Obtain receipt for the deposit. If any blood is sought for the operation, contact nearest blood bank by taking address from Hospital authorities. Obtain the same on payment from blood bank and deliver it to the hospital authorities. Use contract vehicle or taxi for this purpose. Pay the fare from the emergency fund held. Be there in the hospital till operation is over. Come back to Apmt only when the patient goes to the ward or relieved by a suitable person from Apmt. When in Hospital, keep in touch with Apmt and post the RO with the latest developments. On coming back submit account for Rs 10,000/- with the bills and give a feed back to the departmental head. /8

: 8: 12. The RO after dispatch of injured to hospital will continue to perform following. 13. If third party vehicle is involved, have the vehicle documents such as Registration Papers, Road permit, Insurance Papers etc. and the driving license confiscated through the DSS. Instruct him to hand over the same to Manager S&S. 14. Ensure that the evidences are not disturbed till police or the appropriate authorities carry out such time a proper investigation. If the work is going to be hampered due to non-removal of evidence, special permission may be obtained from the Departmental Head to this effect over phone or local police as the case may be. 15. Ring up Jessai police station (Ph: 27242452) and lodge a verbal First Information Report (FIR) on the accident and request them to visit the accident spot. Seek the advice of the Departmental Head over phone before filing FIR with the local police. 16. After spot visit by the Police authorities, direct Security Officer to Jassai Police Station to obtain the FIR number from them and also a copy of the FIR. 17. If PFS/OFS/Transporter employee is injured in the accident, their respective supervisor/staff will inform the family of the injured about the accident. 18. In case of injury to others, inform the next of the kin of the injured over telephone or by sending a Security Guard to his house, if address is known from any other source. Otherwise, inform the police accordingly that the address of the injured is not known and that they take action accordingly. 17. Forward a written report on the incident to the Departmental Head at the first available opportunity with details. The format for report is given at Annexure A. 18. The accident site and all evidences in the site may be photographed through a professional photographer at the earliest opportunity in consultation with the Departmental head. Severe injury to person and also damage to property 1. As far as injury to the person is concerned, the steps as given above under Severe injury to person and no damage to property are followed. 2. As far as the damage to property is concerned, the steps given above under No injury to person and damage to property (vehicle, containers, buildings etc.) Be followed. General Instructions: 1. If the accident takes place during office hours, the Head of the departments, on coming to know of the same will immediately gather maximum information. They will assemble in the Conference Room at the time given by the Manager Safety & Security, in consultation with GM or the senior most Managers in the CFS, to discuss the further course of action and guidance. 2. In case of accident during non office hours, one of the Departmental Head preferably one of the Managers/Sr Managers will make himself available at the earliest opportunity at accident spot to guide the Apmt Staff. Managers may interact within themselves to decide as to who would come to the accident spot without any delay. /9
