Form II - Grant of registration
Rule 18 Certificate in the form mentioning
certificate
conditions stipulated in the rules.
If there is any change in the
particulars specified in the RC, it
has to be intimated along with
Rule 18 RC Amendment
original RC within 30 days from the
date of change specifying the
reasons. RC has to be amended.
Certificate has to be issued by the
Form V - Form of Certificate principal employer. Contractor has
Rule 21
by Principal Employer to submit the certificate along with
Form IV.
