Procedure to Register Homebirth in JPN Just noting what members suggested from recent comments about homebirth registration

By Fazirah Abd - My advise, call first before u go. Make sure u get the right document they need. In my case, went to putrajaya jpn, they dont accept the green form without endorsement frm medical personnel eg: kk nurse, doctor etc. Doesnt matter who cut the cord whtsoever, but your birth must be "verified" by those ppl. Whatt la!

Salinan buku checkup ibu (the government version's red book)- front cover- m/s pertama pemeriksaan kandungan (m/s 14-15)- kemasukan ke hospital, perihal kelahiran bayu (m/s 2829)- perihal post-natal (m/s 30-31) 4. Laporan polis5. Surat pengesahan kelahiran oleh pakar pediatrik

From Tria Aziz: (Puan Rumina is no longer at JPN Maju Junction. my husband asked for her on the phone to ask abt Shamimi's registration n he was told she doesn't work there anymore.) So hubby just went to the JPN at Kota Damansara. They asked for: 1. Ante-natal checkup book, 2. Copies of ic, 3. Police report, 4. Marriage cert, 5. surat akuan bidan/midwife oath (my husband was declared the bidan coz he cut the cord), 6. surat akuan 2 saksi/2 witnesses oath (so we named Nadine n my friend Stephanie the saksi)

From yat/fazirah - as of 2/11/2011 - En Azizul told Fazirah they just need police report, antenatal card, parents' IC & Marriage cert/ID (they don't require the statement at the Commissioner of Oath as that is only reserved for 'special circumstances')

To report birth to JPN within 42 days from birth date. Afiza suggested home birthers to go to JPN Maju Junction branch (Maju Junction 7th floor). Need to bring: 1. police report (not necessarily within 24 hours though the hospital may think so, like reporting car accident, he he - Zas) 2. commissioner of oath statement (there is one in Maju Junction building) 3. your ante-natal apponitment/checkup card and gynae's letter (gynae's letter is acttually ur gynae's appointment card.) 4. baby's check-up card (optional but nice to have - Wai Han) 5. marriage registration card 6. mother's IC (identity card) - Natasha said

Note: The JPN Maju Junction (tel: 03-26925044). Call, ask about the procedure to register for homebirth and note the name of the officer who inform you about the procedure. Then go there and ask to see the officer. Jpn maju junction is on the 7th floor, so drive up all the way to 7th floor and park there. Jpn office is near the 7th floor car park, so its very convinient. Also Jpn maju junction is open on sundays till 2 pm.
