GENERAL DESCRIPTION 57,930 Total Existing Square Footage 1966,1993 Building Dates K-6 Grades 617 Current Enrollment 35 Teaching Stations 20 Site Acreage Maddux Elementary School, which is not on the National Register of Historic Buildings, is a 1966 2-story, 57,930 square foot brick school building located in a suburban residential setting. The existing facility does not use modular buildings. The structure of the original building is a steel frame and nonbearing masonry wall construction except for the Gymnasium, which is brick veneer on masonry bearing wall. The floor system consists primarily of concrete slab on grade with precast concrete used over the basement area. The roof structure is metal deck and steel joists except for the Gymnasium, which has tectum deck. The structure of the 1994 addition contains bearing masonry exterior and interior wall construction. The floor system consists of concrete slab on grade. The roof structure is metal deck and steel joists. The roofing system over the original building is EPDM installed in phases over the past 17 years. The roofing system of the addition is built-up with gravel ballast installed in 1994. The ventilation system of the building is inadequate to meet the needs of the users. The classrooms are slightly smaller than the current standards established by the State of Ohio. Physical Education and Student Dining spaces consist of one Gymnasium and separate Student Dining. The electrical system for the facility is inadequate. The facility is equipped with a security system, but it lacks some of the OSFC standard features. The building has a non-compliant fire alarm system. The facility is not equipped with an automated fire suppression system. The building is reported to contain asbestos. The overall building is not compliant with ADA accessibility requirements. The school is located on a 20 acre site. The property is not fenced for security. Access onto the site is unrestricted. Site circulation is fair. There is space for school buses to load and unload on the site, which is shared with parking. Parking for staff, visitors and community events is adequate.

A. Heating System
Description:
The existing system for the overall facility is a gas fired hot water system, installed in 1966, and is in fair condition. The two hot water boilers, manufactured by International, were installed in 1966 (1 rebuilt 4 years ago) and are in fair condition. Heating water is distributed to terminal units consisting of unit ventilators, cabinet heaters, and air handlers. The terminal equipment was installed in 1966 and is in fair condition. The 1993 addition is an extension of the original hot water unit ventilator system. The system does not comply with the 15 CFM per person fresh air requirements of the Ohio Building Code mechanical code and Ohio School Design Manual. The pneumatic system temperature controls were installed in 1966 and are in fair condition. The system does not feature individual temperature controls in all spaces required by the OSDM. Louvered interior doors are a violation of the Ohio Building Code, and interior doors in this facility are louvered to facilitate corridor utilization as return air plenums. Louvered doors are replaced in Items J and O. The existing system is not ducted. The overall heating system is evaluated as not being in safe and efficient working order, and long-term life expectancy of the existing system is not anticipated. The structure is not equipped with air conditioning. The site does not contain underground fuel tanks. 3 Needs Replacement Provide new overall heating, ventilating, and air conditioning system to achieve compliance with Ohio Building Code and Ohio School Design Manual standards. B. Roofing
Description:
The roof over the original construction is an EPDM system that was installed in three phases (1987, 1992, 1997) and is in fair condition. There are District reports of current leaking in the corner of the gymnasium. Signs of past leaking were observed during the physical assessment. The roof over the 1993 addition is a built-up system with gravel ballast that was installed in 1993, and is in fair condition. There are no District reports of current leaking. No signs of past leaking were observed during the physical assessment. Access to the roof was gained by ladder. There were no observations of standing water on the roof. Roof drains are adequate in number, properly located, and in fair condition. 3 Needs Replacement The roof over the original building requires replacement to meet Ohio School Design Manual guidelines for age of system.

D. Electrical Systems
Description:
The electrical system provided to the overall facility is a 1600 amp, 120/208 volt, 3 phase, 4 wire system installed in 1966, and is in fair condition. The system in the 1994 addition is an extension of the original construction. Power is provided to the school by a utility owned, pad-mounted transformer located near the mechanical room, and in fair condition. The panel system, installed in 1966 / 1994, is in fair condition, with limited capacity for expansion. The classrooms are not equipped with adequate electrical outlets. The typical classroom contains 4 general purpose outlets and no dedicated outlet for each classroom television, as well as no dedicated outlets for student and teacher computers. All the spaces have electrical outlets. The corridors are not equipped with adequate electrical outlets for servicing. There are adequate GFI protected exterior outlets located around the perimeter of the building. The overall electrical system does not meet Ohio School Design Manual requirements in supporting the current needs of the school, and will be inadequate to meet the facilitys future needs. 3 Needs Replacement The entire electrical system in the original building requires replacement to meet Ohio School Design Manual guidelines for classroom capacity, due to condition and age, and to accommodate the addition of an air conditioning system. Provide additional panels (circuits / outlets) to increase capacity for the 1994 addition. The overall facility is equipped with thermally broken aluminum frame windows with double pane insulated glazing type window system, which was installed in 2002, and is in good condition. Description:
The original facility is equipped with concrete foundation walls on concrete footings, which displayed no locations of major settlement, or leaking. Minor spalling of the concrete foundations occurs at some of the building corners, but the foundations are in overall good condition and require no repair. The 1994 addition is equipped with concrete masonry unit foundation walls on concrete footings, which displayed no locations of major settlement, cracking, or leaking, and are in good condition. The District reports that there has been past leaking in the mechanical room due to a water main break. 1 Satisfactory Existing conditions require no renovation or replacement at the present time.

The original 1966 building has classrooms with vinyl asbestos tile (VAT) flooring, acoustic ceiling tile (ACT) type ceilings, and concrete masonry unit (CMU) type wall finishes all in fair to poor condition. Corridors are terrazzo and VCT type flooring, ACT type ceilings, and CMU type wall finishes all in fair condition. The restrooms have ceramic tile flooring, ACT type ceilings, and CMU type wall finishes all in fair condition. The 1994 addition has classrooms and corridors with VCT type flooring, ACT type ceilings, and CMU type wall finishes all in fair to poor condition. The facility is equipped with wood louvered interior doors in fair to poor condition. The toilet partitions are metal type construction, and are in fair condition. The existing kitchen equipment is original to the buildings construction and in fair condition. Description:
The typical classrooms in the original facility are equipped with T-8 surface mounted fluorescent fixtures with dual level switching. Classroom fixtures are in fair condition and provide an average illumination of 65 FC, thus complying with the 50 FC recommended by the OSDM. The typical classrooms in the 1994 addition are equipped with T-8 2 x 4 lay-in fluorescent fixtures with dual level switching. Classroom fixtures are in fair condition and provide an average illumination of 60 FC, thus complying with the 50 FC recommended by the OSDM. The typical corridors in the overall facility are equipped with T-8 lay-in or 2x4 surface mounted fluorescent fixtures. Corridor fixtures are in fair condition and provide an average illumination of 45 FC, thus complying with the 20 FC recommended by the OSDM. The gymnasium is equipped with pendant HID type lighting, in fair to poor condition, and providing an average illumination of 30 FC, which is less than the 50 FC recommended by the OSDM. The Media Center is equipped with surface mount T-8 fluorescent fixture type lighting in fair condition, and providing an average illumination of 68 FC, thus complying with the 50 FC recommended by the OSDM. The student dining spaces are equipped with pendant mounted T-8 fluorescent fixture type lighting in fair condition, and providing an average illumination of 26 FC, which is less than the 50 FC recommended by the OSDM. Description:
The overall facility contains a Z1100 type security system in fair condition. Motion detectors are adequately provided in offices, corridors, and spaces where 6 or more computers are located. Some exterior doors are equipped with door contacts. An automatic visitor control system is not provided. CCTV cameras are provided at main entry areas, parking lots, central gathering areas, and main corridors. CCTV is monitored in administrative area with the use of TV, VCR, and multiplexer. The security system is not fully compliant with Ohio School Design Manual guidelines. The exterior site lighting system is equipped primarily with surface mounted metal halide entry lights in fair to poor condition. Parking and bus pick-up / drop off areas are illuminated by pole mounted metal halide fixtures in fair condition. Description:
The 20 acre gently sloping site is located in a suburban residential setting with generous tree and shrub type landscaping. There are no apparent problems with erosion or ponding. The site is bordered by lightly traveled city streets. A single entrance then breaks into separate parking and bus loading areas allowing for proper separation of bus and other vehicular traffic, and one-way bus traffic is provided. Staff, and visitor parking is facilitated by a 91 space asphalt parking lot in good condition, which provides adequate parking for staff members, visitors, and the disabled. The parking lot drainage design, consisting of proper slopes and catch basins, provides adequate evacuation of storm water, and no problems with parking lot ponding were observed. Concrete curbs in fair to poor condition are appropriately placed. The front concrete sidewalks are properly sloped, are located to provide a logical flow of pedestrian traffic, and are in fair condition. The rear concrete sidewalks are somewhat steep, are located to provide a logical flow of pedestrian traffic, and are in fair condition. The site is not fenced for security. Separation of playground areas from vehicular traffic is moderate. The playground equipment is in fair condition, and on a combination of hard and soft surfaces. The athletic facilities are comprised of a baseball field and soccer nets, and are in fair condition. 2 Needs Repair Provide new asphalt wearing course over entry drive and bus loop area. These materials were described in the report to be in non-friable condition. There are no underground storage tanks on the site.
