1 Introduction

The purpose of this site safety plan is given below To provide general guidelines for the development of project specific Procedures to meet the safety requirements of the Company and its Client. The site safety plan is designed to make employees and our clients aware of The Company policy regarding safety and accident prevention and various Organizational relationships necessary for its implementation. To provide for projects (where the Company is the Clients representative on Site), a comprehensive safety and accident prevention procedure, which can Be used as a benchmark to measure and compare other project specific safety And accident prevention procedures.

Its objectives are


To protect the health and safety of the General Public, the Company, its Clients and Installers employees. To prevent accidents through proactive hazard assessment. To plan safety and accident prevention into the execution of all sites Activities. To establish responsibility / accountability for Safety and Accident Prevention. To promote continuous improvement in Safety and Accident Prevention. This manual shall be used for all Projects in conjunction with project specific Safety and Accident Prevention issues.

2. TERMINOLOGY
2.1 SAFETY
Can be defined as freedom from those conditions that can cause injury or death to personnel or damage or loss of equipment and property.

2.2 INJURY
Can be defined as a harmful condition sustained by the body as remit of an accident; it can be take the form of abrasion, a bruise, a laceration, a fracture, a foreign objects in the body, a puncture wound, a burn or an electric shock.

2.3 HAZARD
An inherent property of a substance or an occurrence which has the potential to cause loss, damage or injury to property, person or environment.

2.4 RISK
It is the probability of the realization of the potential for loss, damage and Injury.

2.5 ACCIDENT
It is an unplanned or unexpected event which causes or likely to cause injury to a person, damage to property or environmental pollution.

Work permit is a procedure to limit the hazardous processes and high risks tasks to authorized personnel, who shall be informed of the job safety analysis and the job specific safety precautions, on issue of a work-permit. The work permit issued under the procedure shall be valid for a specified period and shall be issued only after all safety precautions are fulfilled and duly verified by the RBIPL Senior Construction Manager or Senior Project Engineers who is responsible and authorized for safety certification as a prerequisite for issue of a work permit. The work permit shall be appropriate for the purpose for which it is issued. Work Permit will be verified by the RBIPL Safety representative at site and he is authorized to cancel the work permit if the said precautions are not followed or found any unsafe condition or act of if in case of any emergency. 1. Set standards are followed without deviation. Employees are competent to perform as per prescribed operation control procedures, Monitoring of safety of the various work areas/tasks Adequacy of existing operation control procedures and practices to mitigate and eliminate risks Joint inspection along with Client/Consultant will also be done.

RBIPL HSE&S have to initiate the hazards, Job Safety analysis and consultations with the Site Engineers (SE) to deploy appropriate remedial measures and improved operation control procedures. Periodic inspection reports and proposed remedial measures shall be recorded. Records of the change in processes; consultations with the SE and revision of operational

RBIPL will undertake periodic (Monthly) safety audits to confirm through investigative methods. The effectiveness of the measures set out in the POLICY. In order to be effective such safety audit shall be comprehensively covering all aspects detailed in this specification to ensure effective losscontrol/accident prevention program. Safety audits shall take into account the safety inspection records, remedial measures and effectiveness of the safety program. Effectiveness of safety program shall be based on the workers Effective hazard Identification and Job Safety Analysis processes for design of operation control procedures and on the safety statistics. Audit reports and preventive actions and safety improvement programs shall be recorded. The Client/Consultant shall retain their right to audit the Safety Management System either directly by their employees or his nominated representatives for its effectiveness.

4.6

INDUCTION

The Induction meeting should be seen as a start meeting. Preliminary to the general or Project oriented activities. In the Induction meeting beside technical relevant information pay attention to the aspect of Health, Safety and Environment in general. Induction will be giving to all newcomers from Workers as well as Staff. Following things will be explained on our induction. Introduction of Our Company, Consultant & Client. HSE Policy. Rules and Regulations.

Hazard Identification & Precautions. Environmental Care Systems & Implementation. Gate Pass will be upon successful completion of Induction Program.

4.7

PRE JOB MEETING

The Pre Job meeting is meant for consultation before the activity starts. It is a meeting to make detail appointment for specific plant & location directed by HSE methods and the same will be applicable if incase of any changes other than the normal situation of Work. Pre-Job meeting will be conducted by the site supervisor witnessing HSE Personal.

Tool box meeting is a kind of consultation to reach that executing employees are informed about the actual state of activities of the days work. This information can be appointments, instructions which are the result of above mentioned meetings. A tool box meeting is a medium to inform employees and will be conducting on daily basis for 5 to 10 minutes. 4.9 SAFETY MEETING

Safety Meeting will be conducted on Monthly Basis. Conduct a Monthly safety meeting (First Week ) where construction manager / Area Managers

should address all employees on site to discuss some pre-identified safety topics. 4.10 CO-ORDINATION The Project Manager of RBIPL shall be totally responsible for this Site Safety Plan. RBIPL shall appoint HSE Personals to follow this. 4.11 MONITORING Arrangement shall be made for EHS monitoring of the site on a regular basis. This shall included ensuring safety of Scaffolding, Excavations, Plant, Equipments and also Equipment matters such as hazardous fumes, noises etc. Project Manager shall ensure periodical (weekly once at specified area) inspection by visiting the site along with Safety Officers & the Concerned Engineers.

4.12 Specific hazard training Specialized training on following topics shall be given to the employees likely to get involved: Hot work Material handling Pressure testing Heavy equipment lifting & rigging Work permit system Lockout & tag out procedures Entry into confined spaces Fire prevention and control Emergency Response Team (ERT) etc. These courses should provide the time needed to properly address the topic. Completion of specific hazard training is mandatory before working on such tasks. A list of all trained persons shall be maintained at site.

RBIPL shall ensure that all notification, examination and inspection are carried out. All records shall be kept and updated by Project Manager. 4.14 STANDARD RBIPL shall comply with all that required to ensure safety of its Workers, Staff and Environment around as per Owner HSE Requirements.

WELFARE FACILITIES

The following Welfare facilities will be provided for worker at site. 5.1 Medical/First aid Facility

First aid is an immediate and temporary medical care provided to the injured workers at site before the services of a qualified medical practitioner are made available. It is important for the personnel administering First Aid to know what they should do and also what they must not do. Improper and careless movement of the victim may increase the severity of the problem and in extreme cases may cause death. Great haste in administering First aid may not be often necessary but in cases of severe bleeding and in cases where breathing is suspended, immediate action is needed. Bleeding: It is best controlled by direct pressure applied to the wound by a sterile dressing or any clean cloth. Keeping the victim in lying position will reduce the bleeding in some cases. It may be completely controlled by applying finger pressure to the blood vessel that supplies blood to the wounded area. Artificial Respiration: In cases of electric shock, drowning, gas poisoning and suffocation, if breathing stops, immediate action is essential. Artificial respiration should be given using resuscitator. Mouth to mouth respiration should be started without any time loss and should be continued until the victim starts breathing on his own, or is brought to the Doctor.

Fractures: In case of visible or suspected fracture, the adjacent joints should be immobilized. If bleeding also occurs, it should be controlled. While transporting the victim to the doctor, care should be taken not to disturb the fractured limb. First Aid Equipment: A fully equipped First Aid Box must be available at the site. It should contain adequate supply of various types of bandages and field dressings, essential medicines. The first aid box shall be marked with a white cross on a green background. It shall be placed where it can be seen and used. Small travelling first aid kits shall be provided for persons who are required to Work in groups, remote areas, or in isolation. Use potentially dangerous tools or machinery. Proper training shall be given to the persons with responsibility for first aid at site. 5.1.1 Action on occurrence of accident / incident The person at the scene of an accident shall Remove the hazard if safe to do so and call for help. Call for an ambulance if necessary. While awaiting the arrival of an ambulance: The casualty shall not be moved, unless in imminent danger. The attendant shall remain with the casualty and give reassurance. The casualty shall be made as comfortable as possible. The casualty shall not be allowed to smoke and not given any drink or food.

First-Aid center shall be provided near proximity of the area of work. FirstAid center shall be suitably illuminated and cleaned regularly. Doctor will be visiting once in a week for medical checkup. 5.1.3 EMERGENCY VEHICLE

One vehicle shall be there as Emergency Vehicle at any time available with a separate identity.

5.2

DRINKING WATER

Drinking Water Points will be provided at Work Sites and the tanks using for drinking water will be cleaned once in a week. Drinking water quality will be tested for chemical and Biological agent . Aro Plant (Water Purifier) Will be Provide at site as well as Labour Colony.

5.3 SANITATION
Portable Toilets and Urinals will be provided at site. The ratio as per the regulation and sewage will be removed for disposal through authorized People.

6.

Responsibilities

6.1 PROJECT MANAGER Establishing a culture of continuous improvement of health and safety performance on the project and ensuring that such matters are considered to be of equal importance to productivity, progress, profit etc. Demonstrating his personal commitment to a zero accident performance. Review, Develop & Ensure the Project HSE&S Plan to meet the specific requirements of the project. Ensure the selection of experienced and competent management team members and establish a Project HSE&S training plan for all.

Ensure suitable arrangements are in place to improve & monitor HSE performance to achieve goals throughout the Project. Establish a direct line of communication with the Project Safety Manager and other key persons. Establish a monitoring regime to ensure that unsafe systems, practices are identified and remedied at the earliest opportunity. places or

Provide all members of his project management/supervisory team with adequate direction, support and encouragement to fulfil their respective health and safety duties and responsibilities. Ensuring that the attendance at health and safety meetings is comprehensive and appropriate. He will also ensure that the minutes of such meetings assign actions and timings to named individuals and are promptly distributed to all relevant personnel. 6.2 PROJECT ENGINEERS

Demonstrating their personal commitment to and participating in initiatives designed to continuously improve health and safety performance and achieve a zero accident performance. Implementing the detailed requirements of the Client and Company Safety Policy and the Project Environmental, Health, Safety and Security Plan as they apply to their respective work sections. Organizing the management supervisory structure within their respective sections to ensure that the detailed requirements of the Client and Company Health and Safety Policy and Project Environmental, Health, Safety and Security Plan is understood and implemented by all personnel engaged in their respective sections. Ensure that personnel under their control are suitable and competent to adequately discharge their respective duties and responsibilities.

Continuous review of health and safety performance within their sections to ensure that performance targets are being achieved. Ensuring that the specific risk assessments conducted by trained, competent personnel at the appropriate time, and that the resultant control measures are communicated to the persons responsible for supervising and executing the work. Ensure that all management and supervisory personnel engaged within their section conduct routine health and safety surveillance as they go about their day to day work, in addition to the formal inspection regime specified in the Project Environmental, Health Safety and Security Plan. Liaising with project health and safety advisers and other similar competent persons such as occupational hygienists, nurses, first aid providers and taking account of advice rendered. Prepare reports on monthly intervals or as required by the Project Management, regarding overall Project safety performance. Initiate safety-training programs. Monitor work sites. Assist to develop and review of procedures and safe systems of work. Compile and update Safety Statistical information to the HSE&S Manager on monthly basis. Deliver site induction to new workers and site visitors. 6.3 HSE MANAGER

Co-ordinate with Project Manager. HSE&S Manager is authorised to report the MD other than the Project Manager. Provide leadership, planning and motivation in the implementation of HSE&S and loss control. Assist to develop and the review the procedures and systems designed to facilitate safe work.

In consultation with Project Management, regularly review safe work procedures. Ensure Proper Communication with the Top Management, Consultant & Client. Manage and organise the general activities and give guidance to Safety Officers/ supervisors. Liaise with Statutory authorities. 6.4 PROJECT SUPERVISORS

Encourage workers to carry out their work in a safe way. Lead by example. Listen carefully to workers HSE&S concerns and respond quickly. Develop a team approach for all HSE&S planning. SECURITY SYSTEM INCLUDING ENTRY & EXIT RULES.

Security personnel shall do all that is reasonably practicable to ensure the safety of employees and property of the company in the face of accidents by fighting fires and containing losses due to pilferage, theft, vandalism and industrial espionage both by employees and external elements. Security personnel shall be appropriately competent and receive adequate safety training. Security personnel shall routinely report on a standardized basis on aspects such as violation of fire-protection rules, use of alcohol and narcotic drugs, condition of security fencing, floodlighting and storages etc. Site Security shall be covering the following. Local anti social elements who may interfere on work. Security of site infrastructure, offices/documents/assets/machinery/personal Emergency aid. Regular checking along the perimeter fence to discourage potential offenders from throwing tools and equipment outside for later retrieval. Issue of vehicle daily/ permanent pass for entry & exit. 7.3 GENERAL SITE SAFETY RULES


All people to undergo induction training before engaging for job. Consuming of alcohol is strictly prohibited at site. Smoking is strictly prohibited at site other than smoking zone provided. All workers to comply the method statement and JSA Recommendations. Ensure work permit in place as appropriate. All workers to use toilet. Appropriate personal protective equipments such as Helmet, Safety shoe, High Visibility Jacket must be worn at all time. Not to interfere in others work.

Only Competent person/ electricians are authorized for any electrical connection or dis-connection and DG operation. Work place to keep neat and clean, wastage/ scraps to be removed after the completion of each work without delay. Watch the vehicle movement while working and nobody should sleep beneath of vehicle. Driver must check in front, back, and bottom of the vehicle before starting the vehicle. Reverse alarm shall be enforced while vehicle is in backward motion. No Vehicle should run beyond 20 KM/Hr. All electrical cable must have Junction box. During rain, Lighting, heavy wind, nobody should work at height. 8. PERSONAL PROTECTIVE EQUIPMENT Whenever there is a possibility of exposure to hazardous material or operations, personal protective equipment or devices shall be worn or used. These include (but are not limited to) hard hats, safety shoes, safety glasses and goggles, gloves, protective suits, hoods, respiratory equipment and proper hearing protection. (Gloves are to be worn to protect from dermatitis due to cement and wet mortar/concrete). RBIPL at all levels or supervision will be held responsible for Implementing that workers wear proper protective equipment and that it is kept in good repair. 8.1 Hard Hats : All personnel working in areas where these are a possible danger of head injury from impact, or from falling objects, shall be protected by protective helmets. This will be interpreted as all areas within the site boundary except inside permanent or temporary structures not under construction.

Helmets shall meet the specifications contained in the National Standards. Cutting gases should be stored properly. Oxygen /Acetylene (DA) cylinders should be vertically stored separately and locked with chain. Only authorized person is allowed to enter the storage area. Area should be protected from combustible and flammable materials. Proper ventilation to be provided. Portable Fire Extinguishers should be placed. HSD should be stored away from combustible and flammable materials.

10. ELECTRICAL SAFETY


Preventive maintenance is carried out by competent and authorized electrical personal in order to prevent unsafe conditions which lead to electrical failure & accidents.

Only authorized and competent persons should operate and maintain electrical equipment. For safe operation all the Electrical portable tools and devices are connected with Earth Leakage Circuit Breaker (ELCB) protection at Distribution Board. Switches, Circuits, Breakers, Feeders etc. should be provided as interlocking devices. Portable light, Head lamps etc. should be made and used on low and safe voltage. Make shift temporary arrangements should be avoided to the maximum extent. Unknown circuits should be treated as live and risk taking is always dangerous. Switches will be locked for maintenance by putting Lock Off tags. Work sequence and methods will be strictly observed. Personal protective will be used as required. Earthing, Fuses, Protective relays etc. will be installed and maintained for proper circuit protection. Emergency training for first aid, fire fighting and CPR training will be given to all electrical maintenance personal. Safety posters like Dos & Donts will be placed to remind electrical safety.

10.1 Cables
Cables shall be kept off the ground whenever possible and not run through wet areas, puddles or mud. Cables on the ground shall be protected from damage, be of the right type for the job and not a trip hazard. Extension cables shall be kept as short as possible. Extension cables shall not be used while they are still coiled up on a reel.

Insulating tape shall not be used to make joints or to cover faults in cables.

10.2 Plugs & fuses


Plugs shall be correctly wired and fitted in the correct socket. No temporary repairs shall be made. Fuses shall not be replaced by anything other than the proper fuse with correct rating. When fuses or circuit breakers operate, only an electrician shall check for the fault before the circuit is remade or the fuse replaced. The following Electrical Safety guidelines are designed to minimise risks from hazards associated with Electricity and Static Electricity: Work on electrical equipment and electrical power supply, shall only be carried out by qualified personnel. Electrical hazards shall be identified and safety precautions followed. Any source of electrical power that could be energized, before commencing work or electrical equipment shall be locked out. When working on conductors, cables or other energized devices, an insulating material between personnel and grounded device shall be placed. Additional insulation shall be used when working on damp or wet surfaces. Cathode protection shall be shut off and equipment earthed before starting any work. When checking / servicing batteries: Open flames shall be kept away and sparks near batteries avoided. Protective clothing shall be worn. No smoking shall be permitted. Tools with insulated handles shall be used. Dropping tools on the batteries shall be avoided. Rings / wristwatches /other clothing with metal parts shall be removed. Shopping seals shall be removed. Battery cells shall be vented.

When charging batteries, work area shall be ventilated and charging units properly connected to the battery. Adequate ventilation shall be provided while storing batteries in a closed area. Location of all underground cables shall be determined before starting excavation. Supply on all high voltage cables shall be switched off, tags placed to identify digging points and cables exposed manually. Extreme caution shall be used when working near poles with ground wires attached. In the event a ground wire or its protective coating being damaged, workers shall stay clear of the area and contact local electric utility immediately. Only authorized personnel shall replace fuses, use electrically tested safety gloves, stand on rubber mat and shut off electrical circuits. A heavy duty extension cord, with an isolating step down transformer and a low voltage secondary inserted in the line shall be used with portable lighting units in damp areas Storage of materials and ravel of equipment under temporary power lines shall be avoided at all times. 10.3 POWER TOOLS Power operated tools are widely used in the building industry. Driven by electricity, air, fuel or explosive cartridges, they are safe in trained hands, but may prove lethal if misused. 10.4 ELECTRICAL POWER TOOLS Before use: No unauthorized persons shall operate power tools. Machines shall be mounted on rigid foundations and earthed adequately Tool shall be checked for double insulation. Tools used shall be suitable for 110 volts power supply and residual current devices shall be used for additional protection. Plug shall be of correct type, undamaged and securely clamped to cable. Supply cable shall be checked for cuts, splits or damage. In use:

Makeshift repairs shall not be carried out. Qualified electricians shall do regular maintenance and repairs. Avoid standing on wet or damp surfaces when using electrical equipment Dead man Switches shall be in use.

10.5 CIRCULAR SAWS Before use: Power supply shall be isolated before adjusting saw and checking the operation of swivel guard and trigger switch. Saw blade shall be correctly set and sharp. Loose fitting clothing shall not be worn. Protective gloves / goggles shall be used.

In use

Saws shall be held firmly with both hands using main and front handgrips. Resin deposits on the blades shall be removed by the riving knife When mounted on a bench the saw shall have: 1. Suitable top guard, riving knife and ripping fence 2. Switch attachment to provide control of saw at bench 3. Underside guard for the blade.

10.6

DRILLS Before use: Condition of drill chuck shall be checked, using the correct key Drill bits shall be fitted after isolating power supply. Materials to be drilled shall be firmly secured before drilling to avoid spinning.

Before use: Only trained, experienced and authorized persons shall mount abrasive wheels and discs. Names of competent persons permitted to mount these wheels and discs shall be entered in register. Abrasive wheels and cutting discs shall be selected for the type of work and correctly fitted. In use: Guards shall be in place, adjusted and secure. Firm grip shall be maintained while operating. Name plate Details shall be provided in the grinders. Dead man switches shall be in use. Spindle speed shall not exceed the disk rated speed.

11. FIRE
11.1 FIRE PROTECTION AND PREVENTION Fire Protection and Preven tion Procedu re s shall comply w ith the requirements of the regulations laid down by th e respective authorities. The provisions of this section are inten ded for referen ce to supplement the regulation s, w hich shall govern. Fire is basically a chemical reaction. It is an oxidation process accompanied by heat, light and smoke. For its sustenance fire needs fuel, heat and oxygen. To eliminate the fire, it is important to know the source and its location. Nearly 25% fires are due to electrical causes in building construction. The known causes of fire are listed below. 11.2 Electrical: Temporary or makeshift and substandard wiring, and its overloading is the most common reason of electrical fires. Loss of insulation, loose fitting plugs and sockets and unmatched electrical equipment are a few other causes. 11.3 Smoking : Habit of casual and careless smoking at all the places at the work site without due regard to the attendant hazards like presence of rags, clothes, wooden scrap, discarded / stored chemicals, fuels, oils

Fire extinguishing equipment of appropriate size and type shall be: Capable of controlling any fire that may occur in the area (that such equipment is designed to service) to permit the worker to escape. Protected from damage. Readily accessible at adequately marked stations. Maintained in good operating conditions at all times. Regularly inspected to ensure serviceability in time of need. Site Safety Officer shall organize specific duties, which shall include: Locating the fire, its source and probable cause. Raising the alarm and evacuate personnel if any. Summoning the Site Fire Fighting Team or local municipality fire department or both depending on the severity potential of the fire. Directing the fellow workers to safe areas. Fighting the fire with available equipment until assistance arrives. Shutting off power to machines and fans. Turning off fuel to machines or other such facilities in applicable areas. Re-establishing the fire fighting capability after the fire has been extinguished. Fire Prevention measures shall depend on identifying potential hazards and taking appropriate remedial actions. Inspections shall focus on the following concerns. House -keeping. Handling and storage of flammable and combustive liquids, compressed gas cylinders such as oxygen, acetylene and fuels.

Observance of No Smoking in restricted areas and appropriate signs posted. Fire warning systems availability. Storage areas shall free from accumulation of unnecessary combustible materials. Sufficient numbers of fire extinguishers shall be installed at all temporary buildings and storage area where required. All flammable liquids, gas cylinders stored properly and kept separately. Flame cutting and welding shall be carried out with proper precaution. Flash back arrestors at two points to be ensured in flame cutting instruments.

11.6 MEANS OF ESCAPE. All gang ways, stairs, platform shall be free from obstructions. Every one on site shall be trained and shown the way how to escape safely in case of an emergency. Emergency exercise and evacuations shall be carried out. 11.7 FIRE FIGHTING.

All fire Extinguishers are checked regularly. Fire extinguisher shall be provided at vintage points for easy accessibility. Fire fighting training shall be imparted to supervisory staff & operators.

12. Civil Work

All excavations more than 0.60 mtr shall be Hard barricaded. Excavations less than 600 mm shall be cordoned off and suitable notices/ warning tapes posted. The most appropriate method of stabilizing the sides of an excavation can be achieved by properly designed shoring, which is to be approved by consultant. Ladders should be provided at intervals of not more than 15 m, depending on the number of men present. All excavated materials shall be deposited minimum 1.5 M away from the edge of the excavation. Illumination inside the excavations, whether natural or artificial, should be adequate at all times. Walkways between trenches should be kept clear of obstruction.

Any openings in barricade that may have been necessary for operational purposes should be securely closed before the site is left at night. 12.2 CONCRETE BATCHING & MIXING PLANT Travelling skip, weigh batcher and rotating drum shall be clearly visible to the trained and experienced operator in the cabin. Access to the cabin shall be controlled and cordoned. Weighing attachment shall not be manually tampered with. Mixing drum shall be thoroughly cleaned after the end of each work cycle and whenever the plant is shut down for more than ten minutes. The adjustment of clutch and brakes shall be checked as per the plant operation manual. Plant control panel shall be properly earthed and workers shall be prevented access to the plant area. Wire ropes shall be checked daily before commencement of operation and overloading of skip and drum shall be avoided. Any unwarranted mechanical noise shall be immediately investigated by stopping the plant.

Protective clothing / headgear and masks shall be worn by workers as required. Checked all moving parts are covered. 12.3 TRANSIT MIXER Only trained operators shall operate the mixers and Signal man to be provided for the reversing vehicle and stopper will be provide (wedge block). Lubrication and cleaning of rotating drums shall be done after every work cycle. Rear-view mirrors and all indicator lights shall remain serviceable at all times. Access to the rear / sides of the mixer while loading/ unloading shall be restricted. Water & air connections, pneumatic hoses, indicator gauges shall be checked every day for serviceability. 12.4 CONCRETE PUMP Only trained operators shall operate the concrete pump. All pipes/bends/snap couplings and other components shall be checked for leakage and cracks. Piping shall be cleaned by using sponge balls and the equipment greased periodically. Cement slurry shall always be pumped before pumping concrete and the end elephant hose shall be held fast in position using firm stand and fiber ropes. Pump assembly and piping shall be firmly anchored and fast. While using in high rise buildings, counter blast shall be provided. 12.5 CONCRETE VIBRATORS Vibrator unit shall be fully enclosed and belt drives if any shall be fully guarded. Electrically operated vibrators shall be fully enclosed with overload relays and have proper earthing. Sufficient length of cable shall be provided for shifting the vibrators. Needles and other components shall be checked for lubrication, free motion and serviceability every day before starting work.

12.6 REINFORCEMENT

Proper PPE s like hand gloves, safety shoes, helmets, goggles shall be used while cutting, fixing or transporting steel rods. For supplying steel rods at levels above 2 M suitable lifting device should be used and manual handling should be avoided. During manual transportation of rods or any other steel materials, pads should be used on shoulders. Unloading should be synchronized. If the rods are transported mechanically following precautions shall be taken: a) The rods must not protrude behind the deck of the truck. b) In no case, the rods should protrude in front or by the side of the drivers cabin. This imposes hazards for other road users. c) The bundle of rods should be kept properly chained on the deck of the truck to prevent them from getting dislodged or bumping out of the truck while passing through uneven roads. d) None should be allowed to sit on the bundle of rods during transportation. e) Crow bars should be used to push the bundle of rods out from the deck of truck. f) For bending rods tools of right size shall be used. Oversized tools or mushroomed chisel and hammer shall not be used. 12.7 CONCRETING AND CEMENTING Working men at height should be done under proper supervision provided covered platform with hand rail , Knee rail and Safety Harness along with the provision of life lines wherever required for which hooking points needs to be designed along with structure. Workmen handling cement or concrete should be provided with goggles, rubber gloves and gum boots other than PPEs. All rotating parts, gears, chains and rollers of mixer shall be properly guarded. All cables, clamps, hooks, wire ropes, gears, clutches etc of mixer shall be checked cleaned, oiled, greased and service once in a week.

While cleaning the inside of the mixer drum operating power shall be disconnected and suitable notice shall be hung at the place. The moving part of mixing and grinding machines used at the construction site should be covered with guards.

13. Height Work


Precautions shall be taken to avoid from falling down and use full body harness with shock absorber while working at height. For work above ground floor level, proper scaffolding /staging shall be erected depending upon the type of work. 13.1 USE OF LADDERS. Ladders shall be fixed near the top if practicable or near the bottom and secured. Shall be placed on a firm level base. Shall be intermediately secured to prevent slaying and sagging. While worker working above 2mtrs fall protections shall be used. Ladders shall be extended at least 1mtrs above any landing place beyond the highest rung from which a person shall work or have a nearby hand hold of univalent height. Ladders shall be placed so that there is space behind each rung for proper foot hold. 13.2 SCAFFOLDING /STAGING. All scaffolding shall be erected and dismantled by qualified and experienced personnel and supervised by competent supervisor. Load bearing staging will be designed by competent person. Work activities above1.8mtrs/6fts from ground floor level.

Proper steel scaffolding /staging shall be erected for works above 2mtrs height above the floor level under the supervision of a trained & authorized person. Ladders properly secured shall be used. Supervision of work and inspection of materials. Scaffolding /staging shall be erected altered or dismantled only under a supervisor as far as possible. All scaffolding /staging shall be inspected and tagging system will be done before use. 13.3 MATERIALS Sufficient sound materials shall be provided for scaffolding /staging strong enough for the job. Scaffolding/staging tubes and fitting shall not be bent, distorted and unduly rusted. 13.4 DEFECTIVE MATERIALS Scaffolding /staging tubes, couplers, fittings bent unduly rusted or distorted shall be rejected. All scaffold/staging components shall be properly stored when not in use and kept separately from other building materials. 13.5 MAINTENANCE OF SCAFFOLDING/STAGING Scaffolding/staging shall be kept in good condition and every effort made to prevent the accidental displacement. 13.6 PARTLY ERECTED OR DISMANTLED SCAFFOLDING/STAGING If any scaffolding/staging are partly erected/partly dismantled and is still capable of being used to some extent shall have warning notice fixed (Tag system) and alliaceous blocked at point beyond which cannot be safely used.

BARRICADES

Barricades shall be erected, maintained when required in connection with concern engineers operation. Engineers, supervisors shall become acquainted with P.M, HSE officer and barricading practices and shall respect of provision there off.

CRANE & RIGGING

The contractor shall appoint a suitably qualified and experienced person to act as supervisor for rigging and lifting works. His responsibilities shall include preparation of the rigging plan, and the safe rigging and lifting of the load at work site. All works involving the use of a crane shall be properly planned in advance and a rigging study carried out to ensure that, - The crane is capable of lifting the load, given the known working radius, boom length, weight of the load, etc. - The condition of the ground at the crane location is satisfactory to support the crane and the load; The rotation of the cab, and therefore the boom is not restricted

- Suitable matting or plates are available to protect underground services and paving - All slings, shackles, hook, etc., of the correct rating are available and in good condition (i.e. without lapping, knots or artificial extensions). Crane hooks shall be fitted with properly functioning safety clips to prevent the displacement of the sling from the hook during the lift. For wheeled cranes outriggers shall be used for each lift, regardless of the size, shape and load always be fully extended.

Shackles shall be complete with the original pin and under no circumstances shall a substitute bolt be fitted. A trial lift shall be carried out, raising the load a short distance above its pick-up point, to check the stability of the crane, and the efficiency of the brakes. Only one person, usually the Supervising Rigger, shall give instructions to the crane operator. At no time whilst the cranes machinery is running, shall the operator leave the cabin. At no time whilst a load is suspended shall the cranes engine(s) be intentionally turned off. At no time whilst a load is suspended, shall personnel perform any works directly beneath. Particular consideration to the presence of low-level structures such as pipe badges shall be given where crane is to move about the site. Consideration shall be given to prevailing weather conditions, and where a greater than normal risk exists, the lift shall be postponed until more favorable condition develop. The training, skill and experience of the crane operator and crew is important element of a properly organized lifting operation. For day-today routine crane operations, careful selection of the most suitable type of crane, plus the establishment of a safe system and method of work would be essential. The statutory provisions, which apply to lifting operations during construction operations, are contained in Building & Other Construction Workers (Regulation of Employment and Conditions of Service) Act 1996 and Rules, 1998. Every contract agreement for a Contract Lifting Operation should state the following: That all work will be carried out in accordance with safe practices, and that the contractor will appoint one person to have overall control of the Lifting Operation The following item should be checked before allowing the crane to work:

Certificate of Testing and Examination issued by a competent person as per statutory rules. Record of the periodic inspections on the crane, signed by a competent person of contracting agency. That the lifting equipment to be used are suitable for the job and that the test certificates for such items are available. The crane hook is provided with safety catch. Loads should only be moved when the signaler can see the load and communicate with the driver. Copies of the sling chart and safe working load tables shall be used. The counter weight of the crane and boom movement area shall be cordoned off. No one should ride on load being slung. Tag lines should be used to control load swinging. Load shall not be lifted if there are knots / kinks in the slings. Lifting Gear The severe usage to which lifting gear is often subjected, may result in failure, it is important that attention be paid to the correct use and maintenance of such lifting gear. This can best be achieved by: Good design and workmanship Careful testing and inspection after manufacture or repair and regular maintenance. Detailed planning, correct and careful use of the gear and storage under cover When U clamps are used on wire rope slings the rounded portion of U clamp shall be on the free end of the wire rope. Minimum 3 nos of U clamps shall be used for splicing and more clamps required depending upon the diameter. The Safe Working Load (SWL) markings on an item of lifting equipment shall be inspected before it is used. These numbers are normally stamped

on to the master eye or ferrule of slings and the body of eyebolts, shackles, etc. Alternatively, metal tabs, which bear this information, may be fitted to slings. On web slings the information may be on a label stitched into the sling, normally at the eye, and they may be additionally colour coded to identify SWL. 16. WELDING AND CUTTING All welding and cutting shall be done by workmen who are thoroughly trained in the work screen shall be placed around the work to protect persons from glare. Welding and cutting shall not be done in the proximity of flammable material.100% Spark containment will be introduced. All hot work area will be provided fire extinguisher, Fire blanket and trained fire watchman. Welders and helpers shall wear non-combustible helmets and gloves during welding operations. They should be careful to keep out of the line of sparks and hot metal. They should wear clothing free from grease, gasoline, oil and other flammable material. Oxygen and acetylene cylinders or containers shall never be permitted in small spaces or compartments where welding operations are in progress. A helper shall always be at hand to shut off the gas in case of an accident when the welder is working in a space where it is difficult to escape. Oxygen and acetylene hose should be regularly checked and those, which have developed Cracks, should be replaced. Care should be taken to see that sparks of grinding and slag of gas cutting do not reach near the vicinity of hoses of oxygen and acetylene. Preferably these hoses should be laid away from electrical cables. Gas equipment shall be regularly inspected and a thorough system of inspection / maintenance is to be in place. All gas equipment used will have flash back arrestors fitted with proper gauges. Joints on welding cables should be avoided. In case it is necessary the cable joint connecter will be provided (Not to be bolted) and the joint should be wrapped with a non-conducting material to avoid sparks caused due to contact with conductor. Formation of water pools under gas cutting beds should be avoided. It may cause explosion.

Gas cylinders should be kept vertical in shades. If the work is being carried out away from the shade then water soaked absorbent gunny bag shall be placed on the valves and regulators. Sufficient numbers of water containers should be placed at different locations in the fabrication yard to facilitate wetting of gunny cloth and quenching of red-hot cut out pieces of structural. All welding machines should be properly earthed. Gunny packing or rubber pads should be provided between the structural and the slings to avoid damage of slings. All combustible materials like packing, boxes, cotton rags soaked with oil place where hot work is being carried out, cylinders in use, must have valid test certificates. Cylinders should be transported on trolleys only and not by rolling them on the ground. Prior to being used all cylinders should be checked for leakage. Separate enclosures should be provided for storing oxygen and DA cylinders. All hose fittings shall be kink free. All leads, grounds, clamps, hose gouges, torches, cylinders flash back arrestors must be inspected before use. The compressed gas regulators and other fittings shall be free from oil, grease etc. Do not store wrenches, dies, cutters or other grease covered tools in the same compartment with oxygen equipment. Do not exceed 15 PSI on the torch side of the gauge when using acetylene. Lift cylinders to upper levels with approved methods only. Do not use slings or lift a cylinder by the protective cap. Welding shall never be carried out by metallic ladder. Welding rod should be removed from the electrode holder when laying it down. Slab ends should be discarded in metal containers.

17. PAINTING:
17.1 STORAGE:

Packages containing paints, varnishes, lacquers and other volatile painting materials shall be kept tightly closed when not in actual use and shall be placed where they will not be exposed to excessive heat, sparks, flame or direct sun light. Material Safety Data Sheet (MSDS) shall be displayed & Explain to working crew in local language.

17.2 FIRE HAZARDS: Most paint materials are highly combustible and every precaution should be taken to eliminate possibilities of fire. No attempt should be made to heat paint materials except by placing containers in air or water at moderate temperature. All rags used in painting works should be disposed off at frequent intervals. Smoking, open flame exposed heating elements and other sources of ignition of any kind should not be permitted in paint stores or areas where spray painting is going on. Fire extinguishers of appropriate capacity shall always be available in working condition where flammable paint materials are being mixed, used or stored. Also sufficient number of sand pails should be kept and maintained in fire stands. 17.3 RESPIRATORY PROTECTION: Injurious fumes are given off when volatile paint materials are being mixed or applied especially when they are sprayed. Coal tar paint fumes are particularly noxious. Workmen must be provided with ample supply of fresh air. If natural ventilation is insufficient, forced or artificial ventilation should be provided. Workmen shall wear approved type respirators if required. Spray painting operation shall be so confined as not to contaminate the air where other men are working. Spray gun operators should wear clothing, which fits snugly at the ankles, neck and wrists and should wear gloves, goggles and respirators.

17.4 HANDLING PAINT MATERIALS: If the skin is exposed to prolonged contact with paint materials, injury of skin may take place in the form of burns of toxic effects resulting from absorption into or through the skin. To avoid this protective cream shall be applied to the skin before exposure to paint materials. Painters should clean their hands before eating.

18.

HOUSE KEEPING

Good housekeeping is a pre-requisite for a tidy and safe working environment. It is also an important element of accident prevention. Housekeeping measures shall be planned at the commencement of site operations and supervised through to the completion stage till the site is handed over to the client. Housekeeping as an effective tool for accident prevention shall be explained to all the site personnel and everyone shall be expected to participate in it. All engineers / supervisors shall ensure adequate time and effort is available in their areas and shall Endeavour to keep the site clean and tidy. Orderliness shall be ensured in all locations at all times. In the storage areas, all the materials shall be stored/ stacked in an orderly manner with well - laid aisles/ walkways for easy access. There shall be no projections/ protrusions in the walkways. In the work areas, access to walkways, roadways, and fire and rescue equipment shall be ensured. Loose material, scrap, tools and equipment shall not be allowed to accumulate near ladders, ramps / stairs and other accesses to work areas. Oily rags, greasy material and other potentially dangerous items shall be effectively removed and disposed off. Protruding nails are dangerous. Persons shall be assigned the task of removing wooden pieces with nails. Scrap yard for wooden scrap shall be located well away from fabrication and welding shed. Adequate lighting shall be provided at all work areas and approaches. All passageways, accesses, stairs and ladders shall be well lit.

All openings in floors/ shuttering shall be well marked and cordoned with caution tape and warning signs posted as appropriate. Approach roads shall never be blocked without adequate notice and for minimum possible duration only when inevitable. Fire hazards shall be minimized by keeping flammable and combustible substances in labeled containers and away from the sources of fire at site. Vehicles shall be parked in their assigned places. Light objects shall be stacked on heavy and bulky objects (and not viceversa). Stakes, driven into the ground; to prevent rolling, shall secure Round/circular objects, pipes and such other material. Drinking water, wash and toilet facilities shall be maintained clean and broken and burned out lamps shall be replaced immediately. ORDERLINE SS AND GOOD HOOUSEKEEPING SHALL STAND IN GOOD STEAD AND ENABLE THE PROJECT MANAGER TO ACHIEVE THE ZERO ACCIDENT O B J E C T I VE .

To maintain a well-managed safe working place in sound clean condition to ensure that there is a place for everything and everything in its place so that optimum use is made of valuable floor space with commensurate cleanliness and reduced handling time. General housekeeping in working areas will be maintained. Scrap bins will be placed where ever required. Regular collection and disposal of waste shall be done. Pathway/walkway shall be cleaned as far as possible. Adequate illumination will be provided. All sanitary facilities shall be maintained and cleaned at regular intervals. Water leakage and over flow shall be checked.

19.1 Inspection
All Vehicles/Mobile Plants will be inspected and valid copies of all statutory documents will be collected. All Vehicles/Mobile Plants will be inspected as per Checklist by Drivers/ Operators on Daily basis and Quarterly by competent person along with colour coding system & record will be maintained.

19.2 Traffic Rules & Regulations


Motor vehicles / traffic / powered mobile equipment shall comply with the following rules regarding vehicle operation at site: Personnel operating vehicles shall hold valid Operators License and comply with specific traffic rules and signs. Only trained operators shall be permitted to operate Cranes, Excavators, Forklifts, Dozers and other heavy equipment. No small vehicles shall be parked behind heavy vehicles. Separate areas outside the site shall be earmarked for parking of visitors cars. All vehicles shall be driven under control and within specific speed limits and give pedestrians the right of way. When backing/ reversing, a ground guide shall be used for signaling, to inspect all around the vehicle and to remove chocks if any. Back-up alarms shall be used. Motorized equipment shall not be left unattended at any time. All equipment shall be shut down /closed down and switched off, when not in use. Occupants of vehicles shall use seat belts at all times while travelling. Passengers shall not be permitted on heavy equipment such as cranes, loaders, and earthmoving equipment. Booms, protruding jibs and A frames shall be safely stored when not in use and not protrude into access ways, roadways, or over buildings or pedestrian walkways. Traffic Wardens will be provided at site with high reflective vest if required to control the traffic.

No powered mobile equipment shall be allowed within a distance of 3m from power lines. And NO SMOKING SHALL BE PERMITTED IN VEHICLES. No vehicles should move more than 15 km/hr. Control of all traffic on site, movements of materials and parking shall be planned and executed according to space availabilities. JCB scrapper blades of similar equipments shall be either fully lowered/blocked where being repaired or when it is not in used. All controls shall be made to be in neutral position and motor stopped and break set. Other standard on Vehicles/Equipments such as rear view mirrors, operating levers shall be in accordance with standard vehicle codes and will inspect.

19.3 MOBILE AND STATIC PLANT & EQUIPMENT


Mobile plant is mechanical plant, which moves under its own power, e.g. excavators, forklifts, dumpers. Static plant, despite being wheeled, tends to remain in place when operating, e.g. cement mixers, compressors. 19.4 EXCAVATORS Operators shall be authorized, trained and experienced on excavators. Signalers signals shall be used where operators vision is obscured. Warning and caution boards shall be used to isolate the area of work. Operator shall be cautious and alert while operating and shall not indulge in any other activity. He shall not load dumpers till the driver is in a safe position. Due care shall be taken to switch off overhead power lines while operating in their vicinity. Lifting capacity of the machine shall not be exceeded. Machine shall be kept away from the edge of trenches and care shall be taken while operating on slopes. Workers shall not stand below a raised bucket and within the operating circle of an excavator They shall not ride in the bucket, or use it as a means of access.

They shall not work in narrow gaps between the machine and adjacent structures. 19.5 ROAD ROLLER/ VIBRATORY ROLLER Operators shall be authorized, trained and experienced. Operator and other workers shall use protective footwear near the machine. Persons working nearby shall keep clear of the machine and not touch the controls. Vibrations shall be reduced near structures & stopped when the roller is not moving. 19.6 Operator checks: Operator shall conduct daily and weekly checks of machine, including operation of dead mans handle. He shall ensure that persons and objects are clear of path of travel. He shall check to the rear frequently when reversing. He shall park the machine on level ground, and chock wheels on sloping ground. 19.7 DUMPERS/ TIPPERS/ WATER TANKERS The operator shall be authorized, trained and experienced. Operator shall not Obscure view with load &carry passengers, unless a seat is fitted. Drive at excessive speed and discharge loads only after carefully positioning the vehicle. Drive across slopes and park the vehicle on firm, level ground. Remain in the drivers seat when the vehicle is being loaded. Operator shall Check machine daily and weekly and report /rectify defects. Use correct grip on starting handle. Distribute loads evenly and avoid overloading. Flag projecting loads outside the vehicle body, if any. Remember that wet brakes do not work frequently. Use stop blocks when tipping into excavations or pits.

Apply hand brake when stopped. Place gear lever in neutral when parked. Chock wheels when parked on slope. Secure or immobilize the machine at the end of days work. Persons working near the machine shall Keep clear of machine operating routes. Not to ride on dumper/tanker. Not operate machine without authorization. 19.8 CONCRETE MIXER Operators shall ensure that All guards on gears, chains and rollers are in place and hopper protected from sides to prevent entry of workers under it. Correct grip exists on starting handle and wire rope for hopper hoisting is checked and serviceable. Loose clothing or cuffs, which may become entangled in rotating drum, are not worn during work and hopper hoist and anchoring brakes are properly adjusted and functioning. Skin contact with cement mortar is avoided. For electrically operated mixers, motor fan guard is secured and wiring properly connected and insulated. Double earthing is provided to the unit. Machine shall not be operated with the engine covers raised. Body parts do not get in mixer bowl while it is rotating 19.9 VIBRATORY COMPACTORS Only trained operators shall operate the machine. If electrically operated, the wires / cables shall be properly laid to avoid obstruction to compaction work and proper earthing shall be provided. 19.10 PAVEMENT BREAKERS/ CONCRETE DRILLS Only trained operators shall operate the machine. All pneumatic hoses/ connections shall be checked for leakage and damaged hoses / components replaced immediately. Pneumatic hose connection should have whip check on all connection.

Protective goggles/Ear plug or muff/ clothing / shoes shall be used by operators. 20. CONCRETE BATCHING & MIXING PLANT

Travelling skip, weigh batcher and rotating drum shall be clearly visible to the trained and experienced operator in the cabin. Access to the cabin shall be controlled. Weighing attachment shall not be manually tampered with. Mixing drum shall be thoroughly cleaned after the end of each work cycle and whenever the plant is shut down for more than ten minutes. The adjustment of clutch and brakes shall be checked as per the plant operation manual. Plant control panel shall be properly earthed and workers shall be prevented access to the plant area. Wire ropes shall be checked daily before commencement of operation and overloading of skip and drum shall be avoided. Any unwarranted mechanical noise shall be immediately investigated by stopping the plant. Protective clothing / headgear and masks shall be worn by workers as required.

20.1 CONCRETE PUMP


Only trained operators shall operate the concrete pump. All pipes/bends/snap couplings and other components shall be checked for leakage and cracks. Piping shall be cleaned by using sponge balls and the equipment greased periodically. Cement slurry shall always be pumped before pumping concrete and the end elephant hose shall be held fast in position using firm stand and fibre ropes. Pump assembly and piping shall be firmly anchored and fast. While using in high rise buildings, counter blast shall be provided.

20.2 CONCRETE VIBRATORS


Vibrator unit shall be fully enclosed and belt drives if any shall be fully guarded. Electrically operated vibrators shall be fully enclosed with overload relays and have proper earthing.

Sufficient length of cable shall be provided for shifting the vibrators. Needles and other components shall be checked for lubrication, free motion and serviceability every day before starting work.

TRAINING

Training for Safety and Accident Prevention shall begin at the time of employment, before employees start work. Initially, the Orientation Session will be conducted by the Safety Representative to cover site safety and accident prevention aspects as described in this Manual. Training is a never-ending process and the supervisors shall continue the training so that the workers know the techniques of performing the job to the requisite standards of safety, quality and output and the degree of responsibility expected of them. Supervisors shall ensure that workers understand the logic behind safe work practices and follow the rules implicitly. Supervisors safety orientation shall cover all possible safety aspects including (but not limited to) the following. a) Existing hazards in the operations in which the new or transferred employee will participate. b) Existing safeguards and precautionary measures that must be taken for those hazards, such as work permit requirements in the work area. c) Specific requirements for safety goggles, safety boots, hard hats, gloves, face shields, or other protective equipment in the work areas. d) The locations of emergency exits (if applicable), phones, fire extinguishers, fire buckets, first aid kits, etc. and other emergency equipment. e) Locations of nearest hospitals and first aid facility, police and fire stations. f) The procedures to be followed in case of emergencies and the specific requirements of the same. g) Reporting hazards and defective equipment and initiating corrective measures. h) The need of good housekeeping and active employee participation in safety and accident prevention program. Supervisors shall bear in mind that accidents generally occur because of the following main factors and shall base the Safety Observation Techniques on them.

22.3 OFFSITE EMERGENCY PLAN A clear organization structure giving details of command structure warning procedures and emergency control centers. An efficient communication system. A list of valuable equipments like bull dozers, cranes, fire fighting and breathing apparatus. Availability of specialist organization and people. Details of organizations like fire service, police, hospital, factory inspectorate, voluntary organizations etc.
