Location CH1160 CH1950 Report No.: CV/2006/01-W27 1.1 Purpose and Scope of This Permit This permit acts as the guidance for supervisors and/or workers who want to do excavation at places where underground utilities may be found. The supervisors must make sure that all workers understand and follow this guidance before carrying out any excavation. This permit-to-dig must be presented together with the cable detection report in the vicinity of trenches and easily accessed or checked by monitoring staff. It applies to all excavations regardless of what underground utilities may be found in the trenches. NO excavation can proceed without this permit, no matter what excavation techniques adopted. 2.1 Working Procedure for Excavation near Footpaths and Roads 1. The Contractor should obtain the latest as-built drawings from the relevant utilities undertakers (abbrev. UUs) and identify what types of cables may be found in the trenches. 2. By means of a cable detector, carry out the cable detection by a competent person (abbrev. CP). Issue the cable detection report and set out the alignments and depths for all utilities on the preexcavated trenches. Take pictures for record purposes. 3. Ascertain the exact position and depth of the utilities by means of hand-dug trial trenches until all the cables are exposed. If the number of cable(s) is/are different from stated in the drawings or the detection report. Contact the relevant UUs for them to identify the unknown cable(s). 4. Avoid using heavy duty machinery such as breakers, backhoes or drillers. If it cannot be avoided due to the site constraints, care must be taken on the operation of those machines. The supervision staff should, if possible, monitor the whole course of operation. 5. For the utilities that are hanged above the formation level for the trenches, they must be supported by the approved method and materials. The temporary protection measures might be checked by ICE when necessary.

6. In case of emergency, contact the appropriate person to arrange for any maintenance or remedial works. See Appendix B for the emergency reporting procedure and the contact lists. 3. Important Points to Note When Doing the Excavation

a. Backfill the trenches with suitable materials in layers with adequate compaction to avoid ground settlement b. Always provide adequate site supervision to ensure that all necessary precautionary measures are taken and remind workers and staff regularly of the importance of avoiding damage to existing utilities and the full implications of the damage c.
