CHECKLIST OF DOCUMENTS FOR LC2


Please attach a copy of this checklist to your application as your covering page. Complete and return this checklist along with all the required documents within 60 days. Please refer to the enclosed Instructions for Permanent Residence for Overseas Family Members of In-Canada Applicants under the Live-In Caregiver Program in case you have any questions regarding any of the documents required. Send originals of the police certificates and forms listed in items 1 to 6 below. Send photocopies only of all other documents, unless instructed otherwise.
