A separate IMM5604 form must be completed for each child. IMM5604 must be witnessed by a notary public. III. Unaccompanied Minors / Guardian of Minor child Must be submitted by: (1) Guardianship Questionnaire (Enclosed) Form to be completed by parent in Canada
