Procedures for Registration 1.

FOR FIRST REGISTRATION Two sets of self attested photocopy of following documents are required . 1.Marksheet of High school 2.Marksheet of Intermediate. 3.Mark sheet of Diploma/Degree in Pharmacy of all the years. 4.Certificate of High school. 5.Certificate of Intermediate 6.Diploma / Degree of Pharmacy or Provisional certificate issued from the college showing passed Diploma/Degree in pharmacy examination 7.Practical Training certificate of 500 hrs from PCI recognized Government Semi Government and Government aided Hospitals for Diploma Pharmacy candidate only. 8.Current Domicile Certificate ( issued within six month ) For those condidate who are passed from outside of UP. 9.Six Coloured Passport Size Photograps. 10.Rs. 500 Registration Fee, Rs.200 Development Fee. Note: Kindly bring original copy of all documents for verification. 3. RENEWAL The following set of self attested Photocopy are Required. 1.Green Card holder are required to bring recent coloured passport size photographs. 2.For those ,Applying for Green Card, two recent coloured passport size photographs to submitted. 3.Green Card in orginal issued by UP Pharmacy Council. 4.Photocopy of Diploma/Degree(if not deposited) Renewal Fee 1.Rs. 500/- for Five Years 2. Rs. 0/- for those Pharmacist who are Registred under Life Renewal Scheme. 4. FOR TRANSFER CASES 1. Self attested two photocopy Marksheet and Certificate/Diploma/Degree of High School , Intermediate, B.pharma. 2. U.P. State Domiciles Certificate (issued within six month). 3.Registration Certificate and Renewal Certificate (in original ) issued by other state's pharmacy council where pharmacists is currently registred fee Rs. 2000. 5. ENTRY OF ADDITIONAL QUALIFICATION. 1. R1. Registration Certificate and Renewal Certificate (Green Card) in original. 2.Two self attested photocopy of Marksheet and Degree of additional qualification along with original document for verification fee Rs. 100. Fee Structure

1.NEW REGISTRATION Registration Fee Rs. 500/- , Rs.200/- Development Fee. 2. RENEWAL a. Rs. 500/- for Five Year b. Rs. 0/- for those Pharmacist who are Registred under Life Renewal Scheme. 3.LATE FEE RENEWAL/RESTORATION a.Within Ten Years Rs.200/-. b.More Than Ten Years Rs.500/- . c.Annual Renewal Charge for Per Back Year ( 1990 Rs. 5/- per annum,1991 to 1999 Rs. 10/- Per annum , 2000 to 2005 Rs. 40/- per annum , 2006 to Till Date Rs. 100/- Per annum). 4.TRANSFER/RECIPROCAL Rs.2000/-. 5. ENTRY OF ADDITIONAL QUALIFICATION. Rs.100/-. 6.*Duplicate copy of Registration certificate Rs.1000/- . 7*.Duplicate copy of Green Card Rs.300/-. * For issue Duplicate Copy of Registration Certificate /Green card The condidate must have provide the following documents. 1.Self attested photocopy of Diploma Certificate/Degree/Registration Certificate issued by UPPC if (Available).. 2.Three Coloured Passport Size Photograps. 3.An Affidavit on non judicial stamp paper. 4.News Paper Cutting ( Publish in news paper ). 5.FIR Copy of Police Station.
