Uri Zernik Conservatee's son = Repeat Request to Inspect Court File, pursuant to the Regulations of the Courts Inspection of Court Files 2003, and Supreme Court Decision in Israeli Civil Rights Association v Minister of Justice (5917/97) I, the undersigned, Joseph Zernik, ID no..., knowing that I must say the truth, and that I may be liable for penalties stipulated by law if I do not do so, declare as follows: 1. The undersigned, Dr Joseph Zernik, files herein a Repeat Request to Inspect Court File, as detailed below. Reasons for the Request 2. I am the Conservatee Rivca Zernik's son. 3. I am the Conservator Robert Zernik's son. 4. In 2010 I was asked to provide my consent for the Conservatorship. 5. In December 2012, I filed a request to inspect the court file, and it was granted by Presiding Judge Ehud Rekem. 6. Judge Esperanza Alon purportedly summoned me to appear in this case. 7. On April 29, 2013, I filed a repeat requerst to inspect the court file. 8. Regarding my April 29, 2013, no decision was received by me to this date. 9. In a visit to the office of the Clerk of the Court, I was informed that a decision on my April 29, 2013 request to inspect was registered, but was not served, for reasons that are unclear to me.

Since no decision was served on my April 29, 2013 Request to Inspect, I filed a Repeat Request to Inspect on May 22, 2013. 11. Also regarding my May 22, 2013 Repeat Request to Inspect no decision was served on me to this date. 12. Instant Request to Inspect pertains to all records in the court file, including: General Details, Parties, Related Cases, List of Hearings, List of Requests (or any other papers filed by parties) List of Decisions, List of Papers Rejected from Filing, Certificates of Service and Delivery. 13. Additionally, instant request pertains to Request for Entry of Decision (probably dated from the end of March 2012), filed in instant court file by Attorney Ofer Ben Ari. A copy of such paper was provided to me during previous inspection, but the copy was unsigned and undated. Instant request is for the record itself and: a. The electronic signature, if created, User Name, date and time of signature. b. Audit data, pertaining to the entry of the paper into Net HaMishpat system, User Name and Login Time and Date, pertaining to the paper. 14. Additionally, I request to inspect the April 4, 2013 Protocol record, which was served on me with each and every page water marked in oversized letter NOT SINGED YET. Instant request pertains to the record itself, and: a. The electronic signature, if created, User Name, date and time of signature. b. Audit data, pertaining to the entry of the paper into Net HaMishpat system, User Name and Login Time and Date, pertaining to the paper. 15. Therefore, the Court is requested to serve me a decision on instant Request. 16. Since access to court records is a fundamental of Human Rights and Fair Hearings, Judge Esperanza Alon is asked to reason her decision in this regard, since other instances may be asked to review the matter.
