of this project. It will help your company articulate and identify overall goals. Each decision-maker should fill out his or her own survey. It is not necessary to answer every question. Some questions may not be applicable. Of those answered, please answer as thoroughly and clearly as possible. Also, please provide any supporting documents you think may be of value. We recognize that completing this survey will take some time.
