i apply to enroll the person named as a student of embassy. i agree to pay all fees incurred by the student as they become due, and either give the required notice of cancellation or to pay the required fees in lieu of notice. i agree to abide by the terms and conditions of embassy. i give permission for the administration of first aid and appropriate nonprescription medication to my child, and give permission to embassy and designated college representatives to seek medical, dental or optical treatment when required. i am the parent or legal guardian named. we, as parents/guardians of the undersigned student, do hereby authorize embassy and its assigned agents, and any designated program representative, as agents of the undersigned parents, to consent to any x-ray examinations, anesthetic, medical or surgical diagnosis or treatment, or hospital care which is deemed advisable by, and is rendered under, the general supervision of any licensed physician or surgeon, whether such treatment or diagnosis is rendered at the office of said physician or surgeon or at a hospital. it is understood that this authorization is not given in advance of any specific diagnosis, treatment or hospital care being required, but is given to provide authority and power on the part of aforesaid agents to give specific consent to any and all such diagnoses, treatments, or hospital care which the aforementioned physician or surgeon, in the exercise of her/his best judgment, may deem advisable. we, the undersigned, waive and release all claims against embassy and its agents for any injury, loss, damage, accident, delay or expense resulting from the applicants participation in the program. we also release embassy and its agents and agree to indemnify them with regard to any financial obligations or liabilities that the applicant may personally incur or any damage or injury to the person or property of others that the applicant may cause while participating in this program. we understand that the program begins with the outbound flight to the united states and terminates when the return flights lands. we realize that any airlines liability for loss or damage to baggage or property, or for death or injury, is limited by the terms outlined by the airlines. we also understand that it is our responsibility to obtain a passport and any required visa. we understand that embassy is not responsible for any loss or injury suffered by the applicant at any time while participating in the program, including but not limited to losses or injury suffered during periods of independent travel or absence from the program. if the applicant becomes ill or incapacitated, embassy or its agents may take such action as it considers necessary, including securing medical treatment and transporting the applicant home at his/her own expense. we release embassy from all liability related to such actions. we understand that the applicants participation in the program may be terminated at the discretion of embassy without any refund of fees and that the applicant may be sent home at his/her own expense if he/she does not adhere to national, state, and local laws and college/university rules. we agree to accept full responsibility for personal expenses, books, any required additional language training and round trip air fare. we understand that this agreement cannot be modified except in a writing signed by embassy.

Embassy Pathways Term and Conditions 2012

These Terms and Conditions should be read carefully and understood before signing and submitting the application. 1. Participant responsibility The price for the Embassy program does not include the following expenses: 1) Personal spending money 2) Meal and housing costs during the few times the university is officially closed usually limited to the Thanksgiving holiday in late November in the USA, the Christmas holiday (which normally lasts about four weeks) and Spring vacation (approximately 10 days). Should students require assistance finding alternative accommodation during these breaks if necessary, Embassy staff is available to help. 3) University textbooks and other necessary school supplies 4) Air fare 5) Graduate students are invoiced for tuition, insurance and campus fees only. Any room and board charges accrued by graduate students will be paid by the student directly to the university. 2. Payment of Fees Students are required to pay an enrolment fee and an accommodation deposit, where required, directly to Embassy at the time of application. In the USA, students applying for campus courses at Woodbury University and Manhattan College are required to pay a $500 deposit. Embassy will accept applications for students who already meet the English language requirements of our host campus institutions (Dean College, Fisher College, Manhattan College, Pacific Lutheran University and Woodbury University). A $500 deposit will be required at the time of application. Additional information may be required to augment the application. Full fees must be paid no later than 14 days before arrival. Students will not receive their pre-arrival information until full payment is received. Students with outstanding balances will be immediately placed on hold by the host institution, and may be barred from attending classes until payment is made in full. Payments by credit card after the due date will be subject to a fee of 2% of the total program value. Students with balances due after 30 days are subject to a $500 late payment fee. 3. Services The company reserves the right to change details of its services, including courses, facilities and course dates, where circumstances beyond the companys control necessitate such changes or where the number of enrolments is not enough to operate a course viably. 4. Course Entry Levels All Pathway courses require minimum levels of English for entry. If after the placement test, a student is found to be below the minimum level required to start the course, the school reserves the right to place the student in a class with fewer lessons and/or and a different curriculum. Where this results in extending the duration of a students course, Embassy may need to issue a new Certificate of Enrolment. Students may be transferred to the nearest centre running a lower level course. 5. Changes to Enrolments The company reserves the right to charge an Administration Fee (GBP50; USD80; AUD200; CAD80) each time course or accommodation details are changed or cancelled after a place has been confirmed. Additionally, when a change request for a previously confirmed accommodation is received less than 7 days in advance of the scheduled arrival date, an equivalent weeks accommodation penalty fee will be charged. These fees will not apply to upgraded or extended courses. 8. Accident & Medical Insurance Every student must have appropriate insurance and provide a document with the students name and date of coverage as proof. The company recommends that all students take our own StudyCare insurance which is tailored to the needs of international students. Cover under the StudyCare insurance policy does not commence until the insurance fees have been paid in full. StudyCare is not available to students within Australia. In the USA, the UK, and Canada, students not wishing to take StudyCare must present an equivalent insurance certificate on arrival. Students without insurance must take StudyCare. While Embassy provides StudyCare Insurance, some Colleges and Universities will require students to have a student health insurance policy directly from the institution instead. In Australia, it is compulsory for those on Student Visas to take Overseas Student Health Cover (OSHC) for the duration of the time they are in Australia. Students need to buy OSHC before coming to Australia, to cover them from when they arrive. This ensures they have adequate health care arrangements while studying in Australia. If the student chooses for Embassy to arrange OSHC for the first course/year only of their programme, it is their responsibility to either extend or take out OSHC for the rest of their time in Australia. They must provide evidence of cover for the entire study period before a Confirmation of Enrolment will be issued. 9. General Refund Policy All refunds will be sent to the account of the Embassy representative to whom the fees were originally paid, or to the individual from whom Embassy has received the original direct payment. If a students visa application is rejected after payment has been received all fees, excluding any cancellation fees detailed in the Cancellation Policy below, will be refunded within 28 days, provided a visa refusal letter is received by our International Admissions Centre at least 2 working days before the course is due to start and the student has fulfilled all requirements for a visa application. No refund will be made if a visa rejection is based on students failure to meet the necessary criteria for their chosen visa category. Students in San Diego, Los Angeles and San Francisco must sign a California State Enrolment & Refund Agreement form. Refunds will be processed according to these terms and conditions. Australia In Australia this agreement, and the availability of complaints and appeals processes, does not remove the right of the student to take action under Australias consumer protection laws. Refunds will be provided to students within 28 days if the student defaults and within 14 days if the school defaults. In the unlikely event that the school is unable to deliver a course in full, the student will be offered a refund of all the course money paid to date. The refund will be paid within 14 days of the day on which the course ceased being provided. Alternatively, the student may be offered enrolment in a suitable alternative course by the school at no extra cost. The student has the right to choose whether they would prefer a full refund of course fees, or to accept a place on another course. If they choose placement on another course, Embassy will ask them to sign a document to indicate acceptance of the placement.

If the school is unable to provide a refund or place the student on an alternative course our Tuition Assurance Scheme (TAS) provider will place them on a suitable alternative course at no extra cost. 11. Tuition Refunds before Arrival UK, USA and Canada Where cancellations are received in writing more than 14 days before the first course start date 100% of tuition fees will be refunded, but any courier fee, enrolment fee, UK Visa application fee (CAS) and added administration fees are non-refundable. Where cancellations are received in writing 14 days or less prior to the first course start date students will be charged GBP190 (UK), USD350 (USA) or CAD350 (Canada), in addition to the fees listed in this paragraph.

16. Visas Students should contact their local embassy, Consulate or High Commission to ensure they are allowed to enter and study in their chosen country. If a student does not possess or maintain a valid visa status in accordance with the visa conditions, then the students course will be terminated without refund.

Publicity and Promotional Activity Students and their parents or guardians, where applicable, agree that the students photo, quotes and details of achievements may be used for promotional purposes without written consent or notification.
