EVACUATION AND FIRE SAFETY PLAN


FIRE EVACUATION PLAN 1. Fire evacuation plans shall include the following: a. Emergency egress or escape routes and weather evacuation of the building is to be complete or, where approved. By selected floors or areas only. b. Procedures for employees who must remain to operate critical equipment before evacuating. c. Procedures for accounting for employees and occupants after evacuation have been completed. d. Identification and assignment of personnel responsible for rescue or emergency medical aid. e. The preferred and any alternative means of notifying occupants of a fire or emergency. f. The preferred and any alternative means of reporting fires and other emergencies to the fire department or designated emergency response organization. g. Identification and assignment of personnel who can be contacted for further information or explanation of duties under the plan. h. A description of the emergency voice/alarm communication system alert tone and preprogrammed voice messages, where provided. FIRE SAFETY PLANS 2. Fire safety plans shall include the following: a. The procedure for reporting a fire or other emergency.

b. The life safety strategy and procedures for notifying, relocating, or evacuating occupants. c. Site plans indicating the following: (a) (b) (c) The occupancy assembly point. The locations of fire hydrants. The normal routes of fire department vehicle access.

4. A list of major fire hazards associated with the normal use and occupancy of the premises, including maintenance and housekeeping procedures. 5. Identification and assignment of personnel responsible for maintenance of systems and equipment installed to prevent or control fires. 6. Identification and assignment of personnel responsible for maintenance, housekeeping and controlling fuel hazard sources. 7. Fire safety and evacuation plans shall be reviewed/updated annually or as needed because of changes to staff, occupancy or physical arrangement of the building. These plans need to be made available in the work place and posted for employee review. Copy shall be provided to the Base/Unit Fire Station. Fire Evacuation Drills 8. Drills are to be conducted at quarterly intervals. Records are to be maintained indicating the time, date, staff present, who conducted drill, weather conditions and the time to evacuate. Employee Training Program 9. Employees shall be trained in fire prevention, evacuation and fire safety as part of new employee orientation and at least annually. Records are to be kept on site and made available upon request.

Fire Prevention Training 10. Employees shall be apprised of the fire hazards of the materials and processes to which they are exposed. Each employee shall be instructed in the proper procedures for preventing fires in the conduct of their assigned duties. Evacuation Training 11. Employees shall be familiarized with the fire alarm and evacuation signals, their assigned duties in the event of an alarm or emergency, evacuation routes, areas of refuge, exterior assembly areas, and procedures for evacuation. Fire Safety Training 12. Employees assigned firefighting duties shall be trained to know the locations and proper use of portable fire extinguishers or other manual firefighting equipment and the protective clothing or equipment required for its safe and proper use. Seating Plan 13. A detailed seating plan shall include the information from the fire safety and evacuation plans as described above. Deviations from the approved plans shall be allowed provided the occupant load limit for the occupancy is not exceeded and the aisles and exit access ways remain unobstructed. Announcements 14. In theaters, motion picture theaters, auditoriums, sporting events and similar assembly occupancy used for non-continuous programs, an audible announcement shall be made not more than 10 minutes prior to the start of each program to notify the occupants of the location of the exits to be used in the event of a fire or other emergency. (Exception- In motion picture theaters, the announcement is allowed to be projected upon the screen in a manner approved by Fire Prevention). Evacuation Plans Are Investment in People 15. EVACUATION GUIDELINES Employees 17. Prepare and evacuate the building by way of the nearest emergency exit. Walk; do not run. Do not use elevators. 18. Close but do not lock all doors as you leave.

19. Before exiting through any closed door, check for heat and the presence of fire behind the door by feeling the door with the back of your hand. If the door feels very warm or hot to the touch, advise everyone to proceed to another exit. 20. In the event you are unable to exit the building: a. b. c. d. Remain calm; do not panic Remain low; crawl if necessary. Place a cloth, wet if possible, over you mouth to serve as a filter Signal for help from a window. Use a towel, clothing, sign etc.

21. Upon exiting the building and proceeding to the assembly area, remain at least 20 feet away from the building walls and overhangs. Do not block any driveways, as Fire Department personnel will need access to these areas. Employees are requested to report to their assigned assembly areas as defined by your Fire Monitor/Evacuation Plan. 22. The cessation of an alarm/departure of the fire department is not an "all clear" to re-enter the building as corrective measures may still be in progress. Stay clear of the building until your appointed fire monitor has advised you to re-enter the building/area. 23. Assist visitors during alarm/emergency situations. Visitors may not be aware of exits/alternative exits and the procedures that should be taken during alarm situations. Employees should calmly inform visitors of the proper actions to be taken and assist them with the evacuation. Fire Monitor (Pegawai /PTTK Api) 24. Collects information or verbal reports for occupants as to the cause of the emergency. Proceeds to the predetermined assembly area to meet the fire department. 25. Meets the Fire Department personnel. Inform fire personnel of any known facts pertaining to the alarm situation. If requested, assists fire department personnel with a walkthrough of the facility. 26. Informs Assistant Fire Monitor if further measures are needed and/or other pertinent facts.

Assistant Fire Monitor 27. Notifies others of evacuation.

28. Ensures the safe and orderly evacuation of the building. If applicable, ensure handicapped evacuation plan is properly implemented. Make quick assessments during evacuation. 29. Knocks on any closed doors in your area after checking for warmth or smoke; and then open the door to ensure proper evacuation of all personnel. Check bathrooms, lunch, mail, and copy rooms last, and advise occupant in these areas to leave immediately. 30. Notes any individuals who do not evacuate and report them to the Fire Monitor. If a problem arises when an area is evacuated, contact the Fire Department responders and advise them of the situation. Evacuate the area and hold all persons out until a representative from the Fire Department/OEHS has given the all clear to reenter the building. (NOTE: Individuals, in teaching facilities faculty members, should be assigned to exit areas to ensure individuals do not reenter the facility until the appropriate officials have given the all clear to reenter the facility. Individuals assigned to monitor the exit areas should remain at a safe distance from the facility. Discovering a Fire 31. At all times, when following any fire procedures, ensure that you are out of danger before trying to complete any emergency tasks. Fire monitors and building occupants are requested to put life safety before any other goal during fire emergencies. 32. If a fire or other emergency is identified: a. Shout FIRE! FIRE! FIRE! And pull the nearest fire alarm immediately if any. b. Try to put out the fire. c. Turn off the main switch. d. Close the doors and windows if needed. e. Call Main Guard Room advice the person in duty that there is a fire/emergency and location; building, floor, room #, etc f. Call Fire Station. g. Exercise the appropriate evacuation plan. 33. If the fire is trash can size or smaller, contained (not spreading) and you have been properly trained in the use of a fire extinguisher: a. Call to the nearest office neighbor to retrieve the nearest charged fire extinguisher. Do not leave the fire unattended. If the size of the fire grows beyond containment, follow the appropriate evacuation procedure. b. Upon receiving the fire extinguisher, use the procedures associated with acronym PASS

PULL pin from extinguisher and hold extinguisher 6 to 8 feet from the fire AIM the nozzle at the base of the fire SQUEEZE the trigger SWEEP the extinguisher hose back and forth until the spray puts the fire out completely, with no smoke left, or until the extinguisher is emptied. (extinguisher lasts approx. 8 seconds) 34. If the fire fails to extinguish, exercise the appropriate evacuation plan.

Fire Monitor (Pegawai/PTBK Api) - Additional Responsibilities 35. Makes decision to call for an evacuation of the building by activating a manual pull station alarm when an emergency occurs and no other alarm is sounded. Requests Fire Department to respond and assess the situation if unsure of the nature of the emergency or the need for an evacuation. Reports all incidents to the Office of Environmental Health and Safety. (OEHS) 36. 37. Coordinates appropriate education and training programs for all employees. Orients temporary staff to evacuation procedures.

38. Ensures employees have a fire and emergency evacuation plan accessible to them and/or a plan are posted in an established and readily accessible area. 39. Establishes and notifies all employees of assembly areas to be used during evacuations.

40. Maintains a current listing of the names and contact numbers for all Fire Monitors, Assistant Fire Monitors, etc. 41. Maintains updated copies of the evacuation plan. Contacts OEHS if there are any needed changes to the evacuation plan: building modifications, staff, etc. 42. Assists OEHS with yearly review of evacuation plan and safety inspections.

43. Ensures the evacuation plan is easily accessible to all employees, reviewed annually with all employees, reviewed annually and amended as needed, and the plan provides for evacuation of handicapped occupants. 44. Informs and ensures each instructor, for both day and evening classes, will review the exit plan at the first meeting of every class each quarter, semester or session. What to do Before a Fire 45. The following are things you can do to protect yourself, your family, and your property in the event of a fire: Smoke Alarms

Install smoke alarms. Properly working smoke alarms decrease your chances of dying in a fire by half.

Place smoke alarms on every level of your residence. Place them outside bedrooms on the ceiling or high on the wall (4 to 12 inches from ceiling), at the top of open stairways, or at the bottom of enclosed stairs and near (but not in) the kitchen. Test and clean smoke alarms once a month and replace batteries at least once a year. Replace smoke alarms once every 10 years.

Escaping the Fire


Review escape routes with your family. Practice escaping from each room. Make sure windows are not nailed or painted shut. Make sure security gratings on windows have a fire safety opening feature so they can be easily opened from the inside. Consider escape ladders if your residence has more than one level, and ensure that burglar bars and other antitheft mechanisms that block outside window entry are easily opened from the inside. Teach family members to stay low to the floor (where the air is safer in a fire) when escaping from a fire. Clean out storage areas. Do not let trash, such as old newspapers and magazines, accumulate.

Flammable Items

Never use gasoline, benzine, naptha, or similar flammable liquids indoors. Store flammable liquids in approved containers in well-ventilated storage areas. Never smoke near flammable liquids. Discard all rags or materials that have been soaked in flammable liquids after you have used them. Safely discard them outdoors in a metal container. Insulate chimneys and place spark arresters on top. The chimney should be at least three feet higher than the roof. Remove branches hanging above and around the chimney.

Heating Sources

Be careful when using alternative heating sources. Check with your local fire department on the legality of using kerosene heaters in your community. Be sure to fill kerosene heaters outside, and be sure they have cooled. Place heaters at least three feet away from flammable materials. Make sure the floor and nearby walls are properly insulated. Use only the type of fuel designated for your unit and follow manufacturers instructions. Store ashes in a metal container outside and away from your residence. Keep open flames away from walls, furniture, drapery, and flammable items. Keep a screen in front of the fireplace. Have heating units inspected and cleaned annually by a certified specialist.

Matches and Smoking


Keep matches and lighters up high, away from children, and, if possible, in a locked cabinet. Never smoke in bed or when drowsy or medicated. Provide smokers with deep, sturdy ashtrays. Douse cigarette and cigar butts with water before disposal.

Electrical Wiring

Have the electrical wiring in your residence checked by an electrician. Inspect extension cords for frayed or exposed wires or loose plugs. Make sure outlets have cover plates and no exposed wiring. Make sure wiring does not run under rugs, over nails, or across high-traffic areas. Do not overload extension cords or outlets. If you need to plug in two or three appliances, get a UL-approved unit with built-in circuit breakers to prevent sparks and short circuits. Make sure insulation does not touch bare electrical wiring.

Other

Sleep with your door closed. Install A-B-C-type fire extinguishers in your residence and teach family members how to use them. Consider installing an automatic fire sprinkler system in your residence. Ask your local fire department to inspect your residence for fire safety and prevention.

What to do During a Fire 46. If your clothes catch on fire, you should: Stop, drop, and roll - until the fire is extinguished. Running only makes the fire burn faster. 47.

To escape a fire, you should: Check closed doors for heat before you open them. If you are escaping through a closed door, use the back of your hand to feel the top of the door, the doorknob, and the crack between the door and door frame before you open it. Never use the palm of your hand or fingers to test for heat - burning those areas could impair your ability to escape a fire (i.e., ladders and crawling). Hot Door Cool Door Open slowly and ensure fire and/or smoke is not blocking your escape route. If your escape route is blocked, shut the door immediately and use an alternate escape route, such as a window. If clear, leave immediately through the door and close it behind you. Be prepared to crawl. Smoke and heat rise. The air is clearer and cooler near the floor.

Do not open. Escape through a window. If you cannot escape, hang a white or light-colored sheet outside the window, alerting fire fighters to your presence.

Crawl low under any smoke to your exit - heavy smoke and poisonous gases collect first along the ceiling. Close doors behind you as you escape to delay the spread of the fire. Stay out once you are safely out. Do not reenter. Call Fire Department.
