AFFIDAVIT OF LOSS OF SCHOOL SOLICITATION LETTER

I, Rona Dina Belo, of legal age, Filipino, single, and resident of 2184 Calle Azul Capitol Heights Phase 3, Brgy. Villamonte, Bacolod City, after being duly sworn according to law, hereby depose and state: That as a bona-fide student of the University of St. La Salle Bacolod, I was issued with yearbook's solicitation letter No. 7402; That I had discovered was lost by my solicitors last November 2012 and that the said solicitation letters could not be found; Despite diligent efforts exerted, I could no longer locate, find or recover the said Solicitation letter, and to the best of my knowledge, the same is truly lost; The said Solicitation letter has not been confiscated by any officer of the law or the issuing office or agency due to the violation of any law, statute, orders, rules or regulations; In the event that the lost Solicitation letter should hereafter be found or located, I undertake to forward or surrender the same to the Yearbook Office of the University of St. La Salle - Bacolod for appropriate disposal; I am executing this Affidavit to attest to the truth of the foregoing statements in lieu of the one which was lost. .
