Data Monitoring Form

The Royal Academy of Music is required to collect certain personal information (in codified format) for statistical purposes and to fulfil governmental and other requirements. The Data Monitoring form will be separated from your application papers and will have no influence on the result of your audition. References It is your responsibility to ask two separate referees to write a Music Reference and a General Reference. We suggest that you provide your referees with a stamped addressed envelope so they can post the completed reference to you or to the Royal Academy of Music. The references must be written by two different people, and we will not accept references from your family, other relatives or close friends. We need to have the original reference document with the referee's signature and date. Faxes or emails are not suitable as they cannot be authenticated. Word-processed references can be stapled to the Reference forms which should be completed and signed by the referee. Music Reference: This should normally be completed by someone who can write about your musical abilities, for example your current principal study teacher. General Reference: If you are still at school, this should normally be completed by your Head Teacher or other senior teacher. If you are at college or university, this should normally be completed by your tutor, but could also be from an employer. If you are applying for the MMus Programme, at least one of your referees should be able to comment on your academic abilities. References should be sent to the Royal Academy of Music by 1 November 2012, or you can include them with your application. All references must be in English; if your referee cannot write in English please include the original document and a translation. Master of Music (MMus) applications: If you are applying for MMus, you must include two pieces of academic written work on musical subjects, each should be between 1,000 and 2,000 words in length. If we do not receive any written work, your application will be transferred to the Master of Arts programme. If the Admissions Board decides that the MMus is not suitable for you, you may be offered a place on the Master of Arts programme instead. Concert Project MMus: In addition to the two pieces of academic written work, send a Concert Project in which you describe your plans and aims for the proposed Concert Project, and should indicate what preparations and researches would be needed in order to make the project successful. Performance and Research MMus: In addition to the two pieces of academic written work, include a Research Project Proposal.

Please book a test for a date in March 2012, and ask the test centre to send the result directly to the Academy by 1 May 2013 using the electronic report system - it usually takes fourteen days for this to be done. English tests must be booked as early as possible because there is considerable demand. Do not

Choice of Teacher (Preferred Professor) The Academy takes great care over allocation of students to teachers. You are welcome to express a preference and and we will consider this when the allocations are made, but the decision is at the absolute discretion of the Principal and Head of Department and will take place in the summer of 2013. Finance This section is compulsory and you should give full details of how you propose to pay your tuition fees and living expenses for the total duration of your programme of study. Year Abroad programmes may not be eligible for external funding support, and are not eligible for our entrance scholarships. 5. Principal Study Information You should include details of your most recent principal study music examination. If you are an international applicant, you should state the highest local examinations you have taken (for example, Jury exams). Please enclose a photocopy of the mark sheet. PAGE 3 6. Keyboard Skills and 7. Theoretical Studies Please complete these sections if you are applying for BMus or for Year Abroad. You do not have to complete these two sections if you are applying for a Masters degree. 8. Personal Statement (on a separate A4 sheet of paper) Please give details of your performing experience and reasons for applying to the Academy. If you are applying to transfer into the BMus programme for entry in Year 2 or Year 3, you should include a brief statement giving the reasons. 9. Your Audition Special Needs / Access Requirements The Academy is committed to ensuring that candidates with special needs, disabilities or learning difficulties are not disadvantaged in any way by the admissions process. Please discuss this with your audition centre administrator before the audition. PAGE 4 and PAGE 5 Do not write anything on these pages, but please include them when you send the application to the test centre. PAGE 6 11. Education and Academic Background Please complete these tables even when you are enclosing transcripts. Candidates for Undergraduate level (BMus or Year Abroad): you should provide details of the school-leaving examinations/ examinations you are aiming for, or which you have already taken, and any major examinations which will be taken between the time of application and entry to the Academy. You should list your high school [or equivalent] qualifications and, if possible, enclose a transcript of your most recent results, certified by your school or college (translated into English). If you have taken UK examinations, please list GCSEs, AS-levels, A-levels or equivalents. Candidates for Postgraduate level (MA or MMus): you should list relevant A-levels [or equivalent secondary school / high school examinations] and also provide details of your higher education (university/conservatoire) current studies and/or qualifications.
