Introduction
During 2012, the YJB is introducing updated planning forms to the secure estate, via training and eAsset, the secure estate case management system. For a number of years the secure estate has been recording sentence planning via sets of forms known as T or training forms and the Life Sentence Plan. As both sets of forms require updating, they have been replaced with a set of generic sentence planning forms that correspond to remand and sentence planning meetings. These are also supported, in the case of young people serving indeterminate sentences, by SPR paperwork from the adult estate that is regularly presented to the Parole Board in the case of adult offenders. There are three specific forms that YOTs are responsible for completing and supplying to secure estate staff to include in each young persons case files where applicable. The most significant of these forms is the YOT Sentence Planning for Parole form, which is specifically tailored to capture the risks and needs of young people who will serve indeterminate sentences. The form assists the YOT Case Manager to consider the areas that will need to be addressed before the Parole Board right at the start of the sentence. YOT staff should continue to contribute to remand and sentence planning meetings with information about the young person, with consideration given to the young persons Asset score and resettlement needs. YOT and secure estate staff will have tasks to carry out in relation to objectives and targets and resettlement actions. The T1:FR will no longer be in use as the sentence planning forms (such as the Final review) capture the key work and information that will follow the young person on their release into the community. YOTs may be sent the Final review paperwork to complete release details in preparation for the young persons release. A note about DTO early release and HDC early release schemes: YOT staff should receive Early release and HDC Recommendation paperwork alongside updated sentence planning paperwork from secure estate staff where relevant; each form has a section that asks for the YOTs view on the young persons eligibility for release. YOT staff unable to complete these sections should ensure their written views on the suitability of the young persons release under these schemes are sent through to the secure estate within the required timeframes. Documentation that supports release schemes, such as Annex C, D and Home circumstances details are available online.

YOT Case Managers Report


Purpose of the form
This form is designed to capture key information that the YOT may have about the young person and their circumstances before each planning meeting.

What is needed to complete this form?


The YOT Case Manager will need to be in contact with the young person, their family or carers, secure estate staff and professionals involved in the young persons case. The YOT Case Manager will need to know how the young person is progressing, not just against their objectives and targets, but also with their attitude and behaviour, particularly towards their remand or their offending. It is also important to include information about diversity, disabilities, and learning needs. This information is likely to come from other professionals or colleagues working within the secure establishment.

How has this impacted on their relationship to date? This section should include information about the relationship between the young person and their carer since the young person was placed in custody. This information should be based on the views of the carer/family and the young person. It should specifically consider the type of custodial outcome/s that the young person has received (e.g. remand or sentence).

This section is likely to change over the course of the young persons time in custody. 2.1 Have you any concerns regarding the young persons stay in this establishment? This section should outline any concerns that the YOT Case Manager has about the young persons stay in the particular establishment. These concerns may arise from the young person, the YOTs own observations or from discussions with secure estate staff or other professionals involved with the young person. This section is likely to be updated over the course of the young persons time in custody. 3.1 What work prior to sentence/remand has been carried out with the young person? This section should include any work that the YOT Case Manager, or any other YOT has done with the young person prior to their current placement in custody. This will include any interventions carried out either in the community or in custody in the last 12 months, as these will have the most relevance. It is possible that the young person has no previous offences and if so, the YOT should indicate this. This section should be completed in full the first time that the report is required. 4.1 How should the young persons offending behaviour be addressed in the sentence plan at this time? This section should include detail of the YOT Case Managers view of the young persons progress with their offending behaviour and what interventions should be in place at that time to support any progress made. This detail may be based on discussion with the young person and other professionals to understand whether there has been any impact on their offending behaviour. Areas that also might be relevant to consider will be factors that affect young people regardless of whether they are in custody or in the community, such as disabilities, diversity and learning needs. Addressing these needs will support the young person to move forward from their offending. This section is likely to be updated over the course of the young persons time in custody. 5.1 What other areas of work should be prioritised for the young person to complete during their stay? This section should include the YOT Case Managers views of any work that young person should complete. This will be based on the information that is known about the young person at the time, such as from discussions with the young person and professionals about their circumstances. Areas that should be addressed as a priority by the YOT Case Manager are the risks associated with the young person, both to themselves and to others. This section is likely to be updated over the course of the young persons time in custody.

6.1 What work has the YOT carried out since the last meeting? This section should include actions that the YOT has undertaken such as resettlement actions, any partnership work with the secure establishment and other agencies, and any other preparation. It may also include any work done directly with the young person.

Updating the YOT Case Managers Report


YOT practitioners may find that it is easier to have one version of this form for each young person, and then date stamp the new, additional entries before each meeting. This reduces duplication of information that has already been recorded. This approach will also be used by secure estate staff for remand and risk assessment paperwork.

Indeterminate sentence forms


This section summarises, for YOT practitioners, the completion of forms for young people who are serving indeterminate sentences. YOT Practitioners requiring additional support (outside of the guidance) to complete these forms should contact the YJB or staff working in specialist long term units in the secure estate. Case management staff in these units will be able to provide assistance. These forms are the same as those that are used in the adult estate. This ensures that young people who are transferred into the young adult (18-21) or adult estate have relevant paperwork that matches the OASys system.

POSTSR B
Purpose of the form
The purpose of this form is to provide a post sentence report of the young person, their offence and their risks after their court appearance and placement into custody. YOTs already complete a post-court report, however the format of the POSTSR B is important as it matches the paperwork used in the adult estate. This means that the analysis completed by the YOT will not be lost in the likely event that the young person transfers to the adult estate. YOTs should pass this form to secure estate staff on its completion to ensure that it will be included with paperwork that would make up the young persons Parole dossier.

What is needed to complete this form?


The YOT Case Manager should populate this report with all the relevant information that is available to them from the point at which the young person is sentenced. It may look very similar in content to the Post court report documentation. The form should be completed in time for the young persons Initial sentence plan meeting (at 10 working days) and does not require updating.

3.0 History and relationships This section should include information about any relevant domestic and historical data about the young person and a summary of their significant relationships. It should also include any relevant medical or psychiatric history that may be involved in the young persons offending. The following areas should be considered:

5.0 Victims issues This section should include information on all relevant victim issues, and a summary of consequences flowing from the offence, particularly if the offence was a domestic murder and children are being cared for. Any confidential victim information must be appended in a separate report. 6.0 Risk of serious harm This section should provide an assessment and analysis of any risk of serious harm to self and others, including:

For completeness, this section should not just focus on risk of serious harm but also on risk of harm in the wider sense as that information will give the clearest picture of the risks that the young person presents.

7.0 Special needs/minority groups This section should include information about any special needs that are relevant to sentence planning, with particular reference to gender, diversity, disabilities, ethnicity or beliefs. 8.0 Additional information This section should include any further information or other comments relating to the YOT Case Managers knowledge of, or contact with, this young person.

Updating the POSTSR B


This form does not require an update, once completed, it is included in the young persons paperwork within the Parole Dossier.

YOT Sentence Planning for Parole


Purpose of the form
The purpose of the report is to specifically identify all the factors that affect the young persons life, risk and offending behaviour. Once identified, the priority is to address these factors in the period leading up to the young persons Parole hearing. All practitioners working with young people serving indeterminate sentences should be focussing on preparing young people for their Parole hearing and doing as much work as possible to reduce their risk of harm to the public. The YOT Case Manager should use the information in this form to complete their Parole Assessment Report to the Parole Board (which will include a recommendation of whether the young person should be released).

What is needed to complete this form?


The report should include key information gained about the young person from the Asset, the RoSH, the POSTSR B, the Multi Agency (Lifer) Risk Assessment Panel and other assessments that have been made available to the YOT. This form must be completed by the young persons YOT Case Manager and should include information specific to the young persons indeterminate sentence, and set out their suggested long-term sentence plan. The form should be completed in time for the young persons 1st Sentence plan review (at four months) and updated annually throughout the young persons time in custody. The completed form (including all original and updated sections) should be included in the young persons Parole dossier and should be referred to as part of the YOT Case Managers report to the Parole board. These details will inform the young persons Parole report.

2a Brief summary This section should provide a summary of the meetings held with the young person and any issues raised by the young person during these meetings. These issues should be shared with secure estate staff where relevant to custody. Court Proceedings and Custodial History This section should include information about the young persons court proceedings relating to the offence/s and their custodial history. 3a Court proceedings This section should include the initial plea by the young person. It should also include information on whether the case went to trial, any relevant police intelligence or any other information relating to the court proceedings. 3b Is the young person appealing his/her sentence or conviction? This section should include information about whether the young person is appealing their sentence or their conviction (or both). Where relevant, it should also include detail of how any appeal may affect the young persons intervention plan; for example, whether it will delay interventions or other work. 3c Previous custodial episodes plus reaction and engagement This section should include brief information about any previous episodes of custody. This should include detail on how the young person reacted to custody, such as information about their engagement, compliance or risk during those periods. 3d Was the young person on remand prior to sentence? This section should indicate whether the young person was on remand and how long for. It should also include details of the young persons remand period and if there is anything relevant that may affect any intervention planning. Initial response This section should include information about the young persons response to sentencing and should be drawn from detail in the Post Court Report. 4a Reaction to conviction, sentence, family response and media interest This section should include information about the young persons views about their sentence and conviction, and their views about the reaction of others (if known) such as family members and the media. This may include details of the young persons verbal or physical reaction. The YOT worker should refer to the Post Court Report in order to complete this section. Engagement in the regime to date This section should outline the young persons engagement with secure establishment staff and the regime, with a view to noting any positive engagement.

5a Incentive levels This section should include information about the young persons current incentive level and detail about their previous incentive levels and the period of time spent on each. 5b Peer relationships This section should include information on whether the young person has formed positive peer relationships or not while they have been in custody. It should also detail whether the young person struggles with pro-social relationships; in this context peer pro-social relationships are those which are thought be having a positive effect on the young persons emotional wellbeing, and/or current behaviour and/or motivation to change their offending behaviour. Where the young person is struggling, this should be noted. 5c Relationships with staff This section should include information on the young persons relationships with establishment staff, healthcare professionals, YOT workers, advocacy representatives or other members of staff, where known. This may also include detail of whether they relate to a particular staff member strongly, showing engagement, respect, willingness to be open or good listening skills. Where the young person is having difficulties, this detail should also be included. 5d Engagement with education/training/activities This section should include information about whether the young person is engaging well or showing interest in areas such as education, training, intervention work or any other activities. This should also the young persons views on their engagement in activities and reasons why they are particularly interested in any activity. Protective factors 6 Protective factors This section should include information about any existing protective factors and whether there are any that could be further built on during the young persons sentence. It should also include detail of ways that the young person can be motivated. These factors should help to reduce the young persons risk of reoffending and improve their welfare. The following areas should be considered:

Accommodation Education, training and employability Financial management and income Relationships and available support Lifestyle and associates Interests and attitudes Skills

De-stabilizers 7 De-stabilizers This section should include information about any factors affecting the young person that may be a barrier to progress with their intervention plan and the reduction of their re-offending. Such barriers could include:

Court processes Personality factors such as low self-esteem or motivation Problems with family Lack of appropriate social networks to support the young person on release
This section should also include detail about how the effects of any destabilizing factors can be reduced. Emerging needs 8.0 Emerging needs This section should report any needs or risk factors that may have arisen since the young person has been in custody or that have come to light post sentencing. These factors may include:

Bullying others Victimisation from others Family contact Learning needs Diversity or disability needs
Information about offence/s for this sentence This section should include information drawn from the Core Asset and other relevant documentation highlighting all factors relevant to the circumstances of the young persons offence/s. 9a Brief summary of offence This section should include detail of the offence, drawn from the young persons core Asset, RoSH, PSR documentation or information shared at the MALRAP/MARAP meeting. 9b Victims This section should include information on all relevant victim details, issues and the relationship of the victim to the young person. Points that should be made should include whether the victim/s of the offence were chosen for a particular reason (e.g. because they belong to a particular group) or at random. Other important details will include whether the victim could be considered as vulnerable (e.g. being younger, smaller, having disabilities, being a repeat victim of the young person or others).

Please note that any confidential victim information must be appended in a separate report. 9c Significant immediate circumstances around the offence(s) This section should include information about the circumstances surrounding the offence which could be considered to have contributed to the offending. These circumstances may include details of whether the offence was:

Committed under the influence of a substance Committed with co-defendants, (with details of the co-defendants relevant to
the offence to be included)

Committed under the influence of friends or associates Committed due to a perceived provocation or other perception The result of learned behaviour The offence was part of a wider or re-occurring pattern of behaviour
9d Background context to offence This section should include information about any background and environmental circumstances which may have influenced the young person to commit the offence/s. This could include any information concerning the young persons family situation, their accommodation and neighbourhood, the young persons engagement with education, training or employment, or any experiences of loss or abuse. 9e Current attitude to offence This section should include information about how the young person feels about their offending and whether they are able to discuss their offending. This detail may include:

Whether the young person has accepted responsibility or is denying any


aspect of the offence

Whether they have displayed remorse Any changes in their personality Attitude towards offending behaviour Feelings toward their family, the victim or the community Feelings of grief and loss relating to their offence and its consequences
9f Does the young persons account differ to the CPS view? How? This section should include information about any discrepancies between the young persons account of the offence and that of the CPS. It should include detail of whether they dispute any aspect of the offence or their offending that the CPS has stated is relevant to the case. It should also include any details that the young person has stated are missing.

Likelihood of Re-Offending This section asks for a rating of the young persons likelihood of reoffending. It should also include detail of the YOT Case Managers views about the likelihood of reoffending. 10a What was the young persons Asset score at the point of sentence? This section should include detail of the young persons Core Asset score (completed on the latest date prior to their sentencing). It should also include detail of whether the scores were affected by the young person being in custody prior to sentencing. 10b Based on your professional judgment, what do you think this Asset score indicates about the likelihood of re-offending in the future? This section should include the YOT Case Managers view of the young persons Asset score in relation to their likelihood of reoffending. The YOT Case Manager should use their professional judgment to analyse whether the young persons risk of re-offending is accurate, or higher or lower than what is able to be captured in the Asset score, detailing any discrepancies and providing evidence where appropriate. 10c If the young person was released without any intervention, how would you assess their current level of risk of re-offending? This section should include the YOT Case Managers view of the young persons likelihood of reoffending, without any intervention. The YOT Case Manager should use their professional judgment to analyse whether the reoffending would be similar or linked to the current offence or different to the current offence and the level of seriousness of that re-offending. Risk of serious harm (from ROSH) This section asks for a rating of the young persons risk of serious harm. It should also include detail of the YOT Case Managers views about the young persons level of risk of serious harm to others. 11a If the young person was released without intervention, how would you assess their current level of risk of serious harm? This section should include the YOT Case Managers view of the young persons risk of serious harm to others, without any intervention. The YOT Case Manager should use their professional judgment to analyse whether the risk of serious harm would be a similar, higher or lower level if the young person was released. 11b Who would be at risk? This section should include information about any specific person or groups of people, whom would be at risk should the young person be released without intervention. It should also consider whether these people would be targeted by the young person (at just at risk generally). 11c Could this risk be managed in a community setting? If so, how? This section should include information on the YOT Case Managers view of whether it is possible to manage the young persons risk in any community setting (whether it is the young persons community or a different one). This

should include detail or what programmes or supervision would be required and should consider the type of issues that the Parole Board may have with releasing the young person, such as public protection. Risk of absconding This section asks for a rating of the young persons risk of absconding. It should also include detail of any history of absconding by the young person and the affects this may have on temporary release schemes. 12a Does the young person have a history of absconding from secure placements, care placements or from home? This section should include information about any history the young person may have of absconding from any placements or living arrangements. This may also include any comments from the young person about their attitude towards absconding. 12b How might this affect Mobilities/ ROTL in the future? This section should include the YOT Case Managers view of how any history of absconding by the young person will affect their eligibility for Mobility or ROTL schemes. This will depend on where the young person has been placed in custody and how each establishment manages these schemes. Security Concerns 13a Please give details of any security concerns in custody To complete this section, the YOT Case Manager should request a security report from the young persons establishment. It should summarise and provide detail on any security concerns that have arisen during the young persons time in custody. Areas that must be addressed to reduce risk prior to Parole application This section should include information about any specific issues and factors that are applicable to the young person and therefore should be addressed in their intervention plan, leading up to their Parole Board hearing. It also asks the YOT Case Manager to consider how the factors would be addressed by interventions. 14a Offending and violent behaviour This section should include information about the young persons history of violent behaviour. This may include:

Early onset violence Violence at home Violence at school Violence in other social settings
This section also requires information about any history of non-violent offending that the young person may have. This may be in list form, if the list is extensive.

14b Home life This section should include any available information about the following home life factors; and note where there is no evidence of the factor being present for the young person:

Witnessing/exposure to family violence in a family or care context


o Outline any exposure o Did the young person receive any support? o Do they need support now?

History of family/care-giver involvement in criminal activity


o Outline any involvement in criminal activity o How has this affected the young persons attitudes? o What intervention might be effective here?

This section should include any other information about the young person that should be addressed in order to reduce their risk of re-offending before Parole. Detail should include whether these issues have been previously addressed and whether this earlier intervention was successful. It should also include detail of the impact of these concerns on the young persons offending.

Addressing Risk Section This section asks the YOT Case Manager to consider each of the areas outlined in section 14 and analyse the level of impact that each area would have on the young persons risk reduction for Parole, if not addressed. It also asks at what point the risk should be managed and why this is the case.

There may be a number of different issues, levels of impact or timescales within one area so a-k may have additional rows added to reflect this. 15 Impact level on risk reduction if not addressed This section asks the YOT Case Manager to assess the level of impact in each area, should no intervention take place:

Low this area would have little or no impact on the young persons reduction
of risk, if not addressed

Medium this area would have some impact on the young persons reduction
of risk, if not addressed

High this area would impact on the young persons reduction of risk, if not
addressed 15 When does intervention need to start? This section asks the YOT Case Manager to indicate when intervention work should start in each applicable area:

months or within the next year

Long term intervention work in this area should begin later in the young
persons sentence, possibly as a result of the sequencing of interventions

Continuous intervention work in this area will need to take place throughout
the young persons sentence, possibly as a result of the nature of the intervention or the issue itself

Not at all intervention work is not required in this area to support the
reduction of the young persons risk prior to Parole there may be no issue at all. 15 Explain reason for answer What exactly needs to be addressed? This section asks the YOT Case Manager to explain their ratings/timescales and to state what exactly needs to be addressed in each area. This detail may include suggested interventions or programmes that are available to support the young person in these areas. This explanation will lead into the Recommended Intervention Plan. 16a Which are the most significant static and dynamic risk factors that need to be addressed during the custodial period? This section should outline the young persons most significant static and dynamic risk factors that should be addressed. The most significant factors will be ones that must be addressed to ensure that the young person, the public and victims are protected should the young person be released from custody. This information should primarily be drawn from the young persons Asset documentation.

16b Are there any factors that it is not possible to address which will remain a factor on release? This section should include detail of community-based factors that may affect the young person, none of which can be addressed while the young person is in custody. These factors should be noted in the young persons sentence plan as resettlement actions for the YOT Case Manager to make arrangements for should the young person be released on Parole. 16c What risks could be addressed/assessed/reduced through a Mobility Programme This section asks the YOT Case Manager to identify risks that would specifically be addressed through a Mobility Programme this view should be evidencebased. It should also include information about how the programme will support the young person. This detail should be used to support any application that may be made on the young persons behalf to undertake a Mobility Programme. Recommended action for further risk assessment This section asks the YOT Case Manager to identify risk areas that need further assessment, the particular assessments that may be required and who will carry out the assessment. This detail should be discussed with secure establishment staff. Recommended Intervention Plan This section should include information about all the recommended interventions that the YOT Case Manager considers should be completed during the young persons time in custody. These interventions should address the areas of risk, need, welfare and safety that the YOT Case Manager has identified throughout the report. 18a General This section should list all the interventions that the young person should have, (please note that more specialist interventions are covered in section 18b below) with the following categories outlined:

For instance should an intervention be recommended in the area of a young persons relationships, the table would read:

Sentence Planning Meeting Minutes This section should include the main comments and actions arising from the sentence planning meetings that took place prior to the form being completed. It should detail the YOT Case Managers view of the discussion about the young person and their needs and progress. It should also outline who attended the meeting, their contact details and actions they are responsible for completing. In the first instance of the form being completed, the Initial sentence plan meeting will be included in this information. Please note that similar information will be included in the young persons sentence planning paperwork therefore this section is optional but may be useful for the YOT Case Managers notes and for general completeness of the information.

Updating the Sentence Planning for Parole form


This form should be completed prior to the young persons 1st Sentence plan review meeting. This form should then be updated annually from that point to ensure that the long-term sentence plan is able to reflect any progress that the young person has made against any of the areas of concern that were highlighted in the report. The latest version of this form should be sent to the young persons assigned secure estate Case Supervisor/worker to be included in their Parole dossier. Please note that opinions on the young persons progress against the plan should be recorded in their sentence planning paperwork and the official report that the YOT Case Manager supplies to the Parole Board; this opinion should be based on progress recorded in all sentence planning paperwork relevant to the young person.
