This Employment contract is executed and entered into by and between: A. Employer: DAVAO CITY GOVERNMENT Address: City Mayors Office, City Hall Building, San Pedro Street Davao City Tel. No.: (082) 2258398 B. Employee: Kristine Mae Mascarinas Quibod Civil Status: Single Address: 123 St. Paul Street, Dona Pilar Village, Sasa, Davao City 1. Employees Position: Secretary to the Head of the Legal Department of the City Mayors Office 2. Basic Monthly Salary TEN THOUSAND PESOS (P10,000.00) 3. Regular Working Hours: Maximum of 8 hours per day, five days per week. 4. Overtime Pay: a. For work over regular working hours: ONE HUNDRED PESOS PER HOUR (P100.00/hr)For work on designated rest days & holidays: TWO HUNDRED PESOS PER HOUR (P200.00/hr) 5. Leave with Full Pay: a. Vacation Leave: FIVE HUNDRED PESOS per day of vacation leave (P500.00/day) b. Sick Leave: FIVE HUNDRED PESOS per day of vacation leave (P500.00/day) 6. Free food or compensatory allowance of TWO THOUSAND PESOS PER MONTH (P2,000/month) 7. Free emergency medical services and facilities including medicine. 8. Personal life accident insurance in accordance with Philippine government laws 9. Termination: a. Termination by Employer: The employer may terminate this Contract on the following just causes: serious misconduct, willful disobedience of employers lawful orders, habitual neglect of duties, absenteeism, violation of the terms of this Agreement. b. Termination by Employee: The employee may terminate this Contract without serving any notice to the employer for any of the following just causes: serious insult by the representative of the employer, inhuman and unbearable treatment accorded the employee by the employers representative, commission of a crime/ offense by the representative of the employer. i. The employee may terminate this Contract without just cause by serving one (1) month in advance notice to the employer. The employer upon whom no such notice was served may hold the employee liable for damages.

c. Termination due to Illness: Either party may terminate the contract on the ground of illness, disease or injury by the employee. The employer shall shoulder the cost of repatriation. 10. Settlement of disputes: All claims and complaints relative to the employment contract of the employee shall be settled in accordance with the policies, rules and regulations of the city government. 11. The employee shall observe employers rules and abide by the laws of the Philippines. In witness thereof, we hereby sign this contract this 2nd day of January, 2013 in City of Davao, Philippines,
