Pre Incident Plan (PIP) Type of Emergency/Scenario: Vapour Cloud Explosion (VCE) due to leakage from Condensate Storage Tank (Tank 445 and 448) Location of Emergency/Scenario: Terengganu Crude Oil Terminal (TCOT) Vapour Cloud Explosion (VCE) due to leakage from Condensate Storage Tank (Tank 445 and 448) RESPONSE OBJECTIVE(S):
1 To secure all personnel from the affected area and headcount to be executed 2 Notify CEFS Response 3 To activate the first line fire fighting system prior arrival of CEFS response 4 To mobilize the DCT to initiate manual first line fire fighting system prior arrival of CEFS response 5 To establish communication with PMO Incident Commander for potential support

IMMEDIATE RESPONSE
1. CCR Panelman

ACTION
1.1 Upon being notified by witness/Fire & Gas Panel about vapour cloud explosion at any Condensate storage tank, immediately notify On Scene Commander (OSC) 1.2 Notify CEFS Response 1.3 Activate the siren to inform all work site personnel 2.1 Instruct CCR Panelman to activate ESD, start the fire water pump and first line fire fighting system 2.2 Proceed to Emergency Operation Centre (EOC) 2.3 Establish contact with Damage Control Team Commander (DCTC) 2.4 Establish contact with Muster Coordinator on headcount status

REMARKS
The potential hazard which could lead to a major accident is the loss of containment of one (1) of the Condensate Tank into the bund area, causing a vapour cloud to form around the tank. Each tank is provided with three (3) fire water hose connections at the fire access road and a foam tank to supply foam in the event of fire. In addition, there is a separate fire hose connection at the fire access road to supply fire water to the gauging platform. The vapour cloud will be blown away from the process area since the prevailing wind directions are Northeast East and South Southwest. The Control Room is located towards the North.

ACTION
1.1 Mobilize to affected tank and activate the first line fire fighting system such as foam system, fire monitor, fire water hose connection at while waiting CEFS arrival. Secure any expose personnel away from the scene. 2.1 Notify PMO Incident Commander (PMO IC) 2.2 Follow up with DCT Commander on current situation and potential incident escalation 2.3 Notify security personnel to control traffic and prevent unauthorized entry to terminal

2ND RESPONSE
1. Damage Control Team Commander (DCTC)

ACTION
1.1 Once CEFS arrives at TCOT, assist them in set up the Fire Fighting Equipment including Fire Water hose, Foam System/Fire adapter etc 1.2 Update OSC on the emergency status 2.1 Follow up with Muster Coordinator to check the headcount status and any potential missing person. If any person found missing, notify DCTC for potential search & rescue 2.2 Continue updating PMO Incident Commander. CEFS will notify OSC once the emergency beyond their control and Jabatan Bomba & Penyelamat will take charge. Prepare to handover the OSC roles to Jabatan Bomba & Penyelamat instructed by Bomba. 2.3 If incident is under control (without any interference by Bomba), declare stand down and update PMO Incident Commander on the status 1.2.1 Radio 2.1.1 Radio

REMARKS
CEFS Response will usually arrive at TCOT about 4 6 minutes after being notified by TCOT ERT about the Fire Incident. Specific tactical Fire Fighting for each TCOT Tank (445 & 448) has been designed by CEFS response. TCOT ERT will provide necessary assistance to CEFS Team whichever necessary to set up the equipment. In the event of fire emergency at KIPC, CEFS will notify Bomba for potential support. Bomba will ready at CEFS Response staging area (Main station, Paka).
