Risk Management and Contingency Plan

The students involved with this event are acting as volunteers on behalf of the Salt Lake Climbers Alliance (SLCA). The event, Send to Defend, will take place on December 1, 2012 at The Front Climbing Club in Salt Lake City. Through our agreement with both the SLCA and The Front, all liabilities will be transferred to both of these organizations.

Goal
Create an efficient plan, which analyzes all aspects of the event that may be susceptible to any and all forms of risk.

Objectives
Be mindful of all risk that may occur prior to and occurring on the day of the event Obtain The Fronts regulations and Site Safety Plan (Appendix 1) to be aware of their protocols and procedures in case of an emergency Ensure that all students involved fill out the necessary forms and releases o The Fronts Liability Waiver (Appendix 2) o Volunteer Release Forms (Appendix 4)

Site and Facility Development


The Front Climbing Club is located just south of downtown Salt Lake City at 1450 S 400 W. The Fronts 10,000 feet of climbing wall was constructed with bouldering as its main emphasis. Despite this being its primary purpose, they do provide sections for top-roping. By having such an expansive area to climb, The Front is able to meet the needs of all climbers, from beginners to competitive athletes. The club offers a wide array of programs designed to better educate the climbing community on proper climbing technique and skill development. As a way to expand their involvement within the community, The Front also offers Pilates and yoga classes. The Front fully understands that the sport of climbing would not be possible if it were not for the natural environment that enables it. With this in mind, they have designated themselves as stewards to help protect the natural environment. This process has required the facility to make extensive upgrades. Some of which are adding solar panels, recycling glass at their facility and offsetting other power sources with wind power.

Supervision
All participants will be required to sign The Fronts Liability Waiver Form prior to climbing. Spectators will not need to sign liability waivers; they must remain off all crash pads at all times. The Front Staff will be in the gym at all times supervising climbers.

The Front Staff will be in the gym at all times ensuring that spectators remain off all crash pads. SLCA volunteers will be supervising the silent auction and food. All injuries will be reported to The Front employees and management. No SLCA volunteer will assist in aid to injuries.

Rules and Regulations

All participants sign Liability Waiver for The Front prior to entering the gym. There are different routes of difficulty posted throughout The Front Climbing gym. By distinguishing these routes it ensures the safety of participants by staying on the route that suits there technical level the best. Any damages done to The Front Climbing Club during Send to Defend will be reported to The Fronts management and that party shall be held liable.

Incident Reporting
All incidents will be reported to The Fronts management. Incident reporting will take place after incident is reported (See example in Appendix 1).

Injury
All participants at The Front have signed waiver forms acknowledging the potential risk of injury or even death at The Front not excluding the day of Send to Defend. Prior to Send to Defend, SLCA volunteers have been made aware of The Fronts Site Safety Plan and know to contact an employee or manager in the case of an injury.

Roles of First Responder


SLCA volunteer will notify The Fronts staff (See Appendix 1, First Responder).

Emergency Exits
The Front Salt Lake City has ten total and eight usable exits. The clubroom and lobby share two exits, one in the front and one in the back of the building. Staff and customers must exit using the front exit. The back door is fenced in and may result in further injury in attempts to find safety.

The climbing area is equipped with four emergency exits. The party room and Vertical Solutions storage room each have one exit. There is one exit near the locker rooms and another near the back stairwell. Persons must use the exit located near the locker room. The exit near the stairwell leads to the same fenced in area as the back clubroom exit and may create the same risks.

Handling Donations Money


If a participant approaches a SLCA volunteer wishing to make a private donation, the SLCA volunteer should direct the participant to the SLCA booth or to The Fronts front desk where they will be accepting donations. SLCA volunteers are only permitted to accept donations in the form of a check written out to the Salt Lake Climbers Alliance. Upon receiving a check, the SLCA volunteer will take the check directly to the SLCA booth. SLCA volunteers should not accept any cash donation for any reason. Cash donations can only be made at The Fronts register or directly at the SLCA booth.

Silent Auction
Silent auction through a private entity such as The Front and SLCA will not need any permitting through Salt Lake Sheriff Department. All money collected through the silent auction will be handled through The Fronts front desk or the SLCA. No volunteers will be collecting money for the silent auction. If silent auction items do not get bid on then the items will be donated to the SLCA for their use in future events.

Opportunity Drawing
Opportunity drawings will not require any permitting through the Salt Lake County Sheriff Department.

Photo
A disclaimer has been placed at the registration desk at The Front. All participants have the option to not have their photo taken and published. But if their photo is taken the photographer has received permission to use their photo.

Food Late Delivery


If food does not arrive on time, then the company should be contacted to receive and estimated time of arrival. If it is evident that the food will not arrive when expected, a volunteer will be designated to pick up the order. If the pick up or delivery is not possible, then the event will go on without food. What money was going to food purchases will be reallocated to the SLCA in form of a donation.

Food Handlers
Within the SLCA volunteers there are several members who are registered food handlers. Some of whom are Rashel Fox, Rainy Butler and Hanna Murphy. If applicable, these volunteers will be responsible for the distribution of food. Salt Lake City accepts food handlers permits from all counties within the state of Utah.

Washing Hands
Post eating participants are expected to wash hands. The day of the event signs will be posted around gym notifying participants to wash hands. This will prevent handholds from getting slippery and participants from getting injured.

Facility Loss of Electricity


In the case that there is a loss of electricity on the day of the event, we will use The Fronts backup generators. If these generators are unavailable, the event will proceed as planned so long as The Front permits it.

Fire
If The Front has a fire prior to the event, the event will be moved to The Fronts Ogden location at 225 20th Street, 0gden, Utah. If there is a fire during the event, ensure that all spectators and participants evacuate the building. (See Appendix 1)

Parking
The Front has designated parking immediately north of the facility. If there is no available parking in the street east of the facility, Wal-Mart has approved the southwest corner of their

parking lot for use on the day of the event. Approval for overflow parking came from Wal-Mart Store Manager Roger Daniels. He is available for contact prior and during the event.

Absences Music
On the day of the event, if the DJ is unable to attend because of unforeseen circumstances, the SLCA volunteers will develop a playlist and play all music through The Fronts speaker system. In such an instance there will be a volunteer designated to operate the music for the event to ensure that there is a positive high energy feel to the event. This volunteer will also keep participants and spectators notified as to what events are occurring and what is next on the event agenda. These events include the silent auction, opportunity drawings, and presentations.

Presentation Equipment
On the day of the event, if the PA system or projector is not in working condition then the event will go on without these tools. SLCA volunteers and the presenter will need to get the attention of the crowd by being very vocal. The presenter will need to be more vocal and descriptive during their presentations.

Presentations
On the day of the event, if the guest speaker is absent, the event will proceed. The SLCA has committed to deliver a presentation regardless of Lynn Hills presence. This presentation will better educate the community and increase the communitys awareness of safety and accessibility issues in the area.

Appendix 1 The Fronts Site Safety Plan Accidents and Incidents Emergency Action Plan Roles of First Responder Gym Directions Staff to Be Notified Other Important Numbers Evacuation Fire Location of Fire Extinguishers Flooding Water Shut-off Valve/ Water Main Loss of Electricity ACCIDENTS AND INCIDENTS Most of accidents at The Front are sprained ankles. Regardless of the type of injury or accident, the procedure is the same: if the incident is reportable, follow these steps. (If you are unsure about whether an incident is reportable or not, REPORT IT.) Assess the situation. How severe is the injury? Can the person walk off the pads and onto a couch unassisted? Or do they need help to get there? Is the incident severe enough that moving the injured person would be a bad idea? If the injury is minor and the gym is busy, escort them to a couch or any safe, comfortable place. If the injury is substantial (and the injured person cannot walk or resume climbing), the employee on duty should notify a manager immediately. In the case of severe injuries (and whenever feasible), the employee should take pictures of the accident scene. Photos should be submitted with the Incident Report Form: Once the scene has been assessed and the person removed to safety (if possible), the employee should utilize basic first aid procedures. Remember RICE: Rest, Ice, Compression, and Elevation. Instant ice packets are available in the first aid kit, and old fashioned ice is also available in the freezer. If the person wants Ibuprofen, you can find this in the first aid kit, and provide it to help reduce swelling. Once first aid has been administered, locate an Incident Report Form, available behind the front desk. Fill this out completely. Record as much information as possible detailing what happened: where, when, what, and how, as well as witnesses present, etc... On the Incident Report Form, get a witness account of the accident as well as your own account of what happened. Document your assessment of the incident as well as any facility or procedural improvements that could be made to help prevent this type of incident from reoccurring.

Speak with the injured participant about their treatment and transportation options. They may want a friend to drive them to a hospital, or let the injury heal on its own. If the injury is severe, an ambulance may need to be called. Employees should take all necessary actions to ensure the safety and health of the injured person. Unless otherwise determined, it is the responsibility of the employee on duty during the incident to follow up with the injured party a few days afterward. Call them and to ask how theyre doing and whether theres anything we can do for them. Make it clear that we care for them. Let the manager know what you did to follow up with the injured person, and record this on the Incident Report Form. After the follow-up conversation, sign and date the form, and leave it in the Completed Box for a manager to review. Note: all Front employees must be CPR and First Aid certified in order to effectively handle any incidents that may occur. Retrieve emergency equipment. 3. Activate the emergency medical system (EMS). Assess and describe the condition of the injured (e.g. conscious, unconscious, breathing, not breathing). Describe the first aid treatment already given (e.g. controlled bleeding, stabilized head, splinted leg, CPR). Give directions to The Front Climbing Club (assuming they will come from downtown): Go west on 1300 South. Turn left/south onto 400 West (behind Walmart Supercenter)

Continue south on 400 West for 0.2 miles. Front Climbing Club is on the right/ west side of the road Provide any other information, as requested.

4. Open doors and direct EMS to the scene. Designate a person to open the appropriate doors to expedite EMS entry Designate an individual to flag down EMS on the street, and direct them to the scene. 5. Control the scene. Limit the number of first aid providers Move bystanders away from area. Depending on severity, you may need to close the gym. GYM DIRECTIONS The Front Climbing Club is located on 1450 South 400 West, southwest of the Walmart Supercenter, and adjacent to Skyline Event Company and Insco Steam Cleaning. We have our own parking lot. The building entrance is located in the far southwest corner of the lot, in the L shaped corner of the building. Staff and customers must exit using front entrance, NOT the back door leading to the dog run; the dog run is surrounded by locked fences that could trap people attempting to exit through this door. The climbing area is equipped with four emergency exits. The party room and Vertical Solutions storage room each have one exit. There is one exit near the locker rooms and another near the back stairwell. Persons must use the locker room-area exit, NOT the back stairwell exit, as the door near the back stairwell also leads to a locked yard. FIRE If there is a fire, locate the fire extinguishers. If the fire is small enough, try to put it out. If the fire is too big, or you cannot put it out, call 911, and help all customers and members exit the building.

LOCATION OF FIRE EXTINGUISHERS There are three fire extinguishers in the building: One at the front desk One in the cardio area upstairs One in the climbing area near the gym bathrooms. Fire extinguishers are checked and serviced once a year each autumn. FLOODING The Front is located in a flood plain. If a major flood occurs, call 911 and the CEO. The parking lot drains slowly, and may back up as snow melts and in heavy rain. If the building seems threatened by this type minor flooding scenario, contact a manger. WATER SHUT-OFF VALVE/ WATER MAIN The water shut-off valve is located behind the top-roping wall. To access this, walk through the door near the campus board (on the east end of the climbing wall), and look for a vertical pipe running floor to ceiling. Near the ground there is a green handle oriented parallel to the pipe. To turn the water off, turn the handle perpendicular to the pipe; water will stop flowing into the building. LOSS OF ELECTRICITY Circuit breaker boxes are located throughout the gym. There are four boxes: One in the setters area behind the climbing wall Two behind the top-roping area One in the Pilates studio

The Front Climbing Club and any of its employees, directors, officers, agents, representatives, or assigns (hereinafter The Front) reserves the right in its sole discretion to refuse entry, or to remove the participant named above, including his/her Guest (Participant) from The Fronts property (the Facility) or from participation in any activity organized by The Front ou tside of the Facility, including but not limited to portable wall climbing or guided instruction in the outdoors (hereinafter Outside Activities) for any reason whatsoever. Any such refusal of entry or removal by The Front shall be in its sole discretion. 2. The Front shall not be responsible for the safekeeping, loss, theft or damage of any Participants property or the property of any Guest of Participant that is brought into the Facility or brought on any Outside Activity. 3. Participant shall comply with and observe all rules and regulations of The Front and the terms and conditions of this Agreement at all times and shall assure that his/her Guest(s) complies with and observes the same. 4. Should Participant or his/her Guest(s) damage or break any of The Fronts equipment or property, Participant shall be liable for the reasonable cost of necessary repairs or replacements to such equipment or property. 5. The Front is authorized by the Participant to use, store or transfer, as The Front may consider necessary, the Participants personal information, for any and all purposes in connection with the Facility and services provided by The Front and/or for the purpose of promoting, improving and furthering the interests of The Front. Participant acknowledges this may include posting their photos and/or names online, and/or in publications. 6. Any delay or failure by The Front to exercise its rights and/or remedies under this Agreement does not constitute a waiver of any of such right or remedy.

7. The terms and conditions herein (as amended from time to time) constitutes the entire agreement between the Participant and The Front (other than the Fronts Membership Agreement if Participant is a Member) regarding the Participants use of the Facility and supersedes all previous agreements, understandings and arrangements, written or oral, between the Participant and The Front in relation to such matters. 8. In the event of an emergency, The Front is authorized to notify the person(s) listed under Participants emergency contact information. THE FRONT LIABILITY WAIVER I (Participant) understand that roped climbing, bouldering, weight lifting, cardiovascular training, yoga, and other activities now available or that may become available at the Facility in their various forms, as well as preparation for participation in, coaching or spotting, volunteering, and all other aspects involved with these activities (collectively referred to hereinafter as Activities) are inherently dangerous activities involving many RISKS, DANGERS, AND HAZARDS. These risks, dangers and hazards include, but are not limited to, falling, collisions with objects, people or structures, being struck by other participants or objects, loose holds or other equipment failure, the actions of other participants including negligence or inexperience of the Participants partner(s), overuse injuries, the aggravation of preexisting conditions, or other foreseeable or unforeseeable events or circumstances. I understand that INJURIES OF ALL TYPES ARE A COMMON AND ORDINARY OCCURANCE of the Activities. I know that the risk of SEVERE INJURY and even DEATH exists in the participation of the Activities. I also understand that maintenance of the Facility and equipment, training, coaching, instruction, supervision, enforcement or lack thereof of any rules or regulations, route setting, or any added safety measures (hereinafter Associated Activities) by The Front, its subsidiaries, affiliates, officers, directors, employees, volunteers, agents, coaches, instructors, contractors, representatives, competition organizers and sponsors, and equipment providers do not and cannot guarantee my safety. I (Participant) hereby represent that I am fit and suffer from no adverse health condition or effect that would limit my ability to participate in any Activities offered by The Front. I (Participant) hereby represent that any of my own equipment that I use at the Facility is safe and in no way shall The Front be liable for any damages caused to myself or a third party for any failure of any such equipment. I (Participant) hereby represent that I have conducted a thorough visual inspection of the Facility and equipment I will be using and I am aware of any potential hazards associated with the Facility and/or such equipment. With full knowledge and understanding of the RISK OF SEVERE INJURY AND DEATH involved in the Activities and the Associated Activities, I FREELY AND VOLUNTARILY ACCEPT AND FULLY ASSUME THE RISK THAT I MAY SUFFER TEMPORARY, PERMANENT, OR EVEN FATAL INJURIES, even if I follow the instructions or advice of The Front. APPENDIX 4 SLCA VOLUNTEER FORM Realease In consideration of The Fronts acceptance of my membership application or day use of the Facility, and in spite of the risk of severe or permanent injury or even death, the undersigned agrees as follows: 1. I (Participant) hereby unconditionally WAIVE AND RELEASE ANY AND ALL CLAIMS AND CAUSES OF ACTION OF ANY KIND OR NATURE AGAINST THE FRONT, AND ANY MANUFACTURERS OR DISTRIBUTORS OF EQUIPMENT USED BY THE FRONT, RELATED IN ANY WAY TO THE ACTIVITIES OR THE ASSOCIATED ACTIVITIES. THIS WAIVER AND RELEASE INCLUDES BUT IS NOT LIMITED TO ANY SUCH CLAIMS OR CAUSES OF ACTION, present or future, related to injury or damage to Participant, his/her property, or to any other person or property, for any loss, damage, expense or injury (including DEATH) suffered by any person from or in connection with Participant engaging in any Activities and from Associated Activities, due to any cause whatsoever, INCLUDING NEGLIGENCE and/or breach of express or implied warranty on the part of THE FRONT. 2. Participant agrees to hold harmless, defend and indemnify The Front from any claim or action, present or future, related to injury or damage to Participant, his/her property, or to any other person or property, for any loss, damage, expense or injury (including DEATH) suffered by any person from or in connection with Participants participation in any Activities and from Associated Activities, due to any cause whatsoever including negligence and/or breach of express or implied warranty on the part of The Front. 3. Participant hereby RELIEVES THE FRONT OF ANY DUTY TO PROTECT PARTICIPANT FROM HARM in connection with any Activities, Outside Activities or Associated Activities in which The Front is involved in any way. 4. In the event Participant does suffer any type of damages or injury, Participant shall notify The Front immediately of any such occurrence or cause. 5. Participant authorizes The Front to stabilize, obtain medical care for, or transport him/her to a medical facility or hospital if, in the opinion of The Front medical attention is required and Participant is unable to make such decisions for himself/herself. Participant agrees to pay all costs associated with such medical care and related transportation and shall DEFEND, INDEMNIFY AND HOLD HARMLESS The Front of and from the consequences of such decision and from any such costs incurred relating to the provision of medical care. Participant also authorizes disclosure of any protected medical information in the possession of The Front that is necessary to provide, coordinate or manage members healthcare consistent with the dictates of HIPAA and to the extent that such use or disclosure is required by law. 6. This Liability Waiver shall continue in effect in perpetuity so that each time Participant or Member uses the Facility or participates in any Outside Activities from the date this waiver is signed forward he/she shall be bound by the terms and conditions herein. 7. If any provision of this Agreement or the application of any such provision to any person or circumstance is held invalid, the remainder of this Agreement, and the application of such provision other than to the extent it is held invalid, will not be invalidated or affected thereby.

8. This Participant and Liability Waiver Agreement shall be governed by Utah law, without regard to conflicts of laws principles. In addition, Member agrees that jurisdiction and venue for any action or claims arising out of or relating to this Membership Agreement shall be in the United States District Court for the District of Utah located in Salt Lake City, Utah or in the Third District Court of the State of Utah.
