Navigator Grant Recipients

Navigators will serve as an in-person resource for Americans who want additional assistance in shopping for and enrolling in plans in the Health Insurance Marketplace this fall. Below are the recipients of Navigator grants in Federally-facilitated and State Partnership Marketplaces, grouped by state. Recipients marked with an asterisk (*) are operating in more than one state. The anticipated grant amount listed in each case only applies to the amount going to that organization for that states specific operations.

ALASKA Alaska Native Tribal Health Consortium Anticipated grant amount: $300,000 The Alaska Native Tribal Health Consortium is a non-profit tribal health organization that will partner with each regional health corporation in Alaska that wishes to participate in outreach activities to raise public awareness of how the new insurance Marketplace impacts them and the healthcare system. United Way of Anchorage Anticipated grant amount: $299,918 The Alaska Navigators Coalition is a group of non-profits, led by the United Way of Anchorage. They plan to do outreach to uninsured populations throughout Alaska to promote awareness of new health care options and facilitate the enrollment of individuals.

ALABAMA Ascension Health* Anticipated grant amount: $202,706 Ascension Health is the nation's largest Catholic and nonprofit health system. The Ascension Health Navigator project will assist consumers (individuals and small employers) in understanding new programs, taking advantage of consumer protections, and navigating the health insurance system to find the most affordable coverage that meets their needs. AIDS Alabama, Inc. Anticipated grant amount: $501,380

AIDS Alabama devotes its energy and resources statewide to helping people with HIV/AIDS live healthy, independent lives and works to prevent the spread of HIV. AIDS Alabama Navigators will conduct community-wide educational events and presentations in an effort to educate Alabamians on the Federally-facilitated Marketplace. The project will focus on those newly-eligible for health insurance, especially reaching out to lower and middle-income populations. Samford University Anticipated grant amount: $326,794 Samford University, located just outside of Birmingham, Alabama will work with existing networks through its pharmacy, nursing, and education and professional studies schools to facilitate enrollment of individuals. The existing networks of schools and churches will reach a diverse community in the rural and metropolitan communities of Northern Alabama. Catholic Social Services Archdiocese of Mobile Anticipated grant amount: $20,750 The Service Center of Catholic Social Services provides essential services and skills training in Mobile County since 1953. The Service Centers Affordable Health Insurance Selection Program will provide enrollment assistance to low-income, under-insured, uninsured and vulnerable participants living in Mobile County, Alabama. Tombigbee Healthcare Authority Anticipated grant amount: $392,356 Tombigbee Healthcare Authority (THA) will place Navigators in 18 counties in the Alabama Delta Region to help consumers understand the new federal Marketplace coverage options and find the most affordable coverage that meets their health care needs. To achieve this goal, THA will develop a contractual agreement with its existing Delta Rural Access Program (DRAP) partners to expand their program focus. THA and these partnering agencies have been providing the Delta Region counties access to primary and preventive health care services, education and resources for more than nine years through outreach efforts in schools, churches, community centers, homes, and other community outlets.

ARKANSAS Southern United Neighborhoods* Anticipated grant amount: $270,193

Southern United Neighborhoods (SUN) is a public charity founded in March 2010 by low to moderate income people that uses research and training to combat the poverty, discrimination and community deterioration that keeps low income people from taking advantage of their rights and opportunities. Southern United Neighborhoods' Tri-State Outreach Project will implement and fulfill Navigation duties in the Tri-State Region of Arkansas, Louisiana, and Texas in Public-Use Microdata Areas (PUMAs) with high considerations of low income uninsured adult populations. University of Arkansas Anticipated grant amount: $774,745 The Arkansas Navigator Coalition is based out of the University of Arkansas, and plans to reach out to areas of Arkansas with the highest level of uninsured. The Coalition is comprised of agencies serving young adults, Latinos, African Americans, persons who are homeless, mentally ill, living in poverty, or who have disabilities.

ARIZONA Arizona Association of Community Health Centers Anticipated grant amount: $1,344,096 The Arizona Association of Community Health Centers has served as Arizona's Primary Care Association since 1985 and continuously strives to fulfill its mission of promoting the development and delivery of affordable and accessible healthcare. The Arizona Association of Community Health Centers Navigators will coordinate outreach opportunities throughout Arizona. Arizona Board of Regents, University of Arizona Anticipated grant amount: $190,268 The Center for Rural Health at the University of Arizona aims to reduce the numbers of Asian American and Pacific Islander uninsured in Pima County, and implement a comprehensive outreach strategy. They intend to use the Southern Arizona Asian & Islander Health Coalition to reach out to these populations and inform them of new coverage options. Greater Phoenix Urban League, Inc. Anticipated grant amount: $523,773 The Greater Phoenix Urban League aims to equip the disadvantaged with tools to achieve economic and social equality, including through improving their health and well-being.Greater Phoenix Urban League's Navigators will provide a comprehensive, statewide, public awareness

campaign aimed at identifying and assisting uninsured individuals across Arizona to access and navigate the Health Exchange Marketplace. Campesinos Sin Fronteras, Inc. Anticipated grant amount: $71,386 Campesinos Sin Fronteras is a Hispanic serving agency, providing services to farm workers and low-income Hispanics, while serving the general population as well. The Campesinos Navigator program will provide enrollment assistance to uninsured individuals in Yuma County, Arizona.

DELAWARE Chatman, LLC Anticipated grant amount: $510,577 Chatman, LLC will serve as a navigator within the state of Delaware by assisting consumers in accessing health coverage and conducting outreach and public education. Chatman, LLC has extensive experience in the areas of health benefits and the policy and benefits structure of the private health insurance providers.

FLORIDA University of South Florida Anticipated grant amount: $4,213,696 Florida Covering Kids & Families (FL-CKF) is a community and consumer-focused nonprofit initiative of The Lawton and Rhea Chiles Center for Healthy Mothers and Babies, based in the College of Public Health, at the University of South Florida (USF), located in Tampa, Florida. FL-CKF, in collaboration with a state-wide consortium of 10 partners, will assist consumers and small employers with the enrollment process and conducting public education activities to raise awareness about the Marketplace. Epilepsy Foundation of Florida Anticipated grant amount: $637,686 The Epilepsy Foundation of Florida is a not-for-profit. The Epilepsy Foundation of Florida plans to utilize their existing partnerships with schools and universities, government agencies, hospitals, and faith-based entities to educate and help enroll consumers in the Marketplace. Advanced Patient Advocacy, LLC*

Anticipated grant amount: $413,152.20 For nearly 14 years, Advanced Patient Advocacy has partnered with health care providers and state and local governments in 21 states to provide services to communities to help educate and enroll uninsured consumers. Advanced Patient Advocacy will work with medical centers to identify uninsured individuals and provide education and assistance to help them make informed decisions about enrollment in the Marketplaces. Legal Aid Society of Palm Beach County, Inc. Anticipated grant amount: $446,783 The Legal Aid Society of Palm Beach County will provide navigators in Palm Beach, Martin, Okeechobee and Hendry Counties. These counties include urban and rural communities that are racially, ethnically, linguistically, culturally and socio-economically diverse. The Legal Aid Society plans to do both direct engagement with consumers and well as provide information and public service announcements in multiple languages. Pinellas County Board of County Commissioners Anticipated grant amount: $600,000 The Board of County Commissioners in Pinellas County, FL will be working to conduct outreach to vulnerable populations in the region. The Board will provide education, information services and work to facilitate enrollment in qualified health plans or other programs primarily to uninsured residents in Pinellas County in a manner that is sensitive to cultural, linguistic, physical, and educational differences. National Hispanic Council on Aging* Anticipated grant amount: $646,825.50 National Hispanic Council on Aging (NHCOA) works closely with its Hispanic Aging Network composed of 39 community-based organizations across the continental U.S., the District of Columbia, and Puerto Rico. The NHCOA will deploy Navigators in Dade County, Florida, and Dallas County, Texas, to enroll the uninsured Hispanic population in these two counties with a focus on members of this population that are socially isolated due to cultural and linguistic differences Cardon Healthcare Network, LLC DBA Cardon Outreach* Anticipated grant amount: $238,000 Cardon Healthcare Network has over 20 years of experience providing in-person eligibility assistance to Medicaid enrollees in 33 states. Cardon Healthcare Network will use this experience to provide outreach and assistance with its existing robust infrastructure.

Mental Health America* Anticipated grant amount: $683,691 The Mental Health America Navigator Initiative will target underserved individuals with behavioral health disorders who are uninsured or underinsured. The National Office of Mental Health America (MHA) will serve as the lead agency in the Initiative. The Mental Health America of East Central Florida (MHAECF) will serve Brevard, Flagler, Orange, Putnam, St. Johns, and Volusia counties in Florida.

GEORGIA Structured Employment Economic Development Corporation Anticipated grant amount: $2,159,360 Seedco, a national organization helping low-income households and communities move toward economic prosperity since 1987, will act as lead agency for a consortium of partners in each state and oversee all aspects of the proposed program, providing technical assistance, financial oversight, compliance protocols, and partner facilitation. Seedco and its partners will support the activities of Navigators in Georgia and Tennessee, many of whom will be roving. University of Georgia Anticipated grant amount: $1,657,378 The University of Georgia's College of Family and Consumer Sciences and Cooperative Extension Service (UGA CES) plans to place navigators in several offices outside the Atlanta metro region. UGA CES plans to create awareness through community workshops, engage a network of existing partners to reach uninsured Georgia residents and provide direct services to consumers seeking assistance.

IOWA Genesis Health System* Anticipated grant amount: $128,430 Genesis Health System will implement a navigator program through the Genesis Visiting Nurses Association (GVNA). Genesis plans to use existing relationships to aid residents and small business in accessing new sources of coverage. Visiting Nurse Services of Iowa Anticipated grant amount: $257,142

The Visiting Nurse Services of Iowa will implement a navigator program serving 38 of Iowa's 99 counties, home to approximately half of the uninsured in Iowa. The Visiting Nurse Services will provide navigator services through public awareness, education and assistance in enrolling individuals in qualified health plans. They also will partner with other organizations with considerable expertise in eligibility, enrollment, and knowledge of other health programs such as Medicaid and CHIP. Planned Parenthood of the Heartland* Anticipated grant amount: $214,427 Planned Parenthood of the Heartland (PPHeartland) is a private, non-profit community-based health care agency operating in Arkansas, Iowa, Nebraska and Oklahoma. In Iowa, PPHeartland plans to have Navigators cover 61 counties and will strategically base Navigators in regions across Iowa.

ILLINOIS Genesis Health System* Anticipated grant amount: $137,283 Genesis Health System will implement a navigator program through the Genesis Visiting Nurses Association (GVNA). Genesis plans to use existing relationships to aid residents and small business in accessing new sources of coverage. Sinai Health System Anticipated grant amount: $157,271 Sinai Health System, through its community-based corporation, the Sinai Community Institute (SCI), plans to operate a navigator program together with a consortium of public and community and faith based organizations to raise awareness about coverage options and assist individuals in enrolling in the Illinois Health Insurance Marketplace. DuPage County Health Department Anticipated grant amount: $182,543 DuPage County Health Departments (DCHD) plans to implement a navigator program targeting hard to reach populations. DCHD has identified younger adults 19 to 34 years of age and small businesses and their employees as the target populations. Southern Illinois Healthcare Foundation Anticipated grant amount: $240,113

Southern Illinois Healthcare Foundation is a community-based, Federally Qualified Health Center network, with nearly 40 health centers located in seven counties in Southern Illinois. Southern Illinois Healthcare Foundation Navigators will meet with staff members of individual health centers throughout the service area to coordinate the sign-up and completion of Federal on-line training for additional health centers, to train and prepare additional staff members to be able to assist patients when they are visiting the health centers. A Safe Haven Foundation Anticipated grant amount: $324,736 A Safe Haven Foundation is a non-profit organization that has provided housing and supportive services to homeless individuals and their families for 19 years. A Safe Haven will raise awareness about the Marketplace and coverage options for vulnerable populations. Mercy Hospital and Medical Center Anticipated grant amount: $452,590 Mercy Hospital and Medical Center's Community-based Insurance Navigator Consumer Health (CINCH) Project will focus on identifying and connecting eligible individuals, especially minorities and the newly eligible, to insurance enrollment services at Mercy Hospitals main and satellite sites, Mercy Family Health Center's sites, and in community-based settings using Enrollment Navigators. The CINCH Project's Navigators will help facilitate enrollment in coverage and will educate consumers about available Qualified Health Plans. The Puerto Rican Cultural Center, Inc. Anticipated grant amount: $600,000 The Puerto Rican Cultural Center is a non-profit community-based institution, which seeks to serve the social/cultural needs of Chicago's Puerto Rican/Latino community. The Puerto Rican Cultural Center Navigator will focus on grassroots outreach, door-to-door canvasing, formal and informal popular education activities. Illinois College of Optometry Anticipated grant amount: $504,016 The Illinois College of Optometry will work with partner organizations to leverage a coalition of providers who have experience working with uninsured and underinsured patient populations. Many of the populations served by these partners are African-American, Latino, below the poverty line, or transient. The College and its partners will work to educate this population about their coverage options and help them evaluate those options and enroll in coverage. VNA Health Care

Anticipated grant amount: $132,737 VNA Health Care is the largest low income primary health care provider in Kane County, organized as the Visiting Nurse Association of Fox Valley. VNA Health Care will serve hard to reach populations and customers in their homes, following the successful model used in VNA's skilled home nursing service. The East Los Angeles Community Union Anticipated grant amount: $294,182 The East Los Angeles Community Union (TELACU) is a community development organization. For more than 40 years, TELACU has been improving the lives of individuals and families throughout East Los Angeles. TELACU will use Navigator funds in target markets in Illinois, paying special attention to the Hispanic community due to the high density of uninsured Hispanic residents in the state. National Council of Urban Indian Health* Anticipated grant amount: $35,000 National Council of Urban Indian Health is the only national, membership-based organization dedicated to outreach and education on behalf of Urban Indian Health. It provides training, technical assistance, outreach, and education to Urban Indian Health Programs.

INDIANA Affiliated Service Providers of Indiana, Inc. Anticipated grant amount: $897,150 Affiliated Service Providers of Indiana, Inc., based in Terra Haute, Indiana, is collaborating with community mental health centers and Ivy Tech Community College to develop a navigator workforce consisting of veterans, certified recovery specialists, and Ivy Tech Community College students to reach out to and educate individuals, families and small businesses on the Health Marketplaces and help them select and enroll in qualified health plans. Plus One Enterprises ,LTD, LLC Anticipated grant amount: $130,875 Plus One Enterprises, LTD, LLC is a woman-owned, black-owned, small business based in Anderson, IN and Madison County (east central Indiana). Plus One Enterprises will help individuals and employers in Delaware and Madison Counties, focusing on people who are under and uninsured, facilitating enrollments in qualified health plans and the Small Business Health Options Program (SHOP), and reaching out to small employers.

Health and Hospital Corporation of Marion County Anticipated grant amount: $590,985 Health and Hospital Corporation of Marion County (HHC), part of the Indiana state government, operates the Marion County Public Health Department and Wishard Health Servicesa patient-centered healthcare system for the uninsured and underinsured residents. HHC will hire and train staff to assist clients and implement an extensive education and media campaign to promote the Federally Facilitated Exchange. United Way Worldwide Anticipated grant amount: $424,586 United Way Worldwide, working with local United Ways and 2-1-1the free, three-digit dialing code for health and human servicesinformation, referral, and assistance service centers will work to provide access to health insurance to individuals, households, workers, and outreach to employers in Indiana.

KANSAS Advanced Patient Advocacy, LLC* Anticipated grant amount: $195,556 For nearly 14 years, Advanced Patient Advocacy has partnered with health care providers and state and local governments in 21 states to provide services to communities to help educate and enroll uninsured consumers. Advanced Patient Advocacy will work with medical centers to identify uninsured individuals and provide education and assistance to help them make informed decisions about enrollment in the Marketplaces. Ascension Health* Anticipated grant amount: $165,683 Ascension Health is the nation's largest Catholic and nonprofit health system. The Ascension Health Navigator project will assist consumers (individuals and small employers) in understanding new programs, taking advantage of consumer protections, and navigating the health insurance system to find the most affordable coverage that meets their needs. Kansas Association for the Medically Underserved Anticipated grant amount: $524,846 The Kansas Association for the Medically Underserved (KAMU) will serve as the lead agency and grantee for the Kansas Marketplace Consortium, a coalition of 427 safety net clinic, health

department, hospital, Community Mental Health Center and Area Agency on Aging locations. The Consortium will provide assistance to at least 48,000 eligible Kansans statewide.

LOUISIANA Southern United Neighborhoods* Anticipated grant amount: $486,123 Southern United Neighborhoods (SUN) is a public charity founded in March 2010 by low to moderate income people that uses research and training to combat the poverty, discrimination and community deterioration that keeps low income people from taking advantage of their rights and opportunities. Southern United Neighborhoods' Tri-State Outreach Project will implement and fulfill Navigation duties in the Tri-State Region of Arkansas, Louisiana, and Texas in Public-Use Microdata Areas (PUMAs) with high considerations of low income uninsured adult populations. Martin Luther King Health Center, Inc. Anticipated grant amount: $81,066 The Martin Luther King Health Center (MLKHC) is a private non-profit primary health care center and pharmacy, incorporated in 1986, and domiciled in the parish of Caddo, Shreveport, Louisiana. Navigator program components include a mass media campaign, public informational education presentations, participating in health fairs & trade fairs, one-on-one interactions with consumers, and the dissemination of print materials through various outreach venues. All services, media, and print material will be offered in English and Spanish. Southwest Louisiana Area Health Education Center Anticipated grant amount: $1,099,985.80 The Southwest Louisiana Area Health Education Center (SWLAHEC) was founded in 1991 and aims to improve health status in the region through access to information, education and health services. Their current services include programs on career and professional education, health interventions, preventive health, and public health support. They will help to educate Louisianans about their health coverage options and facilitate enrollment. Capital Area Agency on Aging, District II, Inc. Anticipated grant amount: $100,000 Capital Area Agency on Aging, District II will help individuals and small employers shop for, select and enroll in qualified health plans in Louisianas new competitive health insurance marketplace. The Agency plans to serve a variety of populations, including older and disabled

employees, small employers, and refugees being served by the Refugee Resettlement Program (statewide).

MAINE Western Maine Community Action Anticipated grant amount: $475,000 Western Maine Community Action, Inc. is a private non-profit community agency and is the lead for a state-wide consortium of eight community action agencies, collectively referred to as the WMCA Community Action Navigator Consortium. Funds will be used to create a statewide network of education, outreach and enrollment assistance available and accessible to every uninsured and under-insured individual in Maine. Fishing Partnership Health Plan Anticipated grant amount: $66,846 Fishing Partnership Health Plan (FPHP) is a health plan developed that provides subsidized coverage to uninsured commercial fisherman, many of whom operate as small business owners or employees. FPHP, in collaboration with the Maine Lobstermen's Association, will undertake a series of community health navigation activities, including: outreach; social marketing; distribution of material describing the program, the Affordable Care Act, and the health insurance enrollment process.

MICHIGAN Community Bridges Management Inc. Anticipated grant amount: $896,366 Community Bridges Management, Inc. provides expert IPA services including medical network management, claims processing oversight, provider relations, medical care service, patient relations and services. Funds will be used to provide both training and resources to make the process of enrolling in a health plan easier and more understandable through one-on-one contact. Community Bridges will also conduct widespread outreach and education activities. Arab Community Center for Economic & Social Services Anticipated grant amount: $276,593 ACCESS is non-profit service agency that advocates for, empowers and enables individuals, families, and the Arab, Chaldean, and Bangladeshi American communities in metropolitan Detroit to achieve optimal health and wellness. Funds will be used to proactively reach out to

and engage under/uninsured community members through local, multicultural electronic and print media, public access television and radio, and monthly ACCESS and community partner events, like weekly health Expos at the three County Public Health Offices. American Indian Health & Family Services of SE Michigan, Inc. Anticipated grant amount: $49,583.50 American Indian Health and Family Services is a non-profit health organization serving a large number of the uninsured or under insured. Funds will be used to host educational forums, publications, provide self-service kiosks and expert personnel able to assist consumers to enroll in health plans. Michigan Consumers for Healthcare Anticipated grant amount: $1,319,345 The Michigan Consumers for Healthcare (MCH) network includes statewide organizations and local affiliates with deep experience in conducting Medicaid and CHIP outreach and enrollment and health insurance counseling for uninsured and vulnerable individuals and families. The project will provide services in 10 Michigan Regions and Tribal Health Centers. These organizations reach low-income, vulnerable populations in every region and county in the state, guaranteeing that the project will serve Michigan residents regardless of where they live.

MISSOURI Primaris Healthcare Business Solutions Anticipated grant amount: $1,045,624 Primaris will lead a coalition of eleven Missouri healthcare and social services organizations that will raise awareness of the coverage options available to Missourians. Primaris, a federal health services contractor for the last 30 years, has operated a free insurance counseling service for Missourians on Medicare, where counselors help clients navigate the local marketplace of Medicare prescription drug and Medicare Advantage plans without bias or cost to the consumer. Primaris will use existing infrastructure to help facilitate the enrollment of consumers in to private health plans. Missouri Alliance of Area Agencies on Aging Anticipated grant amount: $750,000 The Missouri Alliance of Area Agencies on Aging (ma4) has been the leading voice for older adult services, information, and advocacy in Missouri since 1973. Funding will be used for extensive statewide outreach and education to consumers, including a special session at ma4s

Show Me Summit on Aging and Health, wellness events, Area Agencies on Aging (AAA) newsletters, ma4 website, public media, social media, and information call centers such as 211.

MISSISSIPPI Oak Hill Missionary Baptist Church Ministries Anticipated grant amount: $317,742 Oak Hill Missionary Baptist Church Ministries serves the lower Mississippi River Delta Region. Oak Hill will train clergy to be health ambassadors that will reach out to uninsured residents to provide information about enrollment in the Marketplaces. University of Mississippi Medical Center Anticipated grant amount: $831,986 University of Mississippi Medical Center will use funds to perform educate and assist patients in enrolling in Medicaid, CHIP, and the Marketplace in all 82 counties in Mississippi,.

MONTANA Intermountain Planned Parenthood, Inc. DBA Planned Parenthood Anticipated grant amount: $295,604 Planned Parenthood of Montana (PPMT) will use funds to establish the Montana Marketplace Connection to assist consumers in navigating the Montana Marketplace. Consumers will be able to access assistance from a Navigator by scheduling an appointment or as a walk-in to a PPMT health center, calling a hotline or by video conferencing. Montana Primary Care Association, Inc. Anticipated grant amount: $299,382 The Montana Primary Care Association (MPCA) is an association of community health centers aimed at delivering primary and preventive healthcare to under-served and vulnerable populations. MPCA will use funds to partner with 17 affiliated federally-qualified community health centers, which provide primary and preventive health care services to 100,000 Montanans in 20 counties, to serve currently uninsured Montanans. Funding will be used to train community health center staff to expand current outreach and enrollment assistance activities and to enroll patients into affordable health insurance coverage. Rural Health Development DBA Montana Health Network Anticipated grant amount: $143,076

Rural Health Development is a non-profit consortium of rural health care providers established in 1990 with a strong network of relationships enabling them to bring value to healthcare through cost savings, research and development of products and services in underserved Frontier communities. Rural Health Development will provide enrollment assistance, through member health care providers, in eastern Montana.

NORTH CAROLINA Randolph Hospital, Incorporated Anticipated grant amount: $352,320 Randolph Hospital, Inc., will use Navigator funds to serve a three county area in North Carolina. Randolph Hospital plans on targeting specific geographic regions with high uninsured populations, and working with community organizations in those areas to leverage resources and reach the most people. Additionally, Randolph Hospital will work with hospital financial counselors that already work with uninsured hospital patients to inform those patients of their coverage options. Mountain Projects, Inc. Anticipated grant amount: $359,750 Mountain Projects, Inc. is a non-profit Community Action Agency serving Haywood and Jackson counties in North Carolina. Mountain Projects plans on providing Navigator services to the seven western-most rural counties of North Carolina, including Haywood, Jackson, Macon, Swain, Graham, Clay, and Cherokee. They will leverage existing relationships in these rural communities with the goal of educating the uninsured and facilitating access to the new insurance options now available. North Carolina Community Care Networks Anticipated grant amount: $1,988,428 North Carolina Community Care Networks, Inc. are consortia that total more than 100 organizations who will work to inform consumers statewide, with particular focus in areas where there is a higher concentration of uninsured. These networks will be serving to reach out, inform, educate and help enroll North Carolinians, and include organizations in the legal rights, faith-based, agricultural, and aging communities. Alcohol/Drug Council of North Carolina Anticipated grant amount: $324,798

The Alcohol/Drug Council of North Carolina plans on using Navigator grant funds to establish Project Jumpstart, implemented by a consortium formed to provide specialized navigation services to people in recovery from mental illness and/or substance abuse. A majority of people working on Project Jumpstart will have personal experience with mental illness, an addictive disorder, and/or a chronic disease, and will draw on these experiences to reach out to and help enroll this target population in new coverage options.

NORTH DAKOTA Great Plains Tribal Chairmen's Health Board* Anticipated grant amount: $186,000 The Great Plains Tribal Chairmens Health Board (GPTCHB) is a community based consumer focused non-profit that will provide enrollment assistance to American Indians residing on and near the eight Reservations in South Dakota and the four Reservations and one Indian Service Area in North Dakota and those residing in major urban areas served by Urban Indian Health Centers in these two States. Minot State University - North Dakota Center for Persons with Disabilities Anticipated grant amount: $414,000 The North Dakota Center for Persons with Disabilities will use grant funds to establish a collaborative network of regional Navigators who already have established the trust of their neighbors. NDCPD will hire Regional Navigators stationed in each of the state's eight Human Service Regions. Navigator support will be provided to currently uninsured and underinsured people, specifically targeting those most at risk of being uninsured in North Dakota, including people with mild disabilities, people with mental health disorders, farmers, young adults, Native Americans, small business persons, people who are unemployed and people who are drug or alcohol addicted.

NEBRASKA Community Action of Nebraska, Inc. Anticipated grant amount: $562,457 Community Action of Nebraska serves community service organizations and non-profits across the state of Nebraska. The organization will build upon existing statewide infrastructure to provide assistance to nearly 40,000 Nebraskans in need of health coverage through the Marketplace. Ponca Tribe of Nebraska

Anticipated grant amount: $37,543 The Ponca Tribe of Nebraska will use funds to provide outreach, education, and enrollment services to American Indians residing in the fifteen counties that constitute the Ponca Service Delivery Area. Funds will be used to hire Ponca Navigators, and assistance will be delivered through their five office locations, in person, and via videoconferencing.

NEW HAMPSHIRE Bi-State Primary Care Association Anticipated grant amount: $434,839 Bi-State Primary Care Association, a nonprofit charitable organization, represents a consortium comprised of 12 health centers and one multicounty rural health network all with the mission of providing individuals and communities with quality, affordable health care. Bi-State and its partners plan on using funds to leverage historical experience with community partners, health coverage initiatives, and trusted relationships with the medically uninsured to build a statewide outreach and navigation infrastructure serving eight counties in the state. Planned Parenthood of Northern New England Anticipated grant amount: $145,161 Planned Parenthood of Northern New England (PPNNE) is a nonprofit health care provider. PPNNE will assist patients and other consumers with understanding new programs, taking advantage of consumer protections, and navigating the health insurance system to find the most affordable coverage that meets their needs. The organization will primarily serve individuals in the following counties: Hillsborough, Rockingham, Grafton, Cheshire, and Sullivan in New Hampshire.

NEW JERSEY Center For Family Services, Inc. Anticipated grant amount: $677,797 Center For Family Services, Inc. (CFS) is a nonprofit human services agency with 90 years of experience serving individuals and families across the life span. CFS will serve the seven lower southern counties of New Jersey which include: Camden, Burlington, Gloucester, Salem, Atlantic, Cape May and Cumberland. The funds will be used to educate and help enroll consumers in the health insurance marketplace. Wendy Sykes - Orange ACA Navigator Project

Anticipated grant amount: $239,810 The Orange ACA Navigator Project (OACANP) will integrate several successfully existing community-based systems to serve as a guide to help uninsured residents and small businesses in underserved and vulnerable populations. OACANP will help consumers learn about insurance options and assist with enrollment in health plans through the competitive health insurance marketplace. The Urban League of Hudson County Anticipated grant amount: $565,000 The Urban League of Hudson County (ULHC) is a community based organization dedicated to advocating, facilitating, and promoting initiatives that allow local residents to participate in the development of urban neighborhoods. ULHC will partner with the Urban League for Bergen County, the Urban League of Morris County and the Urban League Union County to assist consumers in enrolling in the Marketplace. Public Health Solutions Anticipated grant amount: $400,583 Public Health Solutions (PHS) is one of the largest non-profit organizations in New York City. PHS will partner with four community-based organizations in New Jersey to provide outreach and enrollment assistance in Hudson and Essex Counties. FoodBank of Monmouth and Ocean Counties, Inc. Anticipated grant amount: $137,217 The FoodBank was created in 1984 and today serves over 260 pantries, soup kitchens and other feeding programs with almost 7 million pounds of food distributed annually. With the help of roughly 1,000 volunteers each year, the FoodBanks programs work to eliminate hunger by providing emergency food, skills training, outreach programs and advocacy for families in need. Their work will help connect uninsured and underinsured individuals with information about their health insurance options.

OHIO Ohio Association of Foodbanks Anticipated grant amount: $1,958,961 Since 1991 the Ohio Association of Foodbanks has benefited thousands of people in need in the State of Ohio. The Ohio Association of Foodbanks will provide outreach support through a variety of phone, online, and promotional tools.

Children's Hospital Medical Center Anticipated grant amount: $124,419 Children's Hospital Medical Center serves the medical needs of infants, children and adolescents with family-centered care, innovative research and outstanding teaching programs. They plan on reaching out and enrolling the uninsured through the main hospital location in urban inner city geographic area as well as two satellite locations. Clermont Recovery Center, Inc. Anticipated grant amount: $44,938 Clermont Recovery Center, Inc., a not-for-profit drug and alcohol outpatient treatment and prevention agency located in the county seat of Batavia, Ohio. The organization will educate and help enroll the uninsured in coverage in several counties in Ohio, including Clermont, Brown and Adams. Helping Hands Community Outreach Center Anticipated grant amount: $230,920 Helping Hands Community Outreach Center is a nonprofit organization serving the Dayton Ohio region and provides resources to families and individuals who are hospitalized. The organization will attend local events and will work with businesses and public agencies to promote education and awareness of the Marketplaces. Neighborhood Health Association Anticipated grant amount: $684,630 The Neighborhood Health Association (NHA), a Federally Qualified Health Center (FQHC) system located in Northwest, Ohio will through its Affordable Care Act Insurance Enrollment Project, raise community awareness regarding the insurance Exchange and facilitate enrollment in for the uninsured.

OKLAHOMA Cardon Healthcare Network, LLC DBA Cardon Outreach* Anticipated grant amount: $178,500 Cardon Healthcare Network has over 20 years of experience providing in-person eligibility assistance to Medicaid enrollees in 33 states. Cardon Healthcare Network will use this experience to provide outreach and assistance with existing robust infrastructure.

Oklahoma Community Health Centers, Inc. Anticipated grant amount: $860,866 Oklahoma Community Health Centers, Inc., a consortium comprised of a variety of non-profit organizations will apply a statewide approach to providing enrollment assistance to Oklahomans. Consortium members will have Navigators located in 57 cities across the states, enabling them to provide services in 45 counties in Oklahoma. Among other activities, funds will be used to target outreach to small business and small business groups, conduct outreach at community events and health fairs, target outreach to special populations served by consortium partners, and develop state-specific outreach materials for use by consortium members. Little Dixie Community Action Agency, Inc. Anticipated grant amount: $580,733 Little Dixie Community Action Agency, Inc. plans on providing enrollment assistance to lower income families and individuals in local communities in Oklahoma by forming a consortium with 14 additional Community Action Agencies across Oklahoma, serving 63 counties in Oklahoma. Funding will be used to provide Navigator services in each of the 15 service areas, with an emphasis on leveraging existing staff with local ties to be trained as Navigators.

PENNSYLVANIA Resources for Human Development, Inc. Anticipated grant amount: $953,176 Resources for Human Development, Inc. (RHD), a non-profit organization based in Philadelphia, Pennsylvania, will provide enrollment assistance in the ten counties in Pennsylvania with the highest rates of uninsured people. RHD will target specific groups and individuals who traditionally have poor access to healthcare insurance coverage. Pennsylvania Association of Community Health Centers Anticipated grant amount: $694,380 Pennsylvania Association of Community Health Centers (PACHC) represents and supports the largest network of primary health care providers in the Pennsylvania. PACHC will coordinate enrollment assistance efforts in underserved areas throughout the Commonwealth using a connected and networked approach. Pennsylvania Mental Health Consumers' Association Anticipated grant amount: $380,000

Pennsylvania Mental Health Consumers Association (PMHCA) will work in a consortium with Mental Health Association in PA (MHAPA) and Mental Health America Westmoreland County (MHAWC) to provide enrollment assistance to people who use or need behavioral health services in their insurance plans, particularly those who experience serious mental illness or serious psychological distress. Cardon Healthcare Network, LLC DBA Cardon Outreach* Anticipated grant amount: $178,500 Cardon Healthcare Network has over 20 years of experience providing in-person eligibility assistance to Medicaid enrollees in 33 states. Cardon Healthcare Network will use this experience to provide outreach and assistance with existing robust infrastructure. Mental Health America* Anticipated grant amount: $503,129 The Mental Health America Navigator Initiative will target underserved individuals with behavioral health disorders who are uninsured or underinsured. The National Office of Mental Health America (MHA) will serve as the lead agency in the Initiative. The Mental Health Association of Southeastern Pennsylvania (MHASP) will serve Bucks, Chester, Delaware, Montgomery, and Philadelphia counties in Pennsylvania.

SOUTH CAROLINA DECO Recovery Management LLC Anticipated grant amount: $1,211,203 DECO Recovery Management, a national leader in medical assistance eligibility management and patient advocacy, has taken a lead role in the establishment of a consortium with strategically located partners in South Carolina, to perform the cooperative agreement activities as a Navigator. DECO and its consortium partner TBB-SC will conduct targeted marketing campaigns and public education events throughout the state. The Cooperative Ministry Anticipated grant amount: $508,313 The Cooperative Ministry is establishing the Health Insurance Education Cooperative to provide pathways to better health and quality for uninsured persons in South Carolina. This effort targets eight counties that comprise the Columbia-Orangeburg-Newberry Region, and aims to reach, engage, and help enroll all eligible consumers in that area. Beaufort County Black Chamber of Commerce

Anticipated grant amount: $234,099.21 Beaufort County Black Chamber of Commerce will utilize existing networks and infrastructures within various parts of the region to provide outreach around new coverage options. BCBCC will be conducting workshops, community meetings and individual counseling activities as part of the overall education and outreach efforts.

SOUTH DAKOTA South Dakota Community Action Partnership Anticipated grant amount: $336,000 The South Dakota Community Action Partnership will work with partners to assist South Dakotas 92,441 uninsured consumers to prepare electronic and/or paper applications that will establish eligibility and enrollment in coverage through the Marketplace and potentially qualify for an insurance affordability program. The South Dakota Navigators Coalition will also provide outreach and education to raise awareness about the Marketplace, and will refer consumers to consumer assistance programs when necessary. TENNESSEE Structured Employment Economic Development Corporation* Anticipated grant amount: $1,216,013 Seedco, a national organization, helping low-income households and communities move toward economic prosperity since 1987, will act as lead agency for a consortium of partners in each state and oversee all aspects of the proposed program, providing technical assistance, financial oversight, compliance protocols, and partner facilitation. Seedco and its partners will support the activities of Navigators in Georgia and Tennessee, many of whom will be roving. Tennessee Primary Care Association Anticipated grant amount: $781,265

The Tennessee Primary Care Association (TPCA) is a community-focused non-profit that aims to improve access to primary health care through leadership, advocacy and support of community health centers. TPCA will leverage health centers' existing relationships to conduct outreach and enrollment efforts, and reach beyond its member CHC efforts to fill gaps in geographic coverage in the 13 counties not served by community health centers.

TEXAS United Way of Metropolitan Tarrant County Anticipated grant amount: $5,889,181 United Way of Metropolitan Tarrant County has served the people in the Fort Worth and Arlington area of Texas for over 90 years. In collaboration with 17 organizations, United Way of Metropolitan Tarrant County will work across the state to promote enrollment in the Marketplace. Migrant Health Promotion, Inc. Anticipated grant amount: $589,750 Migrant Health Promotion improves health in farmworker and border communities in Florida, Michigan, Ohio, Washington, and South Texas. This project will focus on enrolling and educating migrant populations in the Rio Grande Valley of Texas. National Hispanic Council on Aging* Anticipated grant amount: $646,825 National Hispanic Council on Aging (NHCOA) works closely with its Hispanic Aging Network composed of 39 community-based organizations across the continental U.S., the District of Columbia, and Puerto Rico. The NHCOA will deploy Navigators in Dade County, Florida, and Dallas County, Texas, to enroll the uninsured Hispanic population in these two counties with a focus on members of this population that are socially isolated due to cultural and linguistic differences. Change Happens Anticipated grant amount: $785,000 Change Happens is a non-profit located in Houston, Texas, with experience in conducting community engagement through education, outreach, and developing dynamic relationships with community members. The goal of the project is to provide trained Navigators with expertise in eligibility and enrollment rules and procedures to conduct public education and

outreach activities that result in 98,891 uninsured individuals enrolled in affordable health insurance. United Way of El Paso County Anticipated grant amount: $642,121 United Way of El Paso County has served residents of El Paso, Texas for over 90 years. Through the Enroll El Paso project this grant will provide training to people to work with the community and provide education and assistance with Marketplace enrollment. Southern United Neighborhoods* Anticipated grant amount: $600,678 Southern United Neighborhoods is a public charity that uses research and training to combat poverty. Southern United Neighborhoods Navigators will provide expertise in eligibility, enrollment and program specifications of Affordable Insurance Exchanges in Arkansas, Louisiana and Texas; implement comprehensive outreach and public education to over 835,000 low to moderate income residents who are likely to be eligible for enrollment in a QHP; and coordinate with community groups, employers, faith-based groups and state agencies to integrate outreach efforts. East Texas Behavioral Healthcare Network (ETBHN) Anticipated grant amount: $1,337,520 The East Texas Behavioral Health Network (ETBHN) will serve the uninsured, underinsured, and self-employed population in 75 counties in Texas. This program will assist this population with understanding new healthcare programs, taking advantage of consumer protections, and navigating the health insurance system in order to find the most affordable coverage that meets their needs. National Urban League Anticipated grant amount: $376,800 The National Urban League Navigators plan to conduct outreach and enrollment predominantly in areas that have low-income, medically underserved, urban residents in Texas.

UTAH Utah Health Policy Project Anticipated grant amount: $406,788

The Utah Health Policy Project is a nonprofit organization dedicated to lasting solutions to the crisis of the uninsured and rising health care costs. The Utah Health Policy Project will lead the Take Care Utah Navigator Hub that will connect nonprofit community-based organizations with resources on health coverage options. Utah AIDS Foundation Anticipated grant amount: $126,258 Utah AIDS Foundation (UAF) is the oldest and largest AIDS service organization in Utah, with over 27 years of experience providing effective, culturally-sensitive services to people living with HIV and populations at high risk for HIV in Utah. UAF will hire two full-time staff to serve as Navigators to educate target communities about Exchanges and to help individuals determine eligibility, understand their options, select, and complete all necessary steps to enroll in a Qualified Health Plan. Cardon Healthcare Network, LLC DBA Cardon Outreach Anticipated grant amount: $238,000 Cardon Healthcare Network has over 20 years of experience providing in-person eligibility assistance to Medicaid enrollees in 33 states. Cardon Healthcare Network will use this experience to provide outreach and assistance with existing robust infrastructure. National Council of Urban Indian Health* Anticipated grant amount: $35,000 National Council of Urban Indian Health is the only national, membership-based organization dedicated to outreach and education on behalf of Urban Indian Health. It provides training, technical assistance, outreach, and education to Urban Indian Health Programs.

VIRGINIA Virginia Poverty Law Center, Inc. Anticipated grant amount: $1,278,592 The Virginia Poverty Law Center (VPLC) is a non-profit organization that serves Virginia's legal aid system by providing advocacy, training, and litigation support on civil justice issues that affect low-income Virginians based in Richmond. It provides training and technical assistance to the legal aid community and others and works on health care issues with a wide range of statewide organization and partners. It will be working with a statewide consortium of nine well-established legal services programs in the commonwealth. Advanced Patient Advocacy, LLC*

Anticipated grant amount: $483,433 For nearly 14 years, Advanced Patient Advocacy has partnered with health care providers and state and local governments in 21 states to provide services to communities to help educate and enroll uninsured consumers. Advanced Patient Advocacy will work with medical centers to identify uninsured individuals and provide education and assistance to help them make informed decisions about enrollment in the Marketplaces.

WISCONSIN Partners for Community Development, Inc. Anticipated grant amount: $315,720 Partners for Community Development will facilitate the enrollment of the uninsured with emphasis on reaching out to Hispanic and Hmong families and other hard-to-reach eligible consumers in Door, Kewaunee, Manitowoc, Ozaukee, and Sheboygan Counties in Wisconsin. It will focus outreach and enrollment efforts in its service area on hard-to-reach consumers and families with limited English proficiency. Northwest Wisconsin Concentrated Employment Program, Inc. Anticipated grant amount: $285,035 The Northwest Wisconsin Concentrated Employment Program will help lead a Navigator project in their region to promote accessibility to health insurance Marketplace and the health care system in Wisconsin. The cooperative agreement will be used to provide Marketplace Navigator services at 21 Wisconsin Job Centers without reach provided at libraries, Senior Centers and college campuses by existing staff that have experience working with diverse populations. Legal Action of Wisconsin, Inc./SeniorLAW Anticipated grant amount: $70,000 SeniorLAW is a community and consumer-focused nonprofit located in Milwaukee that provides a variety of services to individuals age 60 or over, mostly related to health insurance and public benefits. This group will conduct outreach to the 60-64 population in Wisconsin, using community education, client contacts and agency relationships that SeniorLAW has established throughout Southeastern Wisconsin. National Council of Urban Indian Health* Anticipated grant amount: $35,000

National Council of Urban Indian Health is the only national, membership-based organization dedicated to outreach and education on behalf of Urban Indian Health. It provides training, technical assistance, outreach, and education to Urban Indian Health Programs. National Healthy Start Association Anticipated grant amount: $191,667 The National Healthy Start Association is a non-profit organization that works to improve birth outcomes and health disparities across the country through the federal Healthy Start programs. They will reach out to the uninsured population and provide education and assistance services for Marketplace enrollment. R&B Receivables Management Corporation DBA R&B Solutions Anticipated grant amount: $104,520 R&B Receivables Management Corporation currently works to connect uninsured and underinsured individuals with coverage options to help them afford health care. They will expand their services to connect with a broader range of underserved populations, and leverage their experience screening people for eligibility for medical assistance programs to connect people with coverage.

WEST VIRGINIA Advanced Patient Advocacy, LLC* Anticipated grant amount: $276,617 For nearly 14 years, Advanced Patient Advocacy has partnered with health care providers and state and local governments in 21 states to provide services to communities to help educate and enroll uninsured consumers. Advanced Patient Advocacy will work with medical centers to identify uninsured individuals and provide education and assistance to help them make informed decisions about enrollment in the Marketplaces. West Virginia Parent Training and Information, Inc. Anticipated grant amount: $365,758 West Virginia Parent Training and Information, Inc. will provide public education about health insurance options, and facilitate enrollment in the new health insurance marketplaces. The effort will include outreach to people with disabilities, people with limited English proficiency and/or limited literacy, people of low socioeconomic status, and people in rural areas.

WYOMING

Memorial Hospital of Laramie County DBA Cheyenne Regional Anticipated grant amount: $401,281 Memorial Hospital of Laramie will help lead "Enroll Wyoming," a statewide effort to reach out to the uninsured in Wyoming. They plan on reaching out to the uninsured population through hospitals, safety-net providers, and others to inform people of their coverage options and facilitate enrollment. Wyoming Senior Citizens, Inc. Anticipated grant amount: $198,719 Wyoming Senior Citizens, Inc. is a non-profit that is working to provide eligible citizens with information about programs that can help them enroll in health care. Counselors from the group will provide public information at libraries, senior centers, town halls, etc., and focus on eligible populations in the rural, frontier, and farming communities in Fremont, Hot Springs, Washakie, Big Horn, Carbon, Natrona, Converse, Goshen, and Laramie Counties.
