Safety on-set and in all instances is the number one priority of Brown University Motion Pictures. As an active Production Crew Member you are responsible for your own safety, for the safety of those around you, and for the safety of your own personal property. This Release of Liability Agreement is made _________ (Date) between ___________(Representative of BMP) and___________________________________, (Signee) This release if Liability is given in exchange for the use of the equipment and facilities to be used by the Production Team and also for the potential use of the signees personal property. The signee hereby expressly agree to the following: 1. By signing this document you acknowledge that you understand and are aware that participating in an independent production involves some inherent risk including the risk of serious injury and/or death and/or property damage. 2. By signing this document you are taking on full responsibility for any and all risks involved in or caused by your use of and/or misuse of equipment without any limitation, including the risks of death, bodily injury, personal injury, property damage, fire or explosion, the unavailability of emergency medical care. 3. By signing you are agreeing to assume fault for any incidents related to or caused by your use of equipment and/or presence at production locations without any limitation, including the risks of death, bodily injury, personal injury, property damage, fire or explosion, or the unavailability of emergency medical care. 4. By signing this agreement you are assuming sole responsibility for any damages to personal property or equipment that may occur without limitation, and acknowledging that no form of reimbursements will be owed to you should any damage or destruction occur. 5. By signing you are agreeing and acknowledging that this Release and Waiver of Liability Agreement is official and is intended to be as broad and inclusive as is permitted by Rhode Island Law, and that in the event any portion of this release is determined to be invalid, illegal, or unenforceable, the validity, legality and enforceability of the balance of this release shall not be affected or impaired in any way and shall continue in full legal force and effect.

6. I HAVE READ THIS DOCUMENT. I UNDERSTAND IT IS A PROMISE NOT TO SUE AND A RELEASE OF ALL CLAIMS.
