Project Closeout Survey Template

Instructions: This template is a suggested framework for your project closeout survey. Using an online survey website (such as: Surveymonkey.com, Zoomerang.com, Fluidsurveys.com, Checkbox.com) is suggested. The introduction and questions below are meant to be copied, then pasted into the appropriate fields in the online survey tool. The roles referred to on this sample survey are: Project Sponsor, Team Lead, Project Manager and Project Team Member. A team lead is usually a VP-level individual who has an interest in the final product. If one or more of these does not exist, please adjust the survey as appropriate. Detailed explanation for each question are in BLUE. Just delete them and paste the questions into the online survey form, following the instructions on the website you choose. It is delivered using an online survey tool so that participants can be assured that the information gathered is anonymous and will be used strictly for making improvements to similar efforts going forward. None of the information gathered will be tied to individuals in any way, so please make your answers as clear and comprehensive as possible. There will be no opportunity for follow-up questions.
