. , West Bengal, in connection with availing of Scholarship Scheme of Teaching Assistantship (M. Tech) at MCKV Institute of Engineering, 243, G. T. Road (North), Liluah, Howrah-711204, West Bengal (herein referred to as Institute for the sake of brevity) under TEQIP II do here by solemnly affirm and declare as under: 1. That I, the awardee will complete full 2-Year M.Tech course in this Institute, failing which I will be liable to refund the entire amount of Teaching Assistantship to the Institute drawn till the date of such discontinuation.

2. That I will be bound to borne all fees of the Institute and the University during the entire M. tech course (2-Year) at the Institute, while availing Teaching Assistantship from the Institute and submitting them on time as specified by the institute from time to time. 3. That I will be bound to take eight (8) / twelve (12) hours teaching workload per week and other duties as assigned by the competent authority. 4. That the continuation of Teaching Assistantship in the second year will be subject to satisfactory performance evaluation at the end of first year*.

5. That my award may be withdrawn at any time under the following circumstances: (a) If the academic performance is found poor and /or fails to get promoted to the next semester. Poor academic performance will mean and include, among other things, poor academic performance in Class Tests and /or in semester examinations in the form getting supplementary in any paper or papers. (b) If does not appear without acceptable justification at any Class Test arranged in accordance with notified schedule. (c) If found indulging in any kind of indiscipline in the Institute against which action has been taken in any form against him/her. (d) If deliberately violates the norms and codes of the Institute Library. (e) If attendance in Classes is found below 75%. (f) If fails to deposit other fees as notified by the Institute authority from time to time. (g) If avails and/or enjoys any other scholarship scheme from other Agencies /Organisation. 6. That I will be entitled to avail maximum 14 days leave in a year from schedule work on proper approval of respective HOD. 7.
