FDAR (BABY) F- Immediate Newborn Care Born an alive baby boy/girl FAMILY NAME at TIME via Normal Spontaneous Vaginal Delivery D- Cried lustily, with good muscle tone, with good sucking reflex, minimal vernix caseosa, with an APGAR score of 9,10 A-received safely, wiped immediately, stimulated to cry, brought to NICU immediately (applicable only if the baby is distress), cord care done aseptically, vital signs taken and charted, anthropometric measurements done, identification band attached to right ankle, terramycin eye ointment applied to both eyes at TIME, Vitamin K 0.1/0.05cc injected intramuscularly at TIME at the right thigh, kept warm and dry R- Endorsed to NICU staff for ballard scoring and further evaluation.
