ATO Module Enrolment Form
September 2009 Intake (Enrolment Deadline: 14 August 2009)
Important Notes

For enrolment in the Qualification Programme Module(s) in collaboration with

Accredited Workshop Organiser

1. The Institute has accredited your employer as an Accredited Workshop Organiser. This provides your employer with more flexibility to
organise your training, a very important part of your career development. Under the Institute’s Accreditation of Training Organisations
(ATO) Scheme, your employer will organise the workshop component of the Qualification Programme under the prescribed criteria laid
down by the Institute. By offering more flexibility in teaching venues and class schedules, the scheme facilitates busy prospective
professional accountants like yourselves to pursue life-long learning to upgrade your competencies and qualify as CPAs. There is no
change to the course content, course materials, assessment methodologies, quality assurance mechanism and eligibility to claim
reimbursement under the HKSAR Government Continuing Education Fund (CEF) if students should so qualify. Registered students
under the ATO Scheme will continue to pay the QP fees to the Institute direct, and the Institute takes full responsibility to manage and
run the programme as required by the Examinations Board and under the CEF scheme.
2. To enrol in the Qualification Programme Module(s) held in collaboration with Accredited Workshop Organiser, you are required to: (i)
register as a QP Registered Student in advance OR submit your QP Student Registration Application to the Institute on or before the
specified registration deadline; and (ii) seek prior approval from your employer, i.e. the Accredited Workshop Organiser, for admitting you
to its workshops. Under this scheme, the workshop component of Qualification Programme Module is conducted by Accredited
Workshop Organiser, while the module examination component remains to be run by the Institute.
3. The Qualification Programme is designed to be taken one module at a time. However, you have the option of taking two modules at a
time should you wish to.
4. Please refer to the workshop timetable to be provided by your Accredited Workshop Organiser for details of the dates and time of
workshops. You should consult your Accredited Workshop Organiser for the Workshop Group appropriate to you, and put down the
corresponding workshop code agreed between you and your Accredited Workshop Organiser in Section II (A) to indicate the workshop
group assigned to you by your Accredited Workshop Organiser.
5. The enrolment fee is HK$4,000 for each module. Your remittance by crossed cheque should be made payable to “Hong Kong Institute
of Certified Public Accountants”. Please issue a separate cheque for each module. No cash or reservation (i.e. completed enrolment
form not accompanied by full payment) is accepted. Enrolment fee will be debited after the enrolment deadline. Any bank charges
and/or expenses incurred as a result of bounced cheque(s) shall be borne by the applicant. Fees are non-refundable and non-
transferable. You should submit your enrolment form together with the required cheque(s) to your Accredited Workshop Organiser for
endorsement, who is entrusted with a responsibility to collect the form and the payments and send them to the Institute’s Student
Education & Training Department on your behalf.
6. Enrolment in the Qualification Programme Module(s) is on a session-by-session and voluntary basis. Once enrolled in a module, you
are required to attend a series of four workshops for that module regardless of your choice of workshop provider (i.e. the Institute or your
Accredited Workshop Organiser). You are not allowed to make any changes without the prior approval from your Accredited Workshop
Organiser and the Institute. It is your responsibility to ensure that there is no clash of Workshops when making your choice. Non-
attendance will result in no marks being awarded for the workshop that you have not attended.
7. All applicants will be notified of their enrolment result in writing. The venue map, if any, and other documents for the workshops will be
provided in a Confirmation Slip. The Confirmation Slip will be issued ONCE ONLY. Please ensure to keep the Confirmation Slip under
your safe custody. The Institute may charge a fee for re-issuance of a duplicate copy. Applicants should contact the Student Education
& Training Department if they have not received any notification about their enrolment one week before module commencing date. Any
change of personal data MUST be notified to the Institute in writing and in good time.
8. An Orientation Session will be held one or two days before the commencement of the modules. The main purposes of the Orientation
Session are to brief QP registered students about the Qualification Programme Modules and help them get started in their study. All
newly registered students will be invited separately.
9. Candidates of the Qualification Programme have obligation to ensure the confidentiality of the workshop questions used for assessment
purposes. For example, candidates must not discuss/disclose the relevant information about the workshop questions to any person
outside your assigned workshop group.
10. In relation to your participation in the Qualification Programme Module(s) in collaboration with Accredited Workshop Organiser, your
personal data contained in this enrolment form, performance in the ATO workshops, module examination results, withdrawal application,
change of workshop group, claim of CEF reimbursements and any related matters may be made accessible to the concerning
Accredited Workshop Organiser for administration purposes as the Institute thinks fits doing so when processing your registration and
examination matters. Please note that detailed workshop marks are awarded by your Workshop Facilitators, who are appointed by the
Institute, and bound by confidentiality rules. Only overall workshop performance and overall module performance are available to your
Accredited Workshop Organiser, subject to your agreement.
