II. Gordons 11 Functional Patterns A.

HEALTH PERCEPTION & HEALTH MANAGEMENT PATTERN The patient rated her own health 6 from the scale of 1-10 and 10 being the highest because of his illness. The client stated, kasi po may sakit pa ako kaya di ko masasabi na malusog ako. For him a healthy person is a person who can do whatever he or she wants to do or manage everything. The patient does not go for any physical examination in the hospital but claimed they sought medical assistance in the medical center whenever they experienced cough or flu. He said that they were advised to drink medicine for cough and bioflu. According to the client, he experienced cough even before his hospitalization. He experienced productive cough with greenish secretions. He is not allergic to any food. His family had history of heart attack in paternal side. He stated he doesnt exercises regularly but he plays soccer with his class mates at school. According to him, he drinks Chinese multi vitamins her mother had brought. He takes a bath every day, trims his nails thrice a month and brushes his teeth twice a day.

B. NUTRITION AND METABOLIC PATTERN The client was asked about his usual eating habits before he got hospitalized. He verbalized, Mahilig ako kumain ng chichirya. Sa school naming palage ako kumakain ng chichirya. Pero kumakain din naman ako ng gulay. Magana naman po ako kumain The client stated that his sister was the one who prepares their food. He always drinks 7-8 glasses of water every day and he does not skip meals. He stated he takes a multivitamin once a day. While in the hospital, the client stated that he ate foods that the hospital provided which was less in fat and salt. Also, it was observed that the client has good appetite. He ate the food her mother bought which was spaghetti which was rich in carbohydrate and fruits like apple and oranges. The estimated oral intake of the client in the hospital was 1400ml/day.

C. ELIMINATION PATTERN The client stated that before hospitalization he defecates twice a day. He verbalized, wala naman problema sa pagdumi ko. Di naman ako nahihirapan. He stated that the color of his stool was brown. He urinate 5-6 times a day with yellowish color and moderate in amount. He said that whenever he feels the urge to urinate and defecate, he does it. He stated that he doesnt feel any pain while urinating or defecating. But since he was admitted in the hospital, he defecates every other day and urinates 5-6 times per day. The color of his stool was also brown and semi-formed.

D. ACTIVITY AND EXERCISE PATTERN He stated that he helped his mother do their householdchores. He said he washed plates and takes care of his younger sibling. The client stated that he doesnt exercise regularly but he

plays soccer whenever he had extra time in school. Also, after school hours the client stated he also plays computer game for two hours. The client stated, kahit di ako nag eexercise naglalaro naman po ako ng soccer sa school namen parang yun na rin ang aning exercise. He said in the hospital he doesnt able to do what he likes to do. She verbalized, Ngayon po ay medyo naiinip na ako. Gustong gusto ko na din po umuwi at magawa ang mga bagay na dati ko na pong ginagawa. E. COGNITIVE-PERCEPTUAL PATTERN The client is able to read, write and speak well. With regards to his senses, he said that he doesnt have any problem with his sense of sight and hearing. He said that he is already in grade six. The client verbalized, maganda naman po mga grado ko pumapasa naman po ako sa school. Namimiss ko na nga po mag aral.

F. SLEEP AND REST PATTERN Before the client was admitted to the hospital, he usually sleeps at 11:00 pm and then he wakes up at 5:00 am for his school. He said he sleep 7-8 hours at night and takes a nap for 34hours at afternoof after school hours. The client stated that wala naman ko problema sa pagtulog ko. Nakakatulog naman ako ng maayos. He also stated that he always pray to the Lord and drinks 1 glass of water before going to sleep. During his stay in the hospital, the patients sleep pattern has been interrupted because of the environment in the ward. He stated, Minsan naninibago ako sa hospital dahil maliwanag at maingay dahil sa makina kaya hindi ako makatulog. But he stated he takes a nap during day. He said he sleeps a total of 7-8hours in the hospital.

G. SELF-PERCEPTION / SELF-CONCEPT PATTERN When the client was asked about his perception about himself, she would rate it 6/10. He stated, medyo di pa kasi ako magaling dahil medyo hirap pa ako makahinga. Hinihingal pa ako. At medyo kumikirot pa rin ang opera saken. When asked about his pain scale, The patient rate it 7/10. He said the pain was not consistent unlike before. It usually happens when his coughing. When asked to rate her self esteem, 1 being the lowest and 10 being the highest, he gave a 8. According to her, even his sick he is an obedient child that is why hes improving. His mood is usually calm. He is very cooperative.

H. ROLE-RELATIONSHIP PATTERN

The patient lives with his mother and 4 siblings in paranaque. He is the 4th child in 5 children of his parent. His father died 3 years ago. He stated, wala naman kong proble sa relasyon ko sa pamilya ko. Sinusuportahan naman nila ako lalo na nang aking nanay. I. COPING-STRESS TOLERANCE PATTERN

The client rated his stress level 8 from the scale of 1-10 and 10 being the highest because eventhough he doesnt had any relative around in the ICU the nurses was helpful and friendly to him. Whenever he had a problem, he seldom opens it up to his family and friends but he opens it up to the Lord by praying. His perception about stress is when he does not know what to do about a certain situation. His hospitalization is one of his stressors. He verbalized, gusto ko na kasi umuwi dahil wala akong magawa dito. Naiinip na ako.

J. VALUE BELIEF PATTERN He is a Roman Catholic and he practices praying and going to mass every Sunday. He also stated that he believes in swerte and Malas. In terms of medical practices, his family consults only in medical advices. They dont believe in any cultural practices regarding health. He practice cultural traditions like pagmamano and saying po and opo to elders.
