1. SITE ACCEPTANCE/COMPLETION CERTIFICATE

Upon completing each phase of work, the supervisor is responsible for documenting that status in a Certificate of Completion and placing it in this section of the Site Folder. Doing so certifies that: Work has been completed to the required standard. All documentation has been completed. Test results are within NSN specifications. Customer-specific parameters have been applied. Outstanding Fault Reports and SDRs have been noted in Section 2.

Without a Certificate of Completion, the next phase of work cannot begin. Only when a Certificate of Completion has been produced for each phase of work can the Site Acceptance Certificate be completed. A NSN Networks Customer Operations Representative and Customer Representative must complete this form upon system acceptance. Original: Customer Copy: Accounting Copy: Project Manager

3. SITE SURVEY REPORT

The purpose of the Site Survey is to check the progress of any applicable Civil Works and identify any shortcomings or potential problems prior to installation. When applicable, Civil Works must be completed before the site will be considered ready for the NSN Networks Installation Team to begin the installation project. It is the responsibility of the Installation Supervisor to determine whether a site is ready for installation, and whether a Site Survey Report is necessary for this project. A site could be rejected by NSN if: Safety to personnel may be compromised. Civil work is not of the required standard. Insufficient utilities exist.
