October 14, 2013 Dear L.A.C.F.C Families, As you are likely aware, there are player-assistance grants available in this region, through the local chapter of Kidsport Ontario, and the Jumpstart program. These grants range from $50-$250 per player for Kidsport, or are based on financial need for Jumpstart, and are available to help families in need, to better afford registering in a recognized league, and/or to assist with the purchasing of equipment (such as shoes and shin guards). In the case of Jumpstart, funding is also available to assist with transportation needs. This letter is intended to provide more information to families that have expressed an intention to participate in the winter indoor season. Please note that the details that are provided below are for information purposes only, and are not intended to pre-qualify anyone for any non-LACFC granting program. Program eligibility is handled outside of L.A.C.F.C. In order to qualify for Kidsport, you must be able to answer Yes to all of the following questions: 1. 2. 3. 4. 5. Is my household income less then $39, 000 per year? Can I provide tax return statements or pay stubs to verify my income? Is my athlete/child between the ages of 6-17 years? Am I a resident of Ontario? Can I get an endorsement letter, verifying my financial need, from one of the following people: caseworker/social worker, teacher/principal, police officer, accountant, member of clergy, lawyer, employer? 6. Would my family benefit from a grant from Kidsport Ontario? 7. Can I complete an application as soon as possible? (Ideally, applications are submitted at least 45 days prior to the start of the season, however, if there is still unused money in the Kidsport budget, you may still qualify if you apply as soon as possible as grants are on a first-come, first-served, basis). The Kidsport application form and completion instructions are provided with this letter. Completed applications should be directed to Kidsport London. 316 Rectory St. P.O. Box 7550. London, Ontario. N5Y 5P8. In order to qualify for Jumpstart, you must be able to answer Yes to all of the following questions: 1. Is my athlete/child between the ages of 4-18 years? 2. Can I get an endorsement letter from a community leader, verifying that I have an athlete/child in need of financial support? 3. Can I complete an application prior to the start of the winter season? (you must submit your application as soon as possible to be considered). 4. Am I prepared to contact Jumpstart at 1-877-616-6600 to identify where in London I will make my application? 5. Would my family benefit from a grant from Jumpstart? 1

The Jumpstart application must be obtained by contacting Jumpstart at the number listed above. Where you make your application will depend on your home address. Families in need, that have answered yes to the above questions, are able to apply to both programs. IMPORTANT: If you intend to apply for a Kidsport or Jumpstart grant, you will not qualify if you have already paid the registration fee to the club or have already purchased your equipment. These programs do not reimburse players for purchases that have already been made. Also, please note, that registration grants are paid directly to the club. In the event that an athlete receives money in excess of the registration fee, by applying to both granting programs, LACFC will ensure your ability to apply for assistance in future years, by returning unused portions to the granting agencies.
