She lives in her own apartment and next door to seven other consumers. When at home, SC spends most of her time alone. She has friends she speaks with on the phone, but does not visit them at their residence. SC gets along with everyone. SC works Monday through Thursday at the Tivoli during school months and at Bruno during the summer months. SC enjoys her job. SC visits her mom about once a month which she enjoys. SC accesses the community I and c staff. SC has safety skills which allow her to walk to the convenience store and back I. SC enjoys going Referral from:_____________________ out to eat, movies, video games, taking care of her bird, swimming, reading, bowling, and going to sporting events. SC does well in small group settings and excels 1:1. When in large groups, SC has more difficulty engaging fully, enjoying her time, and tends to be more emotional. SC has limited opportunity to get out in the community2 limited transportation and funding. Assessment findings: 1 Diagnoses consist of the following: Intellectual Disability, Cerebral Palsy, Bipolar, Schizoaffective Disorder, Hypothyroidism, GERD, Reflux Esophagitis

Limitations: SC is I in ADLs needing some verbal assistance to do laundry and some hygiene and grooming. SC requires occasional support from staff for appropriate expression of wants and needs, problem solving and attending to task, especially when she is anxious or manic. SC has been seeing a therapist for counseling. SC requires staff support and assistance to manage bipolar mood changes, hearing of voices that tell her to do or not do things which sometimes result in inapportporiate behaviors.

Goals and Objectives Goal 1.1 Cl will leisure opportunities (Created 1.15.11) Objective 1.1.1 When asked to participate in a STEP activity by group home staff and/or STEP staff, SC will agree to attend the activity. (Created 1.15.11) Goal 2.1 Cl will 1:1 leisure involvement (Created 1.15.11) Objective 2.1.1 When asked to participate in STEP program by group home staff and/or STEP staff, Cl will agree to participate when she is interested (Created 1.15.11) Goal 3.1 Cl will personal decision making (Created 1.15.11) Objective 3.1.1 During activities, SC will make at least 3 decisions I without assistance from CTRS (Created 1.15.11) Objective 3.1.2 During a session or when asked by CTRS, Cl will choose the next activity she does with step, with assistance from CTRS (Create 3.10.11)

Implementation Plan Goal 1 CTRS will set up and provide transportation once a month to SC on an activity of her choice. Each session will last at least one hour. Goal 2 CTRS will set up and provide transportation once a month for SC. The setting will always be 1:1 unless otherwise suggested by SC. The setting may be in the community, but there will not be any other DDRC consumers during our sessions. Goal 3 CTRS will provide SC with at least three opportunities to make a decision during each session. In addition, SC will be responsible for creating the following months activity. This will be implemented once a month, every month.
