We are very pleased at the membership of our son / daughter, _______________________, with the University of the Philippines Manila Chorale [UPMC], the official choir of the University of the Philippines Manila, under your direction and guidance. In connection with the UPMCs 7th International Concert Tour [the Tour] from 12 April to 12 June 2013, spanning Japan, Germany, Belgium, France, Switzerland, Austria, Slovakia, Hungary, Italy and Spain, we take this opportunity to inform you that WE ARE HEREBY GIVING OUR PERMISSION AND AUTHORIZATION FOR HIM / HER TO TRAVEL WITH THE UPMC for the duration of the Tour. On the part of our child, we warrant that he/she is committed to being physically and mentally fit to perform at his/her best for the entire duration of the trip. We shall take full responsibility for our childs acts or omissions that may jeopardize or put in bad light not only the goodwill and sterling reputation in the international choral scene that the UPMC has established and enjoyed over the years, but also the image of the University of the Philippines. We fully understand that any such untoward behavior on the part of our child shall be dealt with appropriately; in particular, his/her non-appearance during the Tour shall be immediately reported to the police and immigration authorities of the country concerned. We expressly manifest that our consent and our childs commitment to join the touring group are given fully aware of existing contingencies, i.e., the easy and fast spread of communicable diseases and the present

calamitous state of the international peace and order situation, particularly the continuing threat of terrorism in various parts of the world. Please be assured that neither our child nor we shall hold you, other UPMC tour personnel or members, or the University of the Philippines responsible or accountable for any injury or loss that our child may suffer in the course of UPMCs 7th International Concert Tour. Relatedly, we are aware of, and also should like to take this opportunity to express our gratitude for the travel and health insurance that the UPMC has graciously secured for its members. Final paragraph for those with fulfilled quota (100%) on or before April 6, 2013 (please remove after editing): Finally, our child and we are fully aware of his/her commitment and obligation to remain a performing member of the UPMC for at least SIX [6] months from arrival, or until December 2013. We appreciate that this requirement is intended to ensure that all financial obligations of the group [e.g., to replenish the seed funds for continuing and future activities] are provided for and that incoming trainees and members are properly oriented and trained. Final paragraph for those with unfulfilled quota (<100%) on or before April 6, 2013 (please remove after editing): Finally, our child and we are fully aware of his/her commitment and obligation to remain a performing member of the UPCM [a] for at least ONE [1] year from arrival, that is, until June 2014, or [b] until such time that he/she has fulfilled his/her quota, whichever period is longer. We appreciate that this requirement is intended to ensure that all financial obligations of the group [e.g., to replenish the seed funds for continuing and future activities] are provided for and that incoming trainees and members are properly oriented and trained.
