PURPOSE This guideline describes the vaccination requirements for workers of the Division of Facilities Management. The purpose of vaccination is to prevent the risk of acquired infections by vaccine preventable diseases, for categories of workers that are at occupational risk. 2. SCOPE This policy applies to all University workers of the Division of Facilities Management. This includes permanent and casual workers, whether working full time or part time. 3. RESPONSIBILITIES a) Supervisors/ Managers. Supervisors/ Managers must ensure that any workers under their employ defined in the occupational categories below are offered immunisations as listed in Table 1 Recommended immunisations for those at risk of occupationally acquired infections to vaccine preventable diseases. b) Supervisors/ managers should engage a medical practitioner to provide advice on any worker roles and the appropriate level of vaccination, initially and after any role changes as required. c) Workers. Workers offered vaccination are required to maintain their personal vaccination

record Adult Vaccination Record Card. If a worker is in a risk category and does not consent to vaccination, they must indicate this in writing. An assessment of the risk of a non vaccinated person will be undertaken in consultation with the worker. Workers are required to seek timeline for re-vaccination from a medical practitioner and this must be recorded on the vaccination record. d) Supervisors/ Managers and workers must ensure that the records and review dates are provided to the Division of Human Resources for inclusion on the staff members personal file. 5. PRIVACY AND PERSONAL INFORMATION The personal information that you provide is protected by the Privacy and Personal Information Protection Act 1998 (NSW). You are required to provide vaccination information to enable the University to ensure its duty of care required under the Work Health and Safety Legislation 2012. Vaccination guidelines v1.0 ~ 3 ~ Access to the information you provide is available to yourself, and those persons authorised to access the information in the course of their duties to the University. 6. COSTS Where vaccination is required to protect workers while undertaking their normal duties at the University, then the costs associated with the ordinary vaccination is the responsibility of the Organisational unit. These costs are for direct out of pocket expenses and do not include those that are covered from other sources eg Medicare 7. ADDITIONAL NOTES a) This guideline will require review with the release of new vaccines b) If staff feel that their nominated category does not accurately reflect the risks associated with their duties, they are to raise with the Facilities OHS Manager for advice. c) Although not covered within this guideline, staff should consider vaccination, in consultation with their medical practitioner, against childhood illnesses such as Diphtheria, Polio, Measles, Mumps, Rubella, as well as seasonal Influenza and Swine Flu. d) Workers should consult with their medical practitioner regarding their vaccination requirements and any concerns they may have with their Medical Practitioner. Workers should maintain their Tetanus vaccination current with a booster at intervals as recommended by their medical practitioner
