Objectives

A.) GENERAL OBJECTIVES: -At the end of the medical rotation, the students will be able to render safe, comprehensive nursing care to a medical client, assist in close coordination with the member of the health team, and apply the nursing process in rendering nursing care to clients. B.) SPECIFIC OBJECTIVES: 1.) QUALITY AND SAFE CARE -Demonstrate professional attitudes and habits towards: a.) Clients and significant others. b.) Other members of the health team and affiliates. -Evaluation of health care rendered to patients. 2.) MANAGEMENT OF RESOURCES AND ENVIRONMENT -Observes ward policies, procedures and protocols on infection control and waste disposal. -Observes organization of work and proper budgeting of time and resources. 3.) HEALTH EDUCATION -Provides health education based on identified learning needs of the client. 4.) LEGAL RESPONSIBILITIES -Check the completeness of informed consent and other legal forms. 5.) ETHICO-MORAL RESPONSIBILITIES -Respect clients Bill of Rights at the ward. -Adheres to the code of Ethics for Nurses. 6.) PERSONAL AND PROFESSIONAL RESPONSIBILITIES -Demonstrates professional behavior at all times. 7.) QUALITY IMPROVEMENT -Reports significant actual or potential observations regarding the client and the ward practices. 8.) RECORDS MANAGEMENT -Maintains legible, accurate and updated documentation of patient care in the chart. 9.) COMMUNICATIONS -Communicate therapeutically with the client and their significant others. -Establishing professional relationships with members of the health team. -Utilizes proper channel of communication. -Observes complete and accurate endorsements procedures. 10.) COLLABORATION AND TEAM WORK -Maintains a collaborative relationship with other members of health team.

Day 1 (September 16, 2013)


DAY/TIME 6:30-6:45 ACTIVITIES -Arrival at the waiting area. RATIONALE -To meet with other group members.

6:45-7:00

-Checking of attendance, uniforms and paraphernalias.

-To check the readiness of group members and to check if their paraphernalias are complete and functioning. -To pay respect to the health care providers and inform them about our presence. -To know their specific area of responsibility. -To know the assigned task or procedures to be done to the patients and to know any improvements in the patient care.

7:00-7:05

-Courtesy call to the head Nurse, Staff Nurses, and other health care personnel.

7:05-7:15

-Assigning students to their specific patients. -Attend to endorsement and patient rounds.

7:15-7:40

7:40-8:00

-Getting of initial vital signs and checking of availability of oral medications..

-Serves as a baseline data for them and to have a basis in making their charting and in caring for their patients.

8:00-8:30

-Attend to patients need at bedside.

-To provide quality nursing care. -To check patients vital signs if its within normal range. -To make sure that all of the patients are attended. -To make sure that all of the patients are attended. -To record their patients nursing problem and the interventions they have done to alleviate their patients condition. -To check the clarity and soundness of documentation. -To check the patients condition before the end of the shift and check whether the goals of care have been achieved. -To serve as a legal document of patient care.

9:30-10:00

-Checking of vital signs and plotting.

11:00-11:30

-Break time, half of the group will have their break but they are not allowed to go outside. -Other half will have their break and the other half will attend to patient needs.

11:30-12:00

12:00-1:00

-Students will do their sample charting.

1:00-1:30

-Checking of students charting by the clinical instructor. -Getting of Vital Signs, intake and output then documentation. -Charting to patients chart.

1:30-2:00

2:00-2:30

-Giving of necessary reminders regarding activities for the next day. 2:30-2:45 -Instruct the student nurses to do after care in the students lounge and proper disposal of their waste. -Endorsement to the incoming group.

-To prepare for the next activity -To obtain cleanliness of the students lounge before leaving the area.

2:45-3:00

-For continuity of care.

Day2 (September 17, 2013)
