LPN REQUIREMENTS

PLEASE READ THE FOLLOWING INFORMATION CAREFULLY WHEN

YOU APPLY TO THE LICENSED PRACTICAL NURSING PROGRAM

1. You must take the TABE Test or provide proof of an A.A. or College degree from a
regionally accredited institution .There is a $30.00 application fee for the test.
2. Provide either a copy of your high school diploma from an accredited high school or GED.
Please check website at www.accreditedschools.org to determine acceptable accreditation

Please note that your name will be added on the list ONLY after you take the TABE test or proof
of any postsecondary degree earned

Other requirements that will be needed on or after you are informed by mail that you are
accepted into the program

a. Background check (additional cost-approx.$60.00 )

b. CPR-BLS for Health Care Providers ( additional cost –approx $45.00)
c. Physical Exam ( additional cost)
