Classroom Rules of Conduct: 1) Attendance every class schedule EXCEPT (sick or death of immediate family member) Provided submission of: Sick Medical Certificate within one week from the base hospital : Death certificate within two weeks : No make-up quiz, long test, examination shall be given due to absence or tardiness

2) Professional Attitude/ Behavior: Respectful of classmates and instructor : Punctuality : Honest in dealing with others : No use of phones : Study before class : Participate in all classroom activities : Submit requirements as schedule : No cheating : Submit certificate of registration on the first day of class : Complete uniform and grooming : Examination permit is required before periodic examination Course Policies: 1. All requirements must be turned in on the specified deadline. 2. A certificate of registration is required on the first day of class. 3. Wear the prescribed uniform with valid ID. No wearing of jewelry and other accessories. 4. Behave and dress in a manner that is wholesome and not distractive to the learning environment. 5. Mobile phone must be set on silent mode while class is going on. Any electronic device will be confiscated if used during class activities. This will be given to the DSA office. 6. Punctuality is a must. 7. A student is required to attend class regularly. Excused absences are: illness (present a medical certificate from the base hospital), environmental constraints, death of an immediate family member (present a death certificate within two weeks), and field trips or seminars in other subjects. Any student who has incurred more than 20% unexcused absences from class per school term (8.4 hours only allowable absences) will automatically receive a grade of FA (failure due to absences) regardless of his standing in class. 8. Dishonest acts will be dealt with accordingly based on the guidelines set in the student handbook. 9. Quizzes and examination must be taken on the scheduled time. No make-up quiz shall be given due to absence or tardiness. 10. Secure an examination pass from the Business Office before taking any periodic exam. 11. A student who for any valid reason has not completed all class requirements by the end of the term is given a grade of INC (incomplete). Completion of INC grades must be within 45 days after the final examination week; otherwise such INC grade automatically falls into F or Failure. 12. Going to the restroom should be done before class will start or during breaktime.
