GENERAL OBJECTIVES: The student nurses of group 3C will be able to:

1. Conduct house to house visit to the family. 2. Assess the physical, social, cultural, environmental and health status of the individual family and community guided by the Family Health Assessment Form. 3. Illustrate a spot map in areas specifically in Sitio Lupa.

4. Identify the population or the families living in Sitio Lupa. 5. Identify factors affecting the families condition 6. Develop a plan of care for the family, effectively. 7. Implement the family nursing care plan. 8. Organize a team building within the Brgy. Health Workers through coordinating services with various members of the health team; informal leaders in the community Government and Non-Government agencies in order to achieve the goal of public health. 9. Develop Health Teaching programs and activities for the families in community to different age groups.

ACTIVITIES We departed from school to the area in Brgy. Sapangdaku, Cebu City We had our morning circle and instructions given by our clinical instructors.

We conducted a house to house visit and illustrated a spot map particularly in upper Sitio Lupa Brgy. Sapangdaku ,Cebu City We gathered in the Health Center and departed from health center to the school in USJ-R Main Campus. We had our lunch break. We were given final instructions and the activities for tomorrow. Same in Mondays 7:00-7:30 We had our morning circle and instructions were given by our clinical instructor. We continued our spot mapping and we also had our family surveying for the population In Sitio Lupa and assess the familys condition through the Family Health Assessment Form during our house to house visit. Our departure for Sapangdaku was cancelled due to rain so that we had just our breakfast. We had our tallying for the information gathered in our Family Health Assessment Form and continued tallying the population. We had our lunch break. We made an annotated reading about the community diseases.

Same in Monday. We had our morning circle and instructions were given by our Clinical Instructors. We continued our Family Health Asses ment in Sitio Lupa and we also gave food stabs to the children. We gathered back in the Health Center and discuss about what programs we will have for the children in Sapangdaku in the following day, Friday.

We also planned for the giveaways, snacks, and prizes for the children in our activity for the next day. We gathered in the school USJ-R Main Campus for departure. The first were asked to prepare for the food and snacks for the children, and the second group were asked to gather the children in the area for the health teaching,hand washing and proper tooth brushing to the Brgy.
