Subcutaneous Injection (SC) Explanation Subcutaneous (SC) is given the medicine by injection and inserted on a layer of fatty tissue under the skin.

Indication 1. Injection for vaccine 2. Giving medication preoperative 3. Giving insulin for diabetic patient 4. Giving heparin (anticoagulant) 5. Patients receiving narcotic drugs

Contraindication There is injury on the skin or non-intact skin integrity, pain, florid and swelling at the injection area.

Locating Injection site 1. Locate injection sites on the arms, fold one arm across the chest. Place your hand on the shoulder and draw an imaginary line below your hand. Place another hand on the elbow.Draw an imaginary line down the outer side of the arm and down the center front of the arm, starting at the elbow. The area inside these imaginary lines is where injections are given. (If you are injecting imagine the hand placement.)

2. To locate injection sites on the thighs, sit down, place your hand above the knee, and draw an imaginary line above it. Place your hand at the uppermost part of the thigh and draw an imaginary line below your hand. Draw an imaginary line down the outer side of the leg and down the center front of the leg. The area within these imaginary lines is where injections may be given. 3. To locate injection sites on the abdomen, place your hands on the lower ribs and draw an imaginary line them. Use this area below your hands for injections, as far around as you can pinch up fatty tissue. use a 1inch area around the navel.

Preparation of Instrument: 1. Tray Lined 2. Syringe (size have variation, appropriate with volume of medicine are given 1-3ml) 3. Needle appropriate size (25-27G), length the needle is 3/8-5/8 inch 4. Antiseptic swab (Alcohol) on the spot 5. Gloves 6. Cotton Balls 7. Ampul medicine or vial appropriate instruction and liquid solvent (if needed) 8. Container instrument 9. Blank Crooked 10. Lining 11. Disinfectant Solution 12. Drug card or records of medicine are given

Preparation of Patient: 1. Give greetings, introduce your self and identification the patient with investigate identity of patient, check medicine program include 12 correct. 2. Explain the procedures for action.

3. Review medical history and allergy of drugs. 4. Give opportunity to patient for ask and answer all of question. 5. Request visitor to leave the room, give privacy for patients. 6. Adjust the position of patient, so that feels save and comfortable. Procedure: 1. Inform to patient that the action will be initiated. 2. Prepared instruments and drug card. 3. Wash hands and use gloves. 4. Prepare the dose of medication correctly from viral or ampoules. Drugs that have been placed in a tub prepared injection. 5. Determine the area to be injected. Inspection of the injected area if trere are edema, mass or tenderness. Avoid scarring, bruising, abrasion or infection. 6. Help patients take a comfortable position 7. Keep blankets or clothes that do not cover the part of injected area. 8. Place a lining below injected area. 9. Clean the injection site with antiseptic swab. Wipe the center of the injection with the direction of motion of spinning out all around 5cm. 10. Open the cap of the syringe from the needle by pulling it straight direction. 11. Hold the syringe between thumb and index finger or middle finger of the dominant hand. 12. Puncture needle syringe into the injection area. For patients who have an average body using the dominant hand to stretch the skin at the injection site. For obese patients, pinching the skin at the injection site. Insert the needle quickly and firmly at an angle 45-90. 13. Hold the lower end of the syringe body until the end of suckers by the nondominant hand, do the aspirations. If there is no blood, inject the drug slowly, if there is blood in syringe, loose needles, discard syringes and medicine and repeat the procedure. 14. After injection, pull the needle syringe gently but quickly at the same angle insertion. Apply alcohol swab in injection site, give a light massage. When

transfixion bleed, press the puncture area by dry sterile gauze until the bleeding stops. 15. Close syringe needle with the cap, discard syringes on bent before they are discarded into special trashed. 16. Take the lining. 17. Help patients to its original position. 18. Clean up equipment and take gloves off. 19. Note any medication that are given, write down the nurse's initials and signature. 20. Evaluate the patient's response after the action. 21. Give positive reinforcement. 22. Contract for the next meeting and end the meeting well. 23. Return all equipment to the nurse station and wash your hands.
