RISK ASSESSMENT FORM FOR RIGGING & LIFTING

LIFT CATEGORISATION ASSESSMENT PART 1 ROUTINE LIFTING OPERATIONS CHECKLIST Six Basic Questions Yes No 1 Has this lifting operation been performed before? 2 Is there a documented lifting plan and lifting operation risk assessment in place? 3 Are you experience with all the lifting equipment/gear to be used? 4 Has the load been checked and made ready for lifting (e.g. Hold down bolts released)? 5 Have you the experience to lift a load of this weight? 6 Is the lift in an area free from obstructions and other possible hazards? If the answer to any of the above is No, go to Part 2 of the Lifting Operation Assessment Procedure.

LIFT CATEGORISATION ASSESSMENT PART 2 SIMPLE LIFTING OPERATIONS CHECKLIST Questions Yes No 1 Is the lifting operation to be undertaken by a single lifting appliance only? 2 Do you know the weight of the load and does the lifting operation appear to be straightforward? 3 Is the load heavier than you normally handle, do you have the relevant permission and/or permit? 4 Is there a crane or certified support steelwork (eg runway beam or lifting N/A eye) directly above the load? 5 Does the load have certified lifting points (lifting eyes/collar eyebolts, etc) fitted and if not, can slings be wrapped around easily (e.g., no sharp edges, load not fragile, etc) 6 Is there ample headroom for the lifting appliance and slings? 7 Is the lift stable (e.g. center of gravity below lifting points)? 8 Is the lift balanced (e.g., center of gravity in the middle) or fitted with special slings to compensate? 9 Is the load free to be lifted (e.g. all hold-down bolts removed, not jammed, etc)? 10 Is the removal route clear of any obstructions? 11 Can the removal (lift, transfer & landing) be performed without crosshauling? 12 Is there a suitable laydown area and does the load come within the allowable load bearing capacity of the ground/surface? 13 Are you experienced in using all the lifting equipment/gear involved? If the answer to any of the above is NO go to Part 3 of the Lifting Operation Assessment Procedure. If you can answer yes to all the above, proceed with the lift as per the UEP requirements for simple lifts. LIFT CATEGORISATION ASSESSMENT PART 3 COMPLICATED LIFTING OPERATIONS CHECKLIST Complicated lifting operations must be carried out by qualified Riggers, or personnel with similar qualifications and skills in dealing with awkward loads.
