Dental implants in India

Overview: Oral health and dental care is a part of health care and any malfunction in the area needs to be taken care, seek proper advice and treatment to restore dental hygiene and personal well being. If you have gaps in your smile where your permanent teeth used to reside, you may find that more is missing from your life than just teeth. You may also miss out on good nutrition and even social engagements. It doesn't have to be that way, though. Dental implants along with artificial teeth are often a good option to fill the gaps left by tooth loss. Many people find that dental implants are more comfortable, efficient and secure than are dentures or bridgework. What is Dental Implant? A dental implant is a titanium "root" used in dentistry to support restorations that resemble a tooth or group of teeth to replace missing teeth. Virtually all dental implants placed today are root-form endosseous implants, i.e., they appear similar to an actual tooth root (and thus possess a "root-form") and are placed within the bone (end- being the Greek prefix for "in" and osseous referring to "bone"). The bone of the jaw accepts and osseointegrates with the titanium post. The osseointegration is the component of this implant procedure that makes it resemble the look and feel of a natural tooth.

What is the Composition of Dental Implants? A typical implant consists of a titanium screw (resembling a tooth root) with a roughened or smooth surface. The majority of dental implants are made out of commercially pure titanium, which is available in 4 grades depending upon the amount of carbon and iron contained. More recently grade 5 titanium has increased in use. Grade 5 titanium, Titanium 6AL-4V, (signifying the Titanium alloy containing 6% Aluminum and 4% Vanadium alloy) is believed to offer similar osseointegration levels as commercially pure titanium. Ti-6Al-4V alloy offers better tensile strength and fracture resistance. Today most implants are still made out of commercially pure titanium (grades 1 to 4) but some implant systems (Endopore and NanoTite) are fabricated out of the Ti-6Al-4V alloy. Implant surfaces may be modified by plasma spraying, anodizing, etching or sandblasting to increase the surface area and the integration potential of the implant:

What Dental Implants Can Do? Replace one or more teeth without affecting bordering teeth. Support a bridge and eliminate the need for a removable partial denture. Provide support for a denture, making it more secure and comfortable.

Are You a Candidate for Dental Implants? The ideal candidate for a dental implant is in good general and oral health. Adequate bone in your jaw is needed to support the implant, and the best candidates have healthy gum tissues that are free of periodontal disease. Dental implants are intimately connected with the gum tissues and underlying bone in the mouth. Since periodontics is the dental experts who specialize in precisely these areas, they are ideal members of your dental implant team. Not only do periodontics have experience working with other dental professionals, they also have the special knowledge, training and facilities that you need to have teeth that look and feel just like your own. Your dentist and periodontist will work together to make your dreams come true.

What are the types of Dental Implants? There are many types of Dental Implant today. However, they can be categorized in the following three main types, which are as under: Endosteal Implants: - Endosteal implants are similar to subperiosteal in that they are moulded or shaped to fit in a cavity in the jaw rather than sit on top of the jaw. Root Form Implants: These are considered to be the closest in size and shape when compared with the root of the original tooth. They are mostly used in deep and wide jaw bones that provide a wide base for replacing one or more teeth. Plate Form Implants: In cases where the jaw bone is too short or narrow and unsuitable for bone grafting, another type of implants called the Plate form implants are used instead of root form implants. In this method a long and flat implant is fixed into the short or small jawbone. Subperiosteal Implants: - Subperiosteal implants are an option used where the condition of the jawbone is such that an insert is not possible and a bone graft is not feasible. The jawbone is exposed and a mould impression taken from which the subperiosteal implant is created and placed on top of the jawbone under the gum and fusion takes place between the jawbone and the subperiosteal implant. Transosseous Implant: - Transosseous method is similar to endosseous because they are both inserted to the bone. The Transosseous method, however, goes through the entire width of the jawbone to under the chin. It is secured with a devise similar to a bolt and nut.

What are the Common Dental Implant procedures? Root-Form Titanium Implant: Of all the types of dental implants, the root-form titanium dental implant is the most common. This option is also known as an endosseous or endosteal implant, a name that stems from the fact that these tooth implants are placed directly in the bone. Root-form implants are made of the reliably strong and corrosion-resistant metal titanium, making these dental implants very similar in appearance to screws, nails, or cones. Your dentist decides which type of implant to use based on the quality of the bone in your jaw and the type of crown, bridge or denture that will be placed on the implant. Root-form implants can be inserted in a two-stage process the traditional way of placing them or in a single-stage procedure. In the two-stage procedure, the implant is "buried" under the gum tissue for three to four months and then exposed during a second surgical procedure. In a single-stage procedure, the implant is placed in the bone and remains exposed in the mouth. Ramus-Frame Implant: It is commonly used in patients with a thin lower jawbone, ramus-frame tooth implants are placed in the jaw at the back of the mouth and near the chin. Full or partial dentures are then made to fit on the thin metal bar that is visible above the gum tissue, once it heals. This type of implant can be used if the lower jawbone is too thin for a root-form or subperiosteal implant. A Ramus-frame implant is embedded in the jawbone in the back corners of the mouth (near the wisdom teeth) and near the chin. Once it is inserted and the tissue heals, a thin metal bar is visible around the top of the gum. Dentures are made that can fit onto this bar. Ramus-frame implants also can stabilize weak jaws and help to prevent them from fracturing. You are asked to wait for 3 to 6 months to allow healing or osseointegration to take place. In that time, the implant will have fused with the bone of your jaw.

Blade-Form Implant: This type of implant also is known as a plate-form implant. It is a type of endosseous implant (placed in the bone), but it is used less frequently than a root-form implant. These are flat rectangles of metal with one or two metal prongs on one long side. A blade implant is placed in the jaw so that the prong(s) stick out into the mouth where they will support crowns or bridges. These are one of the most popular types of dental implants and have been used by dental professionals for a number of years. An endosseous implant, Blade- Form implants are flat long pieces of metals with 1 or 2 metal prongs that are implanted in your jaw bone. However, these types of implants are very versatile. They are available in different shapes and sizes and many manufacturers even willing to design a unique one for you, matching each of your specifications. Furthermore, these implants can even be shaped and bent to fit disparity in the anatomy. Sub-periosteal Implant: The sub-periosteal implant is commonly used in patients who do not have enough bone in the jaw to support a root-form implant. Sub-periosteal tooth implants are placed over the jaw, rather than in it. A Subperiosteal Implant is used to secure dentures and when the bone has atrophied (receded) and jaw structure is limited. The lightweight, individually designed, metal framework fits over the remaining bone, providing the equivalent of multiple tooth roots. It may be used in a limited area or, if all the teeth are missing in the entire mouth. The amount and location of available bone determine the kind of implant that is best to use. Natural tissue membrane or bone will grow back around either implant making it even more secure. Plate-Form Implant: This category of dental implant consists of a rectangle of metal with either one or two metal prongs on one side. They are placed vertically in the jaw so that the prongs stick up into the mouth and provide a place for the artificial tooth to be placed. This takes the form of a narrow metal plate which is inserted into the jawbone. Your dentist will expose the area of your jawbone for treatment before preparing the bone to receive the implant. He or she will make a series of incisions in the gums before inserting the implants. The number of incisions made will depend on the number of implants. The implants are inserted and the incisions closed with tiny stitches. The dentist will advise you to wait for a set period of time to allow osseointegration to take place. However, in some cases you can have restorations directly after the implant procedure. This is known as immediate loading. Transosseous Implant: Transosseous implants originally were designed to be used in people who had very little bone in their lower jaws and who had no bottom teeth. However, they are rarely used today because placing them requires extensive surgery, general anesthesia and hospitalization. Also, their use is limited to the lower jaw. Placing transosseous implants involves inserting two metal rods from below the chin, through the chin bone, until they are exposed inside the mouth. The rods that can be seen inside the mouth are used to attach a denture. Most clinicians today prefer to use bone grafts and one of the other endosseous implant methods described earlier instead of the transosseous method because they are equally effective and do not require the level of surgery needed when placing transosseous implants. Two stage Dental Implants: Its procedure begins with the surgery. In this surgical process the dental implant is fixed into the jaw bone. The implant is placed in the jaw bone and is screwed in all the way so that the top surface of the screw (implant) is at the same level of the bone. The soft tissue is stitched up and a few months later the tissue has healed. There is no visible evidence of the implant in the bone as it is under the soft tissue and not visible to the eye. During the next appointment, the dentist will perform a minor surgery where he will expose the top surface of the dental implant, connect a dental implant abutment and start the process of the dental implant restoration. At the end of this appointment the patient will go home with the abutment in his mouth attached to the implant. Additional healing time is required, yet it is a short period and there is very little pain involved. Single stage Dental Implants: is designed in such a way that it enables avoiding the second surgery. During the single stage dental implant surgery, the implant is placed in the jaw bone in such a way that the top of the implant is higher than the surface of the bone and is actually at the height of the soft tissue (this is a longer implant). When the soft tissue is stitched up at the end of the dental implant surgery the dental implant head is exposed and visible to the eye. After the healing time has passed there is no need for another surgery for exposing the top of the head of the implant because it is exposed to begin with and all the dentist needs to do is connect the dental implant abutment and start the restoration process. Both dental implant types have the same success rate of 98% and it is usually up to the dentist to decide which type of dental implants to use. There is no big difference, essentially its just what the dentist is used to and feels comfortable working with.

How to Prepare for Dental Implant? Dental implant surgery is an involved process and requires several visits with specialists. Follow these steps to prepare for dental implant surgery: Have a thorough evaluation. You need to get a complete dental exam with X-rays and models of your mouth. Tell your dentist about any existing medical conditions. It's important he know if you have any orthopedic implants, heart or vascular problems. You may need to take antibiotics before getting the dental implants. Run through your dental implant procedure plan with your dentist. Make sure you know exactly what to expect during all your surgeries. Quit smoking and excessive drinking. Both are detrimental to your health and can actually lead to the dental implant procedure failing. Avoid eating or drinking for at least 6 hours before your dental implant appointment. Wear comfortable, loose-fitting clothing on the day of your surgery. You will be sedated, so it's important to feel comfortable. Ask a friend or family member to drive you to the dental office. You will not be allowed to drive for 12 to 24 hours following the dental implant surgery.

What are the Post Operative care following a Dental Implant? A part of making the dental implants surgery successful is to follow your doctors orders during the post-operative care. After your dental implants have been inserted you will be given antibiotics as well as antibiotic oral rinse you need to follow all the specific instructions on how to use both the anesthetic and the oral rinse. Following surgery, you will be asked to bite firmly but gently on a gauze pad to stop the bleeding. Your doctor will give you a package of gauze to take home with you and place over the surgical site. Youll have to make sure to change the pads and use them until the bleeding stops completely. If you have some discomfort after the surgery you will be given a painkiller. You can also decrease discomfort and swelling by applying an ice pack for about twenty minutes for the first six hours following surgery. You can continue applying an ice pack for as long as two days following surgery. In order to minimize swelling and bleeding you may want to keep your head elevated for the first twenty-four to fortyeight hours after the surgery. You have to make sure you relax as much as possible and avoid all strenuous activities for as long as three days following the dental implants surgery. Youll have to drink at least eight glasses of water or fruit juice every day and try to limit your diet to soft foods such as yogurt or soup during the first week of surgery. You will be asked to rinse your mouth with warm salt water solution one teaspoon of salt in one cup of water about three to four times a day to further disinfect the area. You have to make sure you spit carefully to avoid any complications. Once your jaw bone grows around the titanium posts, you may experience a little discomfort around the area that can last for several weeks. An over-the-counter pain reliever, such as ibuprofen or acetaminophen, should make the pain go away.

Knee Replacement Surgery in India

Knee replacement surgery also known as knee arthroplasty is procedure to replace the weight-bearing surfaces of the knee joint to relieve the pain and disability of osteoarthritis. It may be performed for other knee diseases such as rheumatoid arthritis and psoriatic arthritis. The procedure has been proven to help individuals return back to moderately challenging activities such as golf, bicycling, and swimming. Total knees are not designed for jogging, or sports like tennis and skiing (although there certainly are people with total knee replacements that participate in such sports). Knee replacement is a routine surgery performed on over 600,000 people worldwide each year. Over 90% of people who have had Total Knee Replacement experience an improvement in knee pain and function.

Knee Replacement Surgery Candidates A person would be considered a candidate for knee replacement if there is: Daily pain The pain is severe enough to restrict work, recreation and ordinary activities of daily living Significant stiffness in the knee Significant instability (constant giving way) of the knee Significant deformity (knock-knees or bow-legs) that hinders normal function of the knee Damage from arthritic conditions, such as osteoarthritis, rheumatoid arthritis or post-traumatic arthritis

Success Rate of Knee Replacement Surgery: Knee replacement surgery is recognized as a miracle of modern surgery. Most orthopedic experts consider replacement to be the best method of handling arthritis in the knee. Knee replacements have literally put hundreds of thousands of Americans back on their feet and allowed them to enjoy their golden years.

Knee Replacement Surgery Preparations: If you smoke, cut down or quit. Smoking changes blood flow patterns, delays healing and slows recovery. If you drink, don't have any alcohol for at least 48 hours before surgery. Ask your doctor for pre-surgical exercises. If you are having hip or knee replacement surgery, doing exercises to strengthen your upper body will help you cope with crutches or a walker after surgery. Your primary care physician or an internist will conduct a general medical evaluation several weeks before surgery. This examination will assess your health and your risk for anesthesia. The results of this examination should be forwarded to your orthopaedic surgeon, along with a surgical clearance. Shortly before your scheduled surgery, you will probably have an orthopedic examination to review the procedure and answer any last-minute questions. You may need to take several types of tests, including blood tests, a cardiogram, a urine sample and a chest X-ray. Advice your surgeon of any medical conditions you have and of all the medications you are taking. You may need to stop taking certain medications or your surgeon may recommend substitute medications until your surgery. Medications such as corticosteroids, insulin or anti-coagulants will need to be managed before and after surgery.
