with Higher Achievement on Monday, October 12, 2009 from 8:30am to 3:00pm. I understand that I will be
responsible for getting my child to and from my child’s Center on time (see logistics form below).

I give my son/daughter permission to attend this trip and engage in all activities associated with the trip,
as described above. Further, I knowingly assume all risks that may result from his/her participation in
these Higher Achievement activities. I ALSO AGREE THAT TO THE EXTENT PERMITTED BY LAW, I HEREBY
WAIVE ANY AND ALL CLAIMS, LAWSUITS, ACTIONS, OR DEMANDS AGAINST HIGHER ACHIEVEMENT THAT
ARISE OR MAY ARISE IN CONNECTION WITH HIS/HER PARTICIPATION IN THESE HIGHER ACHIEVEMENT
ACTIVITIES. THIS WAIVER INCLUDES ANY CLAIMS OF NEGLIGENCE, OTHER TORT CLAIMS, OR OTHER
ALLEGATIONS OF WRONGDOING BY HIGHER ACHIEVEMENT OR ITS EMPLOYEES. BY SIGNING BELOW I
WAIVE ALL SUCH CLAIMS TO THE EXTENT ALLOWED BY LAW. Nothing in this waiver limits my rights
against any parties other than Higher Achievement.

LOGISTICS:
• Because there is limited availability to attend this trip, only 5 scholars from each
Center will be able to attend. Please turn in your permission slip immediately to
increase your chances of being selected!
• Please plan to have your child at Center no later than 8:30am. Staff will be on-
site by 8:15am.Vans chartered by Higher Achievement will transport scholars to
the law offices from Center.
• The vans will bring scholars back to Center by 3:00pm. Please make
arrangements with your child about how he or she is to get home, and indicate
your plans to staff on the form above.
• Please have your child dress appropriately. No jeans should be worn. Slacks
and a collared shirt should be worn by boys, and girls should wear slacks or a skirt
with a nice top. No hats, please.
• Activities to include: mock legal case on cable network packaging, lunch, office
tour, Q&A session with attorneys and staff.
• Lunch will be provided at the law offices.
• NO MONEY is needed; please do not send money with your child.
