FAMILY NURSING CARE PLAN # 4

Problem Identified: Presence of health threats due to poor home sanitation specifically inappropriate toilet facility. Date Identified: November 13, 2013 Objective Cues: An antipolo type toilet facility. No septic tank. With foul smell. The wall is made up of sacks and it has no roof. It is located 10 steps away from their doorstep.

Family Nursing Problem: Inability to make decisions with respect to taking appropriate health action due to Limited financial resources. Lack of adequate knowledge in the importance of having good comfort room. Inability to recognize the presence of a problem due to ignorance of facts that unsanitary toilet might cause the spread of disease or infection.

Goals of Care: After the enumerate the health of the and keeping it nursing interventions, the family will be able to effects of inappropriate toilet facility to the family and perform proper use of toilet facility clean and therapeutic to all the family members.

Objectives: After 3 days of home visits the family will be able to: 1. Identify the condition as a problem; 2. Demonstrate compliance to the health teachings given: 3. Identify and differentiate the advantages of proper waste disposal in preventing the spread of communicable disease.

Interventions: 1. Establish rapport. R- To gain familys trust and cooperation. 2. Asses the degree of awareness of the family regarding the identified problem. R- To identify the capacity to understand the identified problem.

3. Assess the degree of sanitation of the comfort room. R4. Provide effective information about the various diseases that the family can acquire with inappropriate toilet facility. R- To be aware of the danger that may occur if necessary actions would not be initiated. 5. Discuss the importance of having proper toilet facility. R- To emphasize the need of using the water-sealed toilet bowl. 6. Emphasize the importance of having a toilet-sealed water bowl. R- To increase the awareness on the benefits of having a good toilet facility. 7. Encourage the family to maintain cleanliness of their comfort room. R- To provide cleanliness and good condition of the comfort room. 8. Encourage and demonstrate to perform proper hand washing before and after eating. R- To prevent cross contamination and promote hygiene. 9. Entertain questions concerning about the strategies mentioned to the family. R- To clarify confusions and for the benefit of the family. 10. Refer to the Baranggay Hall for appropriate actions to be made. R- To provide information to the appropriate officials regarding the need of a water-sealed toilet bowl of some residents.

Evaluation: Goal met. The family had identified the effects of inappropriate toilet facility and perform proper use of toilet facility and keeping it clean as evidenced by the family performing the health teachings given.
