DRUG GENERIC NAME: Tranexamic acid BRAND NAME: Hemostan CLASSIFICATION: Hemostatis DOSAGE: 500 mg/cap, 1 cap FREQUENCY: TID ROUTE: PO

MECHANISM OF ACTION Inhibits break down of fibrin clots. It acts primarily by blocking the binding plasminogen and plasmin ti fibrin; direct inhibition of plasmin occurs only to a limited degree.

INDICATION Treatmnet and prophylaxis of hemorrhage associated with excessive fibrinolysis.

CONTRAINDICATION Hypersensitivity Patients with active intravascular clotting because of the risk of thrombosis. Severe renal insufficiency. Patients with microscopic hematuria.

SIDE EFFECTS GIT EENT Gastrointestinal disturbances.

Transient disturbance of color vision. Circulatory System Hypotension Thrombotic complications

NURSING INTERVENTION 1. If a dose is missed, instruct patient to take as soon as possible, unless it is within an hour before next dose. 2. Advise patient to comply with dosage schedule, even if feeling better. 3. Advise patient that excessive perspiration, dehydration and diarrhea may lead to fall in blood pressure; consult physician if these occur. 4. Inform patient that drug may cause dizziness, fainting; lightheadedness may occur. 5. Caution patient to rise slowly to sitting or standing position to minimize orthostatic hypotension.

DRUG GENERIC NAME: Hydrocortisone BRAND NAME: Solu-Cortef CLASSIFICATION: Adrenocorticosteroid Hormone DOSAGE: 100 mg FREQUENCY: q8 ROUTE: IV

MECHANISM OF ACTION Glucocorticoid with anti inflammatory effect because of its ability to inhibit prostaglandin synthesis, inhibit migration of macrophages, luecocytes, and fibro blast at sites of inflammation, phagocytosis and lysosomal enzyme release. It can also cause the reversal of increased capillary permeability.

INDICATION Treatment for respiratory disease

CONTRAINDICATION Systemic fungal infection Acute glumerulonephritis Non asthamatic bronchial disease AIDS TB

SIDE EFFECTS Circulatory system Hypertension Circulatory collapse Thrombophlebitis Hemorrhage Edema Poor wound healing Ecchymosis EENT Increased intraocular pressure Blurred vision Digestive system Increased appetite Nausea Diarrhea Abdominal distention

NURSING INTERVENTION 1. Teach patient all aspects of drug usage, the danger of stopping abruptly after long term therapy and signs of early adrenal insufficiency. 2. Warning patient receiving long term therapy about Cushingmoid symptoms (weight gain and swelling). Advice exercise and use of Vit. D and calcium supplements. 3. Instruct patient to monitor and report signs of infection because long term therapy may be needed to resolve infection. 4. Caution patient on use of OTC Products that contain salicylates and alcohol.

MECHANISM OF ACTION Forms a protective barrier to prevent too much irritating contact with stool. This barrier helps protect inflamed, irritated skin and helps make bowel movements less painful.

INDICATION This medication is used to temporarily relieve burning, pain, and itching caused by hemorrhoids.

CONTRAINDICATION Patients who are allergic to soya and peanut.

SIDE EFFECTS Integumentary system Burning Itching

NURSING INTERVENTION 1. Ask patient is he is allergic to soya or peanuts. 2. Instruct patient that to maintain normal bowel function and reduce the risk of hemorrhoids, eat a proper diet high in fiber, drink 6 to 8 glasses of water each day, and exercise regularly. 3. If a dose is missed, instruct patient to take as soon as possible, unless it is within an hour before next dose.
