Requisitos para la internacin de Brigadas Mdicas Extrajeras: Requirements for Entry of Foreign Medical Missions.

La documentacin debe ser enviada a ms tardar 1 mes antes que ingrese la brigada mdica. (The documentation should be a correspondent to more to take a (1) month before the medical brigade enters to the country). Las Brigadas Mdicas Internacionales deben brindar la atencin mdica y / o quirrgica al igual que el suministro de medicinas completamente gratis. (The International Medical Brigades should offer the medical attention and / or surgical the same as the supply of medicines totally free). 1. Carta del Organismo Extranjero y de la contraparte, explicando los motivos de su visita, la actividad que desempear y los resultados que esperan obtener. (Dirigida a la Licenciada Mara del Rosario Sandino Montes, Directora General de Regulacin Sanitaria), debidamente firmada y sellada en papel oficial del Organismo. 2. Para los Organismos No Gubernamentales establecidos en el pas, deben presentar documentos legales vigentes emitidos por el Ministerio de Gobernacin, detallados a continuacin: (For the Non Government Organisms settled down in the country, they should present effective legal documents emitted by the Ministry of Government, detailed next): a. Fotocopia de constancia de cumplimiento. (Photocopy of certificate of execution). b. Fotocopia de Junta Directiva y Representante Legal (Photocopy of the document of the board of Directors and legal representative) 3. Listado de voluntarios que conforman la Brigada Mdica, con su cargo. [List of names and positions of volunteers travelling with the Medical Mission]. 4. Fotocopia de Currculum Vitae, Licencias vigentes emitidas por el Departamento de Salud y Diplomas de Especialidades emitido por la Universidad , de los profesionales de salud, debidamente certificado por la Seccin Consular Nicaragense en el pas de origen de la Brigada Mdica, o autenticado por un notario pblico autorizado del pas de origen. [Copy of Curriculum Vitae, licenses effective emitted by the Department of Health and Diplomas of Specialties emitted by the University of the Health Professionals, duly certified by the Nicaraguan Consulate located in the Medical Missions country of origin; or authenticated by an authorized public notary of the origin country].

Certificado o carta de donacin emitida por el Donante, con el valor aproximado en moneda del pas de origen, debidamente firmada y sellada en papel oficial del Organismo. (Certified or donation letter emitted by the Donor, with the value approached in currency of the origin country), In official paper of the Organism, properly signed and sealed.

2. Lista de suministros mdicos en espaol, detalladas en el formulario: conteniendo las especificaciones tcnicas de cada producto; (Lists of medical supplies in spanish, detailed in the form, containing the technical specifications of the product; 1. Medicinas: Nombres Genricos, presentacin, concentracin, fecha de vencimiento, cantidades, monto total de la donacin. [Medicines: Generic names, packaging, dosage, expiration date, amounts, total amount of the donation]. Los medicamentos deben ingresar al pas en su empaque original . The medications should enter to the country in their original packing. Fecha de Vencimiento: Debern tener como mnimo un (1) ao de vigencia; vencimientos menores, el Ministerio de Salud determinar de acuerdo a la Ley su ingreso. [Expiration date: Shall have no less than one (1) year of shelf life; if expiration date is less than one (01) year from now, the Department of Health shall determine their entry into the country, as per the Law]. No Se aceptan donaciones de medicamentos que contengan estupefacientes o psicotrpicos, controlados sujetos a fiscalizacin nacional e internacional. [Do not accept donations of medicines containing narcotics drugs and psychotropic substances controlled subject to national and international inspection] 2. Dispositivos Mdicos (DM) [Medical Devices (DM)] a. Material de Reposicin Peridica (Materiales Descartables) nombres genricos, cantidades, fecha de vencimiento (si precede). (Material of Periodic Reinstatement (Material Descartables) you name generic, quantities, expiration date (if it precedes)).

b. Instrumentos y Equipos Mdicos: Nombres, # de lote, cantidad, valor, marca, modelo, serie, Pas de origen, fecha de fabricacin y vida til. (Instruments and Medical Teams: Name, # of lot, quantity, value, marks, model, series, country of origin, manufacturing date and shelf life.) c. Instrumentos y Equipo Mdico de Internacin Temporal: igual que el inciso b), debiendo marcar con una X en la ltima columna, para diferenciarlos de los que son donacin. [Instruments and Medical Equipment of Temporary: Same as item b), must mark an X in the last column, to differentiate them from those who are donating.] 6. En caso de que la brigada mdica no ingrese insumos mdicos (medicinas, materiales descartables, instrumentos y equipos mdicos) como parte de equipaje, deben enviar carta del pas de origen en el que especifique tal caso. (In case the medical brigade doesn't enter medical supplies (medicine, materials of periodic reinstatement, instruments and medical supplies) as baggage part, they should send letter of the origin country in which specifies such a case). 7. Si la atencin ser brindada en los Hospitales, adjuntar carta firmada por el Director General del Hospital, en donde acepta que desempeen la actividad propuesta. [If the care is to be provided in Hospitals, attach letter signed by the Hospitals General Director where they accept the performance of the proposed activity]. 8. Carta emitida por el SILAIS de la localidad donde brindarn la atencin mdica, en la cual autoriza el desarrollo de la actividad en mencin . Letter emitted by the SILAIS of the town where they will offer the medical attention, in which authorizes the development of the activity in mention. En el departamento de Managua, primero deben hacer la solicitud en el Centro de Salud ms cercano a la localidad donde realizarn la jornada, quienes les brindarn carta autorizacin la cual debern presentarla en el SILAIS Managua para que les emita el Aval que debern presentar en esta Direccin General de Regulacin Sanitaria.
