1. Ineffective airway clearance increased sputum production in response to respiratory infection secondary to PNEUMONIA

Justification
>Airway must be given the first attention as based on the rule of ABC which is Airway, Breathing and Circulation. In addition, difficulty of breathing can cause anxiety to the client that is why, immediate attention must be done. Addressing the problem to proper health care provider will give patent airway to the client. Oxygenation is a vital need for every cell, if there are any problems related to it can easily affect the functioning of the individual. >Retained secretions can cause blockage of airway which will further cause difficulty of breathing (Fundamentals of Nursing 8th ed by Kozier and erbs p. 1299) >This is the second prioritize problem because based on the ABC (Airway, Breathing and Circulation) this belong to the circulation where there is an insufficient exchange of gas which decreases the supply of oxygen to the body which is needed by the cell in the body >This problem is under circulation in ABC, which mean there is an inadequate circulating blood in the body and we adequate blood to meet of physiologic need and according to Maslow we need to meet our physiologic need to able to go to the next level of the hierarchy of need. >According to Maslows hierarchy of need we need first to address to physiologic needs, nutrition is one of our physiologic need. >Proper nutrition also helps in the recovery of the patient. >This is under the Safety and Security of Maslow, in which the client is prone to injuries like fall or bumping to thing so we need to keep the patient in bed and attend to her needs, this also hinder the patient in self-actualization. > This problem was verbalizes by the patient which the nurse need to address. This was the result of the infective clearance of the airway in which the client needs to cough persistently. >the pain is tolerable rated as 3/10 >We should not only attend to the physical needs of the client we should deal with their emotion because we should treat them Holistically according to Jean Watson >As a nurse one of our jobs is to educate the client about what they are going through. This will give the patient clearer understanding and also help in reducing their anxiety. > This will promote comfort to the patient, it also reflect the job of the nurse. >Last prioritize problem because the client is only kept in bed and dont do anything strenuous.
