Required Documents

Listed below are the documents that you will need to furnish in Photocopy attested by a Gazetted Officer at the time of joining. Joining will not happen without these documents. a. b. c. d. e. f. Letter of Intent / offer Proof of Date of Birth i.e. Class 10th Board Certificate Photo ID (ONE of the following: Voters ID, Drivers License, Passport, Ration Card) PAN card AADHAR Card (Mandatory for PF Enrolment) Address proof (ONE of the following: Passport, Voters ID, Electricity bill (latest) of Self or Parents, Water Bill (latest) of self or parents, Bank Statement (Latest), Ration Card, Telephone landline bill (Latest) of self or parents or Current lease deed- with you or parents as lessee or co lessee) g. Updated Resume h. Most recent and Highest Qualification Certificates i. Professional Relieving Letter from previous four employers j. Last drawn pay slip k. Nationality Proof (Voters ID, Passport, Drivers License or any Government approved proof indicating nationality) l. 7 recent passport size Photographs(35mm X 45mm) m. Referral Information a) Previous HR contact b) 3 Professional references (should not include friends/relatives/peer group) c) 2 Personal reference ( neighbors/friends/relatives/peer group) Please furnish an affidavit in the event of non submission of any of the above mentioned documents with your Name, Date of Birth, Photograph, Fathers Name, Address (permanent and temporary) and Educational Qualification that has been ATTESTED BY A NOTARY. However, you will be required to produce all of the documents mentioned above within 30days from the Date of Joining. HR shall provide you the required format for the same. 2. ESI Nomination:

If your salary is less than or equal to 15000 /- per month minus Employers PF Contribution, you will need to enroll for ESIC (Employee State Insurance Corporation) which is a Govt. regulation; therefore please carry: a. 3 of your post card size (4X6) photographs (copies of the same photograph) Or b. If you would like your family covered Provide family photograph a. Family definition: Self, parents, spouse and children (4X6, 3 Copies of the same photograph). 3. PF Nomination and Transfer:

Information Required: a. Names and Date of Birth of Family members you would want to mention as nominees for the Provident Fund Scheme b. If already a member of a Provident Fund (PF) Scheme with previous employer, then; I. Employers name II. Date of Joining and Date of Leaving service with them III. Previous PF account no. IV. Names of HR and Business Head DOJ: Candidate Name & Signature
