Required Supporting Documents for Air Pollution Installations/ PTO Application

1. Engineering Report to be prepared and signed by a Professional Mechanical Engineer (PME) or Chemical Engineer (ChE) depending on the nature of the manufacturing process and/or Air Pollution Source Equipment (APSE) and Air Pollution Control Facilities (APCF) covered by the application. Information and data shall include the following: a. Flowsheet and description of manufacturing process including corresponding amount of materials used and produced and/or the material balance. If the documents contain business or trade secret, they must be marked accordingly and presented separately. Their contents must describe to the extent possible, short of divulging the secret, important details in order that the third parties can evaluate the effects of the installation to the environment; b. List of APSE and corresponding APCF. Description of process or mode of operation of the APSE/APCF, indicating the types, nature and quantity of expected air contaminants, include justification if APSE is not provided with APCF; c. The design criteria of APCF shall be based on the results of laboratory analyses and pilot plant scale study. The removal efficiency of the APCF for each air contaminant shall be indicated; d. Types and quantities of solid wastes and management program which includes the proper collection, storage and disposal of wastes; e. Total cost of the installations covered by the application; and f. Timetable of Construction covering the start of the construction up to testing of the installations.

2. Drawing requirements in 50cm x 90cm paper duly signed by PME or ChE or both. a. Vicinity Map of the plant; and b. Plant Machinery Layout showing the connection of the APSE to the APCF with complete specifications. 3. Locational Clearance from the Metropolitan Manila Development Authority (MMDA) or Housing and Land Use Regulatory Board (HLRB); 4. Certificate of Registration from the Securities and Exchange Commission (SEC) or Department of Trade and Industry; and 5. Environmental Compliance Certificate (ECC) or Certificate of Non-Coverage (CNC) for the project.

LLDA CLEARANCE APPLICATION


1. Application form, duly accomplished, signed, and notarized. 2. Environmental Compliance Certificate OR Certificate of Non-Coverage, whichever is applicable; 3. SEC-approved Articles of Incorporation (for corporation) OR Articles of Cooperative duly approved by CDA (for cooperative) OR valid Certificate of Business Registration from the Department of Trade and Industry (for single proprietorship); 4. Locational Clearance, if available; 5. Environmental Impact Assessment, Initial Environmental Examination, OR Project Description/Engineers Report, whichever is applicable; 6. Plans (site development plan, vicinity plan, and drainage/sewer plan); 7. Shoreland Lease Agreement, if the site or project area is below 12.5 m and untitled.

PCO ACCREDITATION
1. Official designation from the Chief Executive Officer/President/Owner/Plant Manager incorporating his/her duties and responsibilities as Companys Pollution Control Officer (PCO). 2. Certificate of employment signed by the Human Resources Officer and certified correct by the CEO/President/Owner/Plant Manager. The certificate of employment should state the ff:

Educational qualification (state if license) Current position in the company (state if supervisory) Years of stay in the company 3. Certificate of Attendance in a 40-hour training/seminar/workshop issued LLDA within the last three (3) years. 4. Resume/CV.

DISCHARGE PERMIT
1. Application form, duly accomplished, signed, and notarized. 2. LLDA Clearance. For LLDA Clearance exempted projects, Government issued documents as proof of existence (e.g. BIR, LGU Certificate of Occupancy, SSS Mechanical Permit, etc. dated before 1976) 3. Water bills/reading registered in the name of the applicant

If the source of water is from supplier/provider, water bills/reading for the last three 3) months; If the source of water is from groundwater/deepwell, water permit or water permit application from NWRB; If the water source is from surface water, water permit or water permit application from LLDA. 4. Water Treatment Facility layout, with complete specifications of the treatment process duly signed and sealed by a licensed structural/civil or sanitary engineer who prepared the report and countersigned by the owner 5. Engineers Report/Technical Report

The declaration of product capacity stating the quantity or volume and the generic name(s) of product, the nature and character of applicants waste, its chemical composition, total daily volume of discharge of raw waste, treatment process, and estimate treatment efficiency; and The total daily volume of water composition and discharge of finally treated waste or effluent

6. For establishments discharging on a by-batch mode, schedule of discharge in tables or report (if schedule is not fixed, a projected schedule of at least a month shall be attached).
