1. Full Circle Solutions Group LLC, ("Company") is a telecommunications carrier subject to the requirements set forth in Section !."##$ of the Fe%eral Communications Commission&s ('FCC&s() rules. Company has establishe% policies an% proce%ures to satisfy compliance )ith the FCC&s rules pertainin* to use, %isclosure an% access to customer proprietary net)or+ information ('C,-.() set forth in sections !."#1 et. seq. ". /he Company reco*ni0es that C,-. inclu%es information that is personal an% in%i1i%ually i%entifiable, an% that pri1acy concerns ha1e le% Con*ress an% the FCC to impose restrictions upon its use an% %isclosure, an% upon the pro1ision of access to it by in%i1i%uals or entities insi%e an% outsi%e the Company. 2. /he Company has %esi*nate% a C,-. Compliance 3fficer )ho is responsible for4 (1) communicatin* )ith the Company&s attorneys an%5or consultants re*ar%in* C,-. responsibilities, requirements an% restrictions6 (") super1isin* the trainin* of Company employees an% a*ents )ho use or ha1e access to C,-.6 (2) super1isin* the use, %isclosure, %istribution or access to the Company&s C,-. by in%epen%ent contractors an% joint 1enture partners6 (!) maintainin* recor%s re*ar%in* the use of C,-. in mar+etin* campai*ns6 an% (7) recei1in*, re1ie)in* an% resol1in* questions or issues re*ar%in* use, %isclosure, %istribution or pro1ision of access to C,-.. !. @. .nformation protecte% by Company inclu%es information that relates to the quantity, technical confi*uration, type, %estination, location an% amount of use of a telecommunications ser1ice subscribe% to by a customer an% ma%e a1ailable to Company by the customer solely by 1irtue of the carrier= customer relationship. 8lso protecte% is information containe% in the bills pertainin* to telephone e<chan*e ser1ice or telephone toll ser1ice recei1e% by a customer. $. Company %oes not use, %isclose or permit access to C,-. to i%entify or trac+ customers that call competin* ser1ice pro1i%ers. 1#. Company has establishe% a system by )hich they can %etermine )hether a customer has appro1e% or %isappro1e% of Company&s release or use of C,-. prior to that information bein* use% or release%. 11. Company personnel are traine% as to )hen they are an% are not authori0e% to release or use C,-., an% 1iolation of these rules )ill subject personnel to e<press %isciplinary action (inclu%in* reme%ial trainin*, repriman%s, unfa1orable performance re1ie)s, probation, an% termination), %epen%in* upon the circumstances of the 1iolation (inclu%in* the se1erity of the 1iolation, )hether the 1iolation )as a first time or repeat 1iolation, )hether appropriate *ui%ance )as sou*ht or recei1e% from the C,-. Compliance 3fficer, an% the e<tent to )hich the 1iolation )as or )as not %eliberate or malicious). 1". .f an% )hen customer appro1al to use, %isclose, or permit access to customer C,-. is %esire%, Company obtains such customer appro1al throu*h )ritten or oral metho%s (ho)e1er, )e only utili0e the oral authori0ation to obtain limite%, one=time use of C,-. for inboun% an% outboun% customer telephone contacts, an% such C,-. authority, if *rante%, lasts only for the %uration of that specific call). Company honors a customer&s appro1al or %isappro1al until the customer re1o+es or limits such appro1al or %isappro1al. 12. Company has establishe% a proce%ure )hereby all sales personnel must obtain super1isory appro1al of any propose% outboun% mar+etin* request for customer appro1al of the use of C,-. an% recor%s reflectin* carrier compliance )ith the Commission 9ules are maintaine% for a minimum of one year. 1!. ,rior to any solicitation for customer appro1al, Company pro1i%es notification to customers of their ri*ht to restrict use of, or %isclosure of, an% access to the customer&s C,-.. 9ecor%s of these notifications are maintaine% for a perio% of at least one year. 17. Company&s notifications pro1i%e information sufficient to enable our customers to ma+e informe% %ecisions as to )hether to permit the use or %isclosure of, or access to, their C,-.. Company&s notifications %o4 (1) contain a statement that the customer has a ri*ht, an% Company has a %uty un%er fe%eral la), to protect the confi%entiality of C,-.6 (") specify the types of information that constitute C,-. an% the specific entities that )ill recei1e the C,-.6 (2) %escribe the purposes for )hich the C,-. may be use%6 an% (!) inform the customer of the ri*ht to %isappro1e those uses an% %eny or )ith%ra) access to or use of C,-. at any time. 1 . Company&s notifications inform the customer that any appro1al or %enial of appro1al for the use of C,-. outsi%e of the ser1ice to )hich the customer alrea%y subscribes is 1ali% until the customer affirmati1ely re1o+es or limits such appro1al or %enial. 1>. Company a%1ises its customers of the precise steps the customer must ta+e in or%er to *rant or %eny access to C,-., an% that %enial of appro1al )ill not affect the pro1ision of any ser1ices to )hich the customer subscribes.

1@. Company maintains a recor% of its sales an% mar+etin* campai*ns that use customer&s C,-.. Further, a recor% of all instances )here C,-. )as %isclose% or pro1i%e% to thir% parties or )here thir% parties )ere allo)e% access to C,-. is maintaine% by Company. /hese recor%s reflect a %escription of the campai*ns, the specific C,-. use% in the campai*n an% )hat pro%ucts or ser1ices )ere offere% as part of the campai*n. /hese recor%s are retaine% for a minimum of one year. 1$. Company maintains appropriate paper an%5or electronic recor%s that allo) its employees, in%epen%ent contractors an% joint 1enture partners to clearly establish the status of each customer&s 3ut=out an%5or 3pt=.n appro1als (if any) prior to use of the customer&s C,-.. /hese recor%s inclu%e4 (i) the %ate(s) of any an% all of the customer&s %eeme% 3pt=out appro1als an%5or 3pt=in appro1als, to*ether )ith the %ates of any mo%ifications or re1ocations of such appro1als6 an% (ii) the type(s) of C,-. use, access, %isclosure an%5or %istribution appro1e% by the customer. "#. ;efore a customer&s C,-. can be use% in an out=boun% mar+etin* acti1ity or campai*n, the Company&s recor%s must be chec+e% to %etermine the status of the customer&s C,-. appro1al. Company employees, in%epen%ent contractors an% joint 1enture partners are require% to notify the C,-. Compliance 3fficer of any access, accuracy or security problems they encounter )ith respect to these recor%s. .f ne), a%%itional or e<ten%e% appro1als are necessary, the C,-. Compliance 3fficer )ill %etermine )hether the Company&s '3pt=3ut C,-. -otice( or '3pt=.n C,-. -otice( must be use% )ith respect to 1arious propose% out=boun% mar+etin* acti1ities. "1. .f a breach of C,-. occurs, Company )ill pro1i%e electronic notification of the breach to the :.S. Secret Ser1ice an% the F;. as soon as practicable an% in no e1ent more than se1en (>) %ays after reasonable %etermination of the breach. Company )ill also notify customer )ithin se1en (>) more %ays unless there is a ris+ of imme%iate an% irreparable harm to the customer in )hich case Company )ill notify the customer imme%iately after consultin* )ith an% in cooperation )ith the rele1ant in1esti*ati1e a*ency. Company )ill +eep recor%s or %isco1ere% breaches for at least t)o (") years. "". .f Company %etermines that the opt=out mechanisms %i% not )or+ properly, Company )ill notify the Commission by )ay of )ritten notification )ithin fi1e (7) business %ays of sai% %etermination. /he notice )ill be in the form of a letter an% inclu%e, amon* other thin*s, a %escription of the opt=out mechanism use%, the problem e<perience%, the reme%y propose% an% )hen it )ill be or )as implemente%. .n a%%ition, the letter )ill inform the Commission as to )hether or not the rele1ant state commission has been notifie%, )hether it has ta+en any action, a copy of the notice pro1i%e% to the customer an% contact information. /his notice )ill be submitte% e1en if Company has other metho%s in place by )hich consumers may opt=out.
