Dear Dr. Moini, Although I have no intentions of seeing another dentist, I have been back forth between you and Dr. Marriano for cleanings over the past 5 years, primarily utilizing your office. Unfortunately I was involved in a car accident last year that nearly messed up my back and neck and have been out of work for over a year now for the first time in 33 years. I have had to apply for temp as well as permanent disability benefits and have a hearing scheduled for Oct 30th at 9am at 1441 Main St, Springfield, Suite #450, Room 1, where a hearings officer will determine if the extent of all of the injuries I have ever suffered will allow me to be eligible for Federal disability insurance benefits. As you can imagine, it is not a lot of money but if approved, will provide me with 18 months of back pay as well as the average amount I earned over the past 15 years on a monthly basis. I worked for my brothers oil company for 2.5 years up until about 18 months ago as the pain I was and continue to endure on a daily basis. I have had to make a list of all injuries I have ever been involved with even though my back and neck are the primary sources of daily problems. As you know, I was involved in a very serious (almost fatal) auto accident back in 1980 which allowed you to perform your magic on several occasions and has allowed me to enjoy an almost normal lifestyle due to your expertise in your field of work. Unfortunately I do still suffer from migraine headaches as well as moderate facial pain at times, seemingly more often the older I have become. During the accident my face hit the steering wheel as you probably know and I sustained extensive injuries including a fractured skull, a broken eye socket, broken jaw, broken nose, 2 broken cheek bones, 3 lost teeth, and a tracheotomy was performed on site due to the facial injuries. There were several more but fortunately the doctors who performed the surgeries and operations that followed. I am hoping that some of these injuries that occurred at that time will have a positive impact on the outcome of my case even though it is not back related, it shows a history of facial damage which does still cause me at times severe pain and discomfort asd well as the traumatic procedures which had to be performed in order for me not to a Dental handicap or eat my food out of a blender like some people typically older than myself who also sustained similar injuries but didnt have the care and expertise of a professional such as yourself to correct many of the problems. Some I have just had to endure such as a deveated septum which bothers me greatyly to this day and many surgeries, procedures and other painful operations. They have requested copies of all of my records as well as, if you could be kind enough, a simple note stating that an accident of this magnitude could very well cause a lifetime of inconsistent pain and discomfort as well as possible issues that could prevent me from doing my applied profession of both short distance and long distance truck driving, as well as what I managed to do for the past 2.5 years prior my descision about 18 months ago to stop as it is a safety sensitive position and deemed too risky while under the influence of the medication I was prescribed. This short note would make all the difference in the decision making process of the hearings officer and would be greatly appreciated by me. If there is anything I can do to return this favor including payment for your time and copies of paperwork, I would be more than happy to compensate you at whatever price you deem appropriate.
