Application Guide
for 5D Programs with Scholarship

I. Admission System
3. Eligibility Requirements
Those who meet one of the following conditions and passed the entrance examination shall be admitted student in the 5D Program, except for those dispatched from companies, Japanese Government (Monbukagakusho) Scholarship students, and foreign government scholarship students. (1) Those who have graduated from a university in Japan or will graduate by the end of previous month of intended admission day. (2) Those who have been granted bachelors degree by the National Institution for Academic Degrees and University Evaluation NIAD-UE or will be granted by the end of previous month of intended admission day. (3) Those who have completed a 16-year course of school education system in countries other than Japan or will completed by the end of previous month of intended admission day. (4) Those who have completed the courses at a foreign-affiliated university in Japan that is assessed to have a universitys education course under the school education system of its country (Its completion must be regarded as the completion of a 16-year course of school education system in its country.) and specifically designated by the Minister of Education, Culture, Sports, Science and Technology or will complete by the end of previous month of intended admission day. (5) Those who have completed a 16-year course of education in a foreign country by completing the correspondence course of an overseas educational institution while residing in Japan or will complete by the end of previous month of intended admission day. (6) Those who have completed, on or after the date designated by the Minister of Education, Culture, Sports, Science and Technology, the specialized course specifically designated -1-
by the Minister of Education, Culture, Sports, Science and Technology at a vocational school, whose minimum period required for graduation is four years or longer, and which also satisfies other condition specified by the Minister of Education, Culture, Sports, Science and Technology or will complete by the end of previous month of intended admission day. (7) Those designated by the Minister of Education, Culture, Sports, Science and Technology (those who graduated from a university and others not under the jurisdiction of the Ministry of Education, Culture, Sports, Science, and Technology).
4. Application
We recommend all the applicants to contact a JAIST faculty advisor by whom they wish to be supervised and also to get his/her acceptance before submitting their application. (1) Application Procedure Please be sure that all required documents are enclosed in the envelope specified by JAIST and bring or send the materials by registered express mail to JAIST.

If you graduated from a foreign university, please submit the official documents in English. If you are not able to submit them in English, please attach English translations or Japanese translations. If you transferred to a university, or attended a special study of junior college or technical college, please submit the transcript of the previous school (university, junior college or regular course of technical college) together. (Form 3) (Form 4) (Form 5) The short essay should be written in about 1,000 Japanese characters horizontally. When typed with a computer, it should be attached inside of the frame on the specified form (if impossible to write in Japanese, type in about 600 words in English.) Subject: Study and research plan in JAIST The subject should be the study and research plan you want to pursue, and the applicant should describe the research background, purposes, contents, and methods in your own words. If you would like to add a further explanation, you can attach support
documentation. Proof of English Language You must submit a copy of one of the following score Proficiency report from a test taken within the last two years. TOEFL PBT: 550 or more TOEIC 730 or more IELTS (Academic: 6.0 or more)
You must submit a copy of unexpired certificate. Applicants to the School of Knowledge Science: GRE General Test Score Applicants to the School of Information Science: GRE Subject Tests Score in Mathematics or Computer Science Applicants to the School of Materials Science: GRE Subject Tests Score in PhysicChemistry, or Biochemistry, Cell and Molecular Biology
You must submit a copy of some form of Identification. It must be written in English and identify the applicant. If you are not able to submit it in English, please attach an English translation or Japanese translation.
Fill out the specified form (Regarding Address Labels, fill out all three labels) -3-
<For residents in Japan> Please pay the 30,000 entrance examination fee through a bank (electronic) transfer. 1. Fill out the specified form and bring it to a bank counter (except the Japan Post Bank). 2. After paying the fee, the applicant should make sure that the seal/stamp of the bank is on the remittance stub of the certificate of entrance examination fee payment. 3. Glue the certificate of entrance examination fee payment to the specified area on the application form. The applicants have to pay the handling charge. If you pay it at the headquarters or branch offices of the banks designated by JAIST, the charge will be lower than the other banks. <For residents overseas> Please ask the Admissions Section how to pay the fee by e-mail at nyushi@jaist.ac.jp.
(b) The application document should be submitted if applicable. (4)Delivery of Admission Card After the application documents are accepted, we will send an admission card to the applicant
after each deadline of applications.
If it does not arrive 1 week before the selection date, contact the Admissions Section.

5. Selection
(1) Selection for residents in Japan Selection Period and others Selections will be conducted at JAIST in Nomi, Ishikawa Prefecture, on a day during the period below. Admission

All the applicants will be notified of the interview schedule when they receive their admission cards. Selection Method Oral examination will be conducted as follows.
School of Knowledge Science 50 minutespresentation time : 15 minutes
1) Presentation and questions about the short essay submitted at the application: The applicant can use a projector with real-object projection function (both regular paper and OHP film are supported) and a white board, if he/she wants to use it for his/her presentation, bring necessary materials at the interview. 2) Oral examination based on keywords from the applicants speciality and the research area in which the applicant has a deep interest.
School of Information Science 60 minutespresentation time : 15 minutes 1) Presentation and questions about the short essay submitted at the
application: The applicant can use a projector with real-object projection function (both regular paper and OHP film are supported) and a white board, if he/she wants to use it for his/her presentation, bring necessary materials at the interview. 2) Oral examination based on keywords in the applicants field of interest.
3) Oral examination to evaluate the applicants ability in the fields of mathematics and information science. School of Materials Science 45 minutespresentation time : 15 minutes
1) Presentation and questions about the short essay submitted at the application: The applicant can use a projector with real-object projection function (both regular paper and OHP film are supported) and a white board, if he/she wants to use it for his/her presentation, bring necessary materials at the interview. 2) Oral examination to evaluate the applicants ability in a field chosen by the
applicant: physics, chemistry, or biology.

(3) Admission Decision We will determine the successful applicants according to the results of the oral
examinations (a review of the short essay for those residing overseas) mentioned above, the certificate of transcript, and other application documents.

(2) Expenses required for entrance (amount as of 2011) Entrance fee: 282,000 Tuition fee: 267,900 (for half-year term
* The entrance fee paid to JAIST is non-refundable. * When the tuition fee is revised after enrollment at JAIST, the new tuition fee system shall be applied upon the revision. * Both the entrance fee and tuition fee are current rates and are subject to change for admission in April, 2012.
7. Scholarship
The successful applicants are entitled to obtain the following scholarship in accordance with the rules of JAIST. Amount of scholarship 1,200,000 yen/year (100,000 yen/month) for the 2-year masters program 1,800,000 yen/year (150,000 yen/month) for the 3-year doctoral program
8. Others
If an unsuccessful applicant for 5D program WITH scholarship passes the general selection, the applicant can be selected as a regular masters program student WITHOUT scholarship. Please check the appropriate box and sign the signature in the space provided on the application form.

II. Others
research activities as Research Assistant to provide financial support and to cultivate their research capabilities, with the goal of effectively conducting research activities and improving research system. (3) Student Housing Student Housing has 533 Single rooms (include 85 women rooms), 33 Couple rooms, and 33 Family rooms. Some of the Couple rooms and Family rooms can be used as shared rooms with another student. Besides, JAIST HOUSE which is for both students and foreign researchers, has 30 Single rooms. The procedure of Student Housing will be informed after the announcement of results. All new graduate students who wish to live in Student Housing will be allowed to move in. However, the type of room may not always meet the applicants preferences. (4) Welfare Facilities For students, we have the student center, health care center, exercise room, tennis courts, refresh room, and so on.
